Abstract
Adolescents with special health care needs encounter barriers and challenges during transition to adult health care that have the potential to impact health outcomes and satisfaction with the experience. The use of transitional care services can provide resources and support throughout the transition process to the adolescent and their parent/caregiver. This descriptive survey study was conducted to evaluate adolescent and/or parent/caregiver satisfaction with the transitional care services provided by an urban, academic, pediatric health care facility. Nineteen of 39 (49%) eligible participants completed the survey. Findings indicate that adolescents and their parent/caregiver were satisfied with the resources and support …

The purpose of this study is to identify techniques that could be developed into a resource for counselors and other professionals who assist college students in dealing effectively with adversity. In a series of four surveys, 200 people were asked to participate. These included professional counselors and ministers both on and off campus, as well as directors in Student Affairs, such as those in student support services and student life. To accomplish this, questionnaires were developed to identify and allow for tabulation of the counseling techniques most commonly used for students dealing with adversity by those working with college students. …

The transition of new graduate nurses from their roles as students to professional nurses is challenging. New graduate nurses have to complete cursory orientation with high performance expectations, increased level of accountability and meet the complex needs of the patients, leading to high turnover rates within the first year of practice.
To address this problem, nurse residency programs have been implemented to support new graduates with the transition into professional nursing. Within the disabilities community, individuals with an Autism Spectrum Disorder (ASD) diagnosis consistently have some of the most detrimental employment related statistics. To address this issue, this study evaluated video modeling, an evidence-based procedure, in teaching professional emailing skills to individuals diagnosed with ASD when applying for a job. Results of the study support video modeling research as an effective teaching tool. Significant skill acquisition, in composing a professional email, occurred across all participants. Seventy-five percent of participants concluded the study at mastery level. Implications of this study are immediately impactful on the ASD community as a new evidence-based transition …

The objective of this investigation is to gain information regarding the transition of adults from a tube feeding to an oral feeding in long-term care facilities. Twelve surveys were administered to the speech language pathologist population from the UTRGV and UTPA communication sciences and disorders alumni via email invitation. Nationally, inpatient psychiatric settings average a 5% readmission rate for their patients, within one month of discharge. A search for articles published within the past 20 years, uncovered critical themes and led to recommendations for practice from 15 articles. Impacting an individual’s readmission are 1) length of stay at the facility, 2) employment opportunities and support after discharge, 3) social and family support, impacting feelings of exclusion/inclusion (stigma), 4) the extent to which the individual embraces the AA recovery philosophy, 5) the specific characteristics of the immediate geographical location of housing, 6) adherence to medical plans, and 7) the availability …

East Tennessee State University (ETSU) has offered an undergraduate degree in public health for 60 years. Alumni survey data have documented that the majority of the graduates from this program enter the workforce [see accompanying commentary by Wykoff, et al. (1)]. To keep pace with ongoing changes in the workforce, the decision was made to completely review, and, as appropriate, revise and restructure the Bachelor of Science in Public Health (BSPH) curriculum.

Purpose: In 2016, there were 22.5 million refugees worldwide (UNHCR, 2017). California resettled just over 5,000 of those 85,000 (Igielnik & Krogstad, 2017). Background: An increased national and local prevalence of heart failure fostered a review of the evidence to identify best practice interventions focusing on improving self-care and knowledge. Heart failure remains a leading cause of 30-day readmission in the United States and in Madisonville, Kentucky, the site of study. A review of the literature emphasized improving transitions from hospital to home with a multi-dimensional approach. Type 1 diabetes is a long-term diagnosis, the prognosis of which is directly related to the patient's ability to self-manage the disorder. Adolescents are not currently taught how to manage diabetes; instead, parents and educators expect self-management to be more of a learned behavior from their parents. A 2009-2010 National Assessment Survey of youth with special needs such as hemophilia showed that 40% of youth between the ages of 12 and 17 had a transition plan before transitioning to adult care services. The lack of a transition plan caused youth to fall into a fragmented care gap when transition services are inadequate. During the gap, youth with hemophilia increased use of the emergency room and were hospitalized for complications such as frequent joint or muscle bleeding and joint disability. BACKGROUND: Understanding the circumstances of adolescents living with HIV is critical in designing adolescent-friendly services that will facilitate successful transition from pediatric to adult care. This study describes access, utilization and ongoing social support needs among adolescents living with HIV aged 15-17 in transition from pediatric to adult HIV care in Cambodia.
METHODS: A cross-sectional study was conducted among 328 adolescents, randomly selected from 11 antiretroviral therapy (ART) clinics across the country. Descriptive analyses were conducted to summarize their characteristics, access to social support and ongoing support needs among male and female adolescents.
