Celli and colleagues have attempted to revise the definition and propose a new severity classification, based on measurable clinical and laboratory variables instead. This file is an exchange of letters, e-mails, and documents between Norman Miller and Duncan MacDonald, MD, including a four-volume collection of MacDonald’s writings, over a 30-year period, all on witchcraft, some 600 pages extracted from the original 1100. As such, the following material is unfinished, presenting sketches of ideas, concepts, and arguments.
Duncan MacDonald served as a physician in Zambia and Kenya, including a period as a "Flying Doctor". He later served as a provincial psychiatrist in Cornwall, UK. Objective To develop and evaluate adjustment factors to convert parent-reported time in bed to an estimate of child sleep time consistent with objective measurement. Methods A community sample of 217 children aged 4–9 years (mean age = 6.6 years) wore actigraph wristwatches to objectively measure sleep for 7 days while parents completed reports of child sleep each night. After examining the moderators of the discrepancy between parent reports and actigraphy, 3 adjustment factors were evaluated. Results Parent report of child sleep overestimated nightly sleep duration by ~24 min per night relative to actigraphy. Child age, gender, and sleep quality all …

BACKGROUND: Referral and consultation letters ferry patients among providers, negotiating co-operative care. Our study examined how "relevance" is signalled and decoded in these letters, from the perspective of both experts and trainees in three clinical specialties. METHODS: 104 letters were collected from 16 physicians representing family medicine, psychiatry and surgery. Interviews were conducted with 14 of these physicians and 13 residents from the three specialties. All documents and transcripts were analysed for emergent themes. RESULTS: Six rhetorical factors influenced expert physicians' decisions about what material is relevant: educational, professional, audience, system-institutional, medical-legal, and evaluative. General correspondence regarding Western Kentucky University. Incoming letters are mainly addressed to Henry Hardin Cherry. Responses are made by Cherry and occasionally by faculty and staff. The president's secretary Mattie McLean is the writer for some of the letters signed by Cherry.
Some of Cherry's personal papers are also found in this series most notably those pertaining to his political aspirations and two candidacies for the Kentucky governorship. Copies of outgoing correspondence begin to appear in Box 1, Folder 50.

This is a letter that accompanied a gift (tube with document inside that was printed by the original Gutenberg printing press) from John Post to Armin. John was a ret. Col. in medical logistics (at the US Dept of State) that helped distribute excess medical and hospital supplies to our AIHA partners in Kazakhstan, Azerbiajan and actually helped with shipment to Gaza as well. Referral and consultation letters are written to enable the exchange of patient information and facilitate the trajectory of patients through the healthcare system. Yet, these letters, written about yet apart from patients, also sustain and constrain professional relationships and influence attitudes towards patients. We analysed 35 optometry referral letters and 35 corresponding ophthalmology consultation letters for reported 'patient voice' coded as 'experience' or 'agenda' and we interviewed 15 letter writers (eight optometry students, six optometrists, and one community ophthalmologist). There were 80 instances of reported 'patient voice' in 35 letters. The majority (68%) of the instances occurred in referral letters, …

A letter to Bolat K. Nurgaliyev the Kazakhstan Ambassador to the US regarding an upcoming meeting we helped orchestrate that included AFRRI. Also an interesting reference to Mr. James Giffin regarding the committee for reorganization of the health care system in the Republic of Kazakhstan that I was asked to comment on. This is a communication from me (Armin) to Boris Gusev a leader of the Kazakhstan research on the effects of the nuclear testing program. Boris is Kazakh and had much data kept for themselves as opposed to the data Russia collected and took to Moscow after the breakup of the Soviet Union. This letter was an attempt to bring them into the efforts of those working on Chernobyl. This an early note to Dennis Sokol who Armin met with at the request of Ambassador Robert Strauss during his first trip to Russia (the meeting with the ambassador was facilitated also by Billy Goldberg). Dennis was a US entrepreneur who was establishing Western style clinics in the former USSR and the ambassador thought we could help especially based on his knowledge of the BCM/TMH affiliation with the Admiral Bristol Hospital that became the American Hospital of Istanbul.
