Abstract

Adolescents with special health care needs encounter barriers and challenges during transition to adult health care that have the potential to impact health outcomes and satisfaction with the experience. The use of transitional care services can provide resources and support throughout the transition process to the adolescent and their parent/caregiver. This descriptive survey study was conducted to evaluate adolescent and/or parent/caregiver satisfaction with the transitional care services provided by an urban, academic, pediatric health care facility. The purpose of this study is to identify techniques that could be developed into a resource for counselors and other professionals who assist college students in dealing effectively with adversity. In a series of four surveys, 200 people were asked to participate. These included professional counselors and ministers both on and off campus, as well as directors in Student Affairs, such as those in student support services and student life. Within the disabilities community, individuals with an Autism Spectrum Disorder (ASD) diagnosis consistently have some of the most detrimental employment related statistics. To address this issue, this study evaluated video modeling, an evidence-based procedure, in teaching professional emailing skills to individuals diagnosed with ASD when applying for a job. Results of the study support video modeling research as an effective teaching tool. Significant skill acquisition, in composing a professional email, occurred across all participants. Nationally, inpatient psychiatric settings average a 5% readmission rate for their patients, within one month of discharge. A search for articles published within the past 20 years, uncovered critical themes and led to recommendations for practice from 15 articles. Impacting an individual’s readmission are 1) length of stay at the facility, 2) employment opportunities and support after discharge, 3) social and family support, impacting feelings of exclusion/inclusion (stigma), 4) the extent to which the individual embraces the AA recovery philosophy, 5) the specific characteristics of the immediate geographical location of housing, 6) adherence to medical plans, and 7) the availability …

Alumni survey data have documented that the majority of the graduates from this program enter the workforce [see accompanying commentary by Wykoff, et al. (1)]. To keep pace with ongoing changes in the workforce, the decision was made to completely review, and, as appropriate, revise and restructure the Bachelor of Science in Public Health (BSPH) curriculum.

Heart failure remains a leading cause of 30-day readmission in the United States and in Madisonville, Kentucky, the site of study. A 2009-2010 National Assessment Survey of youth with special needs such as hemophilia showed that 40% of youth between the ages of 12 and 17 had a transition plan before transitioning to adult care services. The lack of a transition plan caused youth to fall into a fragmented care gap when transition services are inadequate. During the gap, youth with hemophilia increased use of the emergency room and were hospitalized for complications such as frequent joint or muscle bleeding and joint disability. BACKGROUND: Understanding the circumstances of adolescents living with HIV is critical in designing adolescent-friendly services that will facilitate successful transition from pediatric to adult care. This study describes access, utilization and ongoing social support needs among adolescents living with HIV aged 15-17 in transition from pediatric to adult HIV care in Cambodia.

METHODS: A cross-sectional study was conducted among 328 adolescents, randomly selected from 11 antiretroviral therapy (ART) clinics across the country. Descriptive analyses were conducted to summarize their characteristics, access to social support and ongoing support needs among male and female adolescents.
