Abstract Loose bone and cement can promote polyethylene wear that may lead to failure in total knee arthroplasty. The authors propose a technique to enhance the debridement of loose bodies by distracting the cemented components with a lamina spreader following standard debridement with pulsatile lavage. Cement and bone/soft tissue debris was recovered from 51 consecutive primary total knee arthroplasty patients. This technique facilitated the removal of hidden debris in 80% of patients (41/51). On average, 2.6 fragments measuring 158 mm3 were removed per patient. The authors advocate that distraction with a lamina spreader be used to reduce potential sources of …

Vitamin K is known to play an essential role in the coagulation cascade; however, a growing body of research has found that a subtype of this vitamin, vitamin K2 (menaquinone) may have a beneficial effect in osteoporosis, cardiovascular disease, and cancer. This purpose of this article is to provide a comprehensive review of recent literature regarding menaquinone and its role in human health. This review discusses the physiology of menaquinone, its clinical benefits in cardiovascular disease, osteoporosis, and cancer, and how it may interact with certain medications. Estrogen’s biological functions are mediated by estrogen binding to estrogen receptors (ER). Understanding what role both ERα and ERβ have in bone maintenance and formation can contribute to possible treatment of osteoporosis. This study examined osteocyte specific deletion of ERβ in mice. The cross of ERβ-floxed mice with DMP1-8kb-Cre mice provided both experimental knockout mice as well as littermate control mice. At 24 weeks of age the left tibiae of all mice were mechanically loaded five days per week for two weeks to induce bone formation. Osteoarthritis (OA) affects 1 in 10 Canadians and is a leading cause of mobility disability worldwide. This condition is characterized by cartilage degeneration, subchondral bone damage and inflammation of the synovium, resulting in pain and joint failure. No treatments exist to stop the progression of this disease, and its underlying molecular mechanisms remain largely unknown. We previously identified the peroxisome proliferator activated receptor (PPAR) nuclear receptor pathway as altered in OA cartilage. In-vitro studies identified PPARd as a promoter of catabolic activity in chondrocytes, providing the foundation for my overarching hypothesis that PPARd inhibition is protective in OA.

Breast cancer preferentially metastasizes to the lymph node, bone, lung, brain and liver in breast cancer patients. Previous research efforts have focused on identifying factors inherent to breast cancer cells that are responsible for this observed metastatic pattern (termed organ tropism), however much less is known about factors present within specific organs that contribute to this process. This is in part because of a lack of in vitro model systems that accurately recapitulate the organ microenvironment. Fractures range in severity and classification, and are therefore treated differently depending on the type of bone that was broken, the location of the break, and the way in which the specific bone regrows. Objective: The purpose of this study was to explore the relationships between dietary intake of vitamin K, vitamin A, vitamin D, fiber, and bone mineral density in a sample of pre-menarcheal female gymnasts and non-gymnasts in the Syracuse-area. The goal was to observe whether higher intakes of these vitamins were associated with higher bone mineral density as measured by dual-energy X-ray absorptiometry (DXA), and to potentially examine the impact of activity level on these relationships.

Millions of craniofacial bone defects occur annually as a result of trauma, congenital defects, disease, or tooth extraction. When present in the oral cavity, these defects are associated with adverse impacts on speech, mastication, and aesthetics. Thus, there is a clinical need for interventional strategies to restore and preserve alveolar bone mass to improve the success of future treatment options intended to reestablish functionality and aesthetics. We examined the pharmacokinetic properties of vancomycin conjugated to a bone-targeting agent (BT) with high affinity for hydroxyapatite after systemic intravenous administration. The results confirm enhanced persistence of BT-vancomycin in plasma and enhanced accumulation in bone relative to vancomycin. This suggests that BT-vancomycin may be a potential carrier for the systemic targeted delivery of vancomycin in the treatment of bone infections, potentially reducing the reliance on surgical debridement to achieve the desired therapeutic outcome.

Childhood chronic kidney disease (CHD) poses multiple threats to bone accrual; however, the associated fracture risk is not well characterized. This prospective cohort study included 537 CKD in Children (CKiD) participants. Fracture histories were obtained at baseline, at years 1, 3, and 5 through November 1, 2009, and annually thereafter. We used Cox regression analysis of first incident fracture to evaluate potential correlates of fracture risk. At enrollment, median age was 11 years, and 16% of patients reported a prior fracture. End-to-end repair of damaged tendon is not possible in many instances of injury. Staged flexor tendon reconstruction has been proven to be a valuable method of addressing complex flexor tendon injuries when the patient presents late, has failed primary repair, or the injury mandates it (1). Silicone implants have been used for two-stage flexor tendon reconstructions with reports of long-term use as active implants (2,3). Implants have been reported functional approximately one year post-surgery with some still in use up to 25 years after reconstruction (4). Purpose
Asymmetrical loading patterns are commonplace in football sports. Our aim was to examine the influence of training age and limb function on lower-body musculoskeletal morphology.
Methods
Fifty-five elite football athletes were stratified into less experienced ( ≤ 3 yr; n = 27) and more experienced ( > 3 yr; n = 28) groups by training age. All athletes underwent whole-body dual-energy x-ray absorptiometry scans and lower-body peripheral quantitative computed tomography tibial scans on the kicking and support limbs.
Results

Chikungunya virus (CHIKV) is a mosquito-borne alphavirus that circulates predominantly in tropical and subtropical regions. Infection results in severe debilitating polyarthralgia during the acute phase of disease, and reports suggest that chronic arthralgia lasting months to years after the initial infection can occur. More severe and prolonged disease has been associated with pre-existing joint disease, though this has not been experimentally examined. It is known that the vacuolar ATPase has a number of functions related to tumor growth and progression such as involvement in drug resistance, pH regulation, autophagy, and lysosomal acid protease activation, as well as invasion and metastasis. Here we specifically describe the role of ATP6v1c1 in murine and human models of breast cancer. ATP6v1c1 is the dominant isoform of the coordinating subunit (ATP6v1c) involved in the assembly of the vacuolar ATPase complex which plays a key role in cancer growth and progression.
