Sfd Indications And Technique: When And How To Use Sdf To Manage Caries Lesions, Margherita Fontana Dds, Phd
The Journal of the Michigan Dental Association
The aim of this article is to review indications for 38% SDF and the appropriate technique for use. Evidence-based recommendations by the American Dental Association and umbrella reviews of systematic reviews are summarized to provide indications for SDF use in primary and permanent teeth, including sound surfaces and those with non-cavitated and cavitated caries lesions. High-quality evidence supports that SDF is an effective treatment alternative for managing active cavitated coronal and root caries lesions and preventing root caries lesions. Dentists can help by incorporating SDF as an option into their caries prevention and management armamentarium and by helping to train …

There is no shortage of therapeutic modalities and interventions at the therapist’s disposal. Psychotherapy, because it claims to treat the individual, is necessarily multifaceted and complex. The dimension of language which structures much of our experience should be considered as a starting point for a psychotherapeutic technique. French psychoanalyst Jacques Lacan developed a theory and technique of the talking cure beginning from this assumption: that language structures our experience. In the early 1950s he sought to recover a linguistic Freud: to re-center psychoanalytic psychotherapy on language itself. In this paper, I sketch the theoretical reasons for this turn in Lacan …

Objectives To evaluate the flapless thyroidectomy technique in reducing postoperative seroma. Background The incidence of seroma after thyroidectomy has been reported to be between 1.3 and 7%. The history of thyroid surgery reflects evolution of surgery itself, but shifting the focus toward less invasive techniques has resulted in the emergence of minimally invasive approaches for the thyroid compartment. Background: The aim of our research was to determine the contest effectiveness of Japanese judo competitors and to state the quantitative an qualitative indices of the tactical-technical preparation (PTT). The values of these indices may be used for controlling the process of special preparation. Materials and methods: Medal classification of the national teams from all Olympic Games and World Championships, and contest effectiveness of the national teams during 2007–2010 were determined. Background: The aim of the study was to determine differences in the efficiency of executing technical actions by female volleyball players taking into account their positions on the court. Material/Methods: The research material comprised results of the efficiency of female volleyball players’ technical actions (n = 98) participating in the 2015 Women’s European Volleyball Championship. Hallux rigidus is a common cause of foot pain involving the hallux; however, the etiology is yet to be elucidated. There are many different treatments that have been described, including cheilectomy. Taking too much bone can cause joint instability and altered joint mechanics, whereas taking too little bone may not relieve pain. We present a reliable, safe, and reproducible technique for cheilectomy in the hallux rigidus.

Title: Insulin Injection Re-education for Improved Glycemic Control
Background: Patients on insulin therapy often continue to show suboptimal glycemic control. Data from adult patients with T2DM have demonstrated that 56.1% had poor control, despite adherence to treatment, oral and injectable. Poor glycemic control stems from many sources, including poor self-efficacy regarding insulin dosage adjustment, inaccurate insulin dosing, expired insulin, lipohypertrophy of the injection site and technique, and equipment issues.
Purpose: This project aimed to implement and assess the efficacy of re-education in the insulin injection technique for improved glycemic control.

The purpose of this study was to determine the intra- and inter-rater reliability of scoring procedures used in the newly developed Basketball Jump Shooting Accuracy Test (BJSAT) and assess for floor and ceiling effects in test performance. Thirty-one semi-professional basketball athletes completed four trials of the BJSAT. The BJSAT contains one jump shot at eight different locations, equally distributed across two- and three-point shots. Intra-rater reliability was determined by assessing the level of agreement between scores live in-person and watching captured video by the same assessor. Introduction. Gastrointestinal stromal tumors (GISTs) represent a rare type of gastrointestinal neoplasms. Resection with negative margins has been established as a mainstay treatment, but laparoscopic resections are still open to debate. Material and method. This retrospective study was conducted at a single institution, with data collected over 2 years (01.01.2017-01.01.2019). The variables examined were age, tumor location with regard to the gastric wall, the results of the intraoperative endoscopy, intraoperative and postoperative complications, the surgical technique, and histopathological reports. Pediatric neuroendovascular procedures are increasingly performed as several studies have shown the safety of these interventions. In the literature, an upper limit of 4 French catheter size is conventionally used for femoral access in neuroangiography of very young infants. However, this constraint in luminal size may not allow for more complex procedures. We present the previously unreported use of a radial 5 French slender catheter with ultrathin walls for femoral access for aneurysm embolization in a 3-month-old boy presenting with left M2 aneurysm rupture.

The aim of this study was to examine the content validity, construct validity and reliability of the newly developed Basketball Jump Shooting Accuracy Test (BJSAT). Basketball athletes from different playing levels (State Basketball League [SBL], n = 30, age: 22.7 ± 6.1 yr; SBL Division I, n = 11, age: 20.6 ± 2.1 yr) completed four separate trials of the BJSAT with each trial consisting of shot attempts from two- and three-point distances at pre-determined court locations. Meticulous attention to wound closure in posterior lumbar spine surgery is an important principle in reducing surgical site infections. We detail standardized wound closure used for posterior lumbar spine surgery at a tertiary care referral center and illustrate this as a step-by-step cadaveric dissection. The lumbar spine of a cadaveric specimen (male, 73 years at death) was used for dissection. Standardizing wound closure in posterior lumbar spine surgery may help limit wound complications and infection. Some key points of our technique, as demonstrated on a cadaveric specimen, include separating fascial compartments, avoiding suture abscesses, and creating a tension-free wound.

Breaststroke pullouts are now permitted to include a dolphin kick. Two variations of the pullout are distinguished by the timing of the dolphin kick within the movement. One variation uses an early-initiation of the dolphin kick (performed before the initiation of the arm pull) while a second variation uses a late-initiation of the dolphin kick (performed at the completion of the arm pull). The open abdomen (OA) is defined as intentional decision to leave the fascial edges of the abdomen un-approximated after laparotomy (laparostomy). The abdominal contents are potentially exposed and therefore must be protected with a temporary coverage, which is referred to as temporal abdominal closure (TAC). OA use remains widely debated with many specific details deserving detailed assessment and clarification. Abstract Loose bone and cement can promote polyethylene wear that may lead to failure in total knee arthroplasty. The authors propose a technique to enhance the debridement of loose bodies by distracting the cemented components with a lamina spreader following standard debridement with pulsatile lavage. Cement and bone/soft tissue debris was recovered from 51 consecutive primary total knee arthroplasty patients. This technique facilitated the removal of hidden debris in 80% of patients (41/51). On average, 2.6 fragments measuring 158 mm3 were removed per patient. The authors advocate that distraction with a lamina spreader be used to reduce potential sources of …

Triad Visible Light Cure (VLC) Denture Base Material, a light-polymerizing resin material, is commonly used by clinicians to make trial bases during the fabrication of dentures and in planning the prosthetic and surgical rehabilitation of edentulous patients. A well-known drawback to this material, however, is its tendency to distort during the light-polymerization process. This distortion tends to occur most often towards the posterior aspect of the palate, and can negatively impact the bases ability to be retained through suction to the edentulous arch. The pull from the knee is a weightlifting movement derivative that can be used in the teaching progression of the clean and snatch exercises. This exercise emphasizes positional strength during the transition phase and the triple extension of the hip, knee, and ankle joints that is characteristic of weightlifting movements.

After performing open reduction and internal fixation (ORIF) for treating malleolar fractures of the ankle, surgeons typically use plaster splints during postoperative recovery of patients. Use of regional anesthesia during ORIF has been noted as a risk factor for burns in patients using plaster splints, possibly owing to inability to feel pain after undergoing regional block. We describe a successful postoperative splinting technique used for preventing thermal injuries in this patient population. Syndactyly is the most frequently occurring congenital malformation of the limbs. Although common, few studies have reported treating syndactyly using Z-plasty with and without full-thickness grafting. We present a 2-year-old boy who presented to our clinic with ectrodactyly of the feet and simple complete syndactyly of two fingers on each hand. After ectrodactyly reconstruction, we used the excess foot skin to cover finger defects. The patient recovered well postoperatively, with matching skin and no trouble with hair growth. Use of a spare-parts technique can help avoid complications associated with harvesting grafts from other donor sites. The results reinforce the importance …

Fractures of the proximal humerus are common in older patients, and the incidence of these fractures in the United States is expected to notably increase with the aging population. Nonoperative procedures have been preferred in elderly patients with stable fracture patterns to avoid complications associated with osteoporotic bone. However, more complex and unstable fracture patterns often necessitate operative techniques to allow for more anatomical healing. A large percentage of swimmers report shoulder pain during their swimming career. Shoulder pain in swimmers has been attributed to duration of swim practice, total yardage, and break down in stroke technique. Rehabilitation programs are generally land-based and cannot adequately address the intricacies of the swimming strokes. Return to swimming protocols (RTSP) that address progression of yardage are scarce, yet needed. The purpose of this clinical commentary is to familiarize the clinician with the culture and vernacular of swimming, and to provide a suggested yardage based RTSP for high school and collegiate level swimmers.

No study has taken an updated and comprehensive review of Donnel Stern’s writings. An investigation of his philosophical assumptions, locating Stern’s work socioculturally and historically, along with an elucidation of Stern’s background in traditional psychoanalytic literature and clinical practice, brings out the meanings and enigmas present in his theories of dissociation, enactment, unformulated experience, multiple self-states, and reflection. Stern has offered one of the best-integrated theoretical models in relational psychoanalytic theory. Background: Obstructive sleep apnoea is a common condition that is unfortunately associated with a high rate of patient non-compliance regarding device use. Newer surgical interventions have focused on procedures at the palate level, using variants of palatoplasty and transpalatal advancement. However, the extent of tongue reduction surgery required remains controversial. The authors propose an in-between variant that combines midline glossectomy resection (with minimal mucosal sacrifice) and lateral coblation tongue channelling. Objective: Investigation of relative bias in diet history measurement during dietary intervention trials. Design: Retrospective analysis of human dietary data from two randomised controlled trials examining modified fat diets in the prevention and treatment of type II diabetes mellitus. Setting: Wollongong, Australia. Subjects: Thirty-five overweight, otherwise healthy subjects in trial 1 and 56 subjects with diabetes in trial 2. Interventions: Diet history interviews and three-day weighed food records administered at one-month intervals in trial 1 and three-month intervals in trial 2.
