The Basic Unit Of A Myofascial Trigger Point, Ta-Shen Kuan, Shu-Min Chen, Jo-Tong Chen, Chang-Zern Hong

Rehabilitation Practice and Science

This report summarized recent studies on the basic structures of a myofascial trigger point (MTrP). A MTrP is a hyperirritable spot in a palpable taut band of skeletal muscle fibers. Spot tenderness, recognized pain, and taut band are now demonstrated to be reliable diagnostic criteria of an MTrP. Referred pain (ReP) or local twitch response (LTR) is a confirmatory sign of MTrP. There are multiple loci in an MTrP region. The basic unit of an MTrP is the MTrP locus which contains a sensory component (the sensitive locus) and a motor component (the active locus). A sensitive locus is the …

Validation Of The Clinical Swallowing Evaluation In Stroke Patients, Yi-Nien Lin, Tyng-Guey Wang, Yeun-Chung Chang, Fu-Mei Hsieh, I-Nan Lien

Rehabilitation Practice and Science

Although it has variable reliability, the bedside clinical swallowing evaluation (CSE) of dysphagia has long been accepted as the basic screening tool in the examination of the swallowing function of stroke patients. The purpose of this study was to define the reliability of the CSE by comparing it to the standard videofluoroscopic study of swallowing (VFSS). Thirty six stroke patients within a period of two years were included in this study, and all of them received the CSE and VFSS simultaneously. Three of them had a normal VFSS, whereas 21 (58%) aspirated during the VFSS examination. Ten of the people …

Paraplegia Following Epidural Anesthesia: A Casereport, Yu-Ching Lin, Yen-Ho Wang, Kou-Mou Huang, Tien-Shang Huang

Rehabilitation Practice and Science

This article reports the case of a 36-year-old woman who experienced a sudden sharp pain in her lower back and became paraplegic after epidural anesthesia for a cesarean section. Three months later, the appearance of an ascending sensory level despite motor recovery raised the prssibilite of another lesion causing the new neurologic symptoms. Magnetic resonance imaging revealed arachnoiditis with subarachnoid cysts that severely compressed the thoracic spinal cord. Surgical decompression relieved the neurologic symptoms at first, but the symptoms soon deteriorated due to re-formation of adhesions and probably intrinsic cord damage. Based on our experience, carefully history taking and detailed …

Effects Of An Anterior Ankle-Foot Orthosis On Hemiplegic Gait, Chiung-Ling Chen, Kwok-Tak Yeung, Shin-Han Wu, Ping-Yen Chiang Sep 1998

Effects Of An Anterior Ankle-Foot Orthosis On Hemiplegic Gait, Chiung-Ling Chen, Kwok-Tak Yeung, Shin-Han Wu, Ping-Yen Chiang

Rehabilitation Practice and Science

Ankle-foot orthoses (AFOs) are frequently prescribed for hemiplegia to correct the ankle joint, provide mediolateral stability and improve ambulatory safety.Metal orthoses have gradually been replaced by plastic AFOs because metal orthoses are heavy and have a poor cosmetic appearance. Conventional plastic AFOs are designed in posterior leaf type and fabricated by a lamination or vacuum-forming technique over a positive plaster model of the limb. It takes time to fabricate a plastic AFO, and the effect of correcting for inversion of a hemiplegic foot is not obvious. Wu designed a low-temperature anterior direct molding AFO called an anterior AFO. It is …

Electrophysiologic Study Of The Hand Ulnar - Median Nerves Anastomosis In Taiwanese, Yi-Pin Chiang, Baii-Jia Yang, Wang-Lin Su, Jiunn-Ming Wu Jun 1998

Electrophysiologic Study Of The Hand Ulnar - Median Nerves Anastomosis In Taiwanese, Yi-Pin Chiang, Baii-Jia Yang, Wang-Lin Su, Jiunn-Ming Wu

Rehabilitation Practice and Science

There are several variations of nerve communications between the ulnar and the median nerves in the hand, which are referred as the Riche-Cannieu anastomoses. The anatomic and clinical significances of these anastomoses have not been clarified. Because of these variations, electric stimulation may travel from the ulnar nerve, through the communications, to the median nerve impulse and thus induces the contraction of thenar muscles. When performing electrophysiologicexaminations, additional attention should be paid to the variations to avoid misinterpretations. In this study, one hundred hands of fifty normal persons were examined. The ulnar and the median nerves were stimulated, and the …

Relationship Between Ventilatory Equivalent For Carbon Dioxide And Ejection Fraction Of Left Ventricle, Shun-Ping Cheng, Chen-Liang Chou, Huey-Ling Yu, Tcho-Jen Liu, Tao-Chang Hsu

Rehabilitation Practice and Science

Cardiopulmonary exercise testing is a powerful non-invasive mean for objectively assessing the exercise response of patients with cardiovascular or respiratory disease. The ventilatory equivalent for CO2(V E/V CO2)derived from gas exchange analysis is considered to be an estimate of dead space ventilation and provides an assessment of the efficiency of minute ventilation during exercise. Clark and associates showed that the V E/V CO2 ratio differentiated patients into subgroups in terms of the severity of congestive heart failure by measuring the maximal oxygen consumption and theduration of exercise time. Our study was designed to differentiate patients into subgroups by measuring the …

Exercise Training During Hemodialysis For End-Stage Renal Failure:A Casereport, Wil-Ly Chou, Kuang-Shin Chang, Su-Hwei Long, Mu-Jung Kao, Chen-Nan Go

Rehabilitation Practice and Science

Exercise training, especially aerobic exercise, is beneficial to one's health. This is also true for end-stage renal failure patients. However, exercise programs prescribed on non-dialysis days are not usually carried out by the patients, which can decrease the expected effectiveness of exercise training. We present an uremic patient who first underwent a 3 month non-exercise control period, then immediately followed by another 3 months of exercise training during hemodialysis, with a stationary bike. We found the patient's compliance rate for exercise training was 94% (34/36). His maximal oxygen consumption increased by 37.8%, and he was able to discontinue his anti-hypertensive agents …

The Accessory Deep Peroneal Nerve In Taiwanese:Detected By Electrodiagnostic Study, Wang-Lin Su, Yi-Pin Chiang, Jiunn-Ming Wu, Baii-Jia Yang

Rehabilitation Practice and Science

The accessory deep peroneal nerve is a common anatomical variation in innervating the extensor digitorum brevis (EDB) muscle of the foot. Awareness of this anomaly is important in electrodiagnostic study of peroneal nerve lesions. One hundred healthy Taiwanese, including 50 men and 50 women, were tested for the presence of an accessory deep peroneal nerve by an electrodiagnostic method. The presence of a visible EDB twitch and an amplitude of the evoked muscle action potential of 0.2 mV or greater upon stimulation of the accessory deep peroneal nerve were taken as positive findings. This study showed that the EDB is …

The Antiperspirative Effect Of The Silicon Socket:A Qualitative Study, Ming-Chuan Lin, Hsien-Ching Chiu, I-Nan Lien, Jin-Shin Lai Mar 1998

The Antiperspirative Effect Of The Silicon Socket:A Qualitative Study, Ming-Chuan Lin, Hsien-Ching Chiu, I-Nan Lien, Jin-Shin Lai

Rehabilitation Practice and Science

In clinical prosthetic experience, profuse sweating was a major problem when conventional sockets or inserts were used. However, the newly developed silicon suction socket could prevent that. We doubted that these differences came from the different nature of the silicon, pelite and polyethylene. So using the starch iodine sweating test, the forearm skins of the twenty normal subjects were sealed with a piece of 2cm×2cm×0.3cm silicon, pelite or polyethylene respectively. Surprisingly, not only silicon but also pelite and polyethylene could inhibit sweating. This meant that decreased sweating of the silicon suction socket might not be due to the chemical property …

Distribution Of Sensitive Loci Where Localized Twitch Response Can Be Elicited In Rat Skeletal Muscle, Yu-Chang Chang, Sheng-Fen Kao, Ta-Shen Kuan, Chang-Zern Hong

Rehabilitation Practice and Science

Myofascial trigger point (MTrP) is a quite common painful muscular disorder in clinical practice. When an MTrP is mechanically stimulated (needling or snapping palpation), a local twitch response (LTR) can be elicited, which is one of the characteristics of MTrP. Recent studies on humans and rabbits suggest that an MTrP is at the endplate zone. The purpose of this study is to map the distribution of sensitive loci from which an LTR can be elicited by mechanical stimulation of it in rat skeletal muscle. A total of 39 Sprague-Dawley rats were studied in this experiment. After general anesthesia, the biceps …
