A Survey Study Of The Spinal Cord Injuries And Their Activity Of Daily Living In Tainan Area, Tse-Chieh Hsueh, Yue-Nan Jenp, Jiunn-Ren Su, Yen-Chun Wu

During Feb. and May of 1991, we followed up 110 cases of the spinal cord injuries by questionnaire and telephone in the Tainan area. The patients are composed of 95 males and 15 females, ranging of age from 16 to 78 years with the mean age of 43.2 ± 14.7 years. About three-fifth of the subjects sufferred from their injuries in the recent three years.There are 51 cases of quadriplegia, 19 of the cases are complete. 59 cases of paraplegia also appear, 33 of them are complete too. The purpose of this study is to determine the changes of the latency, duration , amplitude, and area of CMAP related to different wrist angle. (max flexion, flexion 30° , neutral, extension 30° , max extension) The results revealed there is significant changes in area, duartion & muscle length of flexor carpi radialis (P<0.001), but not in latency & amplitude. And it is increasing trend related to increasing wrist angle. (P<0.001) It is concluded that we must notice the pt's wrist angle when we perform the nerve conducation study.

Neurogenic bladder is a major problem for the spinal cord injured patients. A successful bladder training may not only reduce the mobility or mortality rate of these patients, but also improve the quality of their life and psychological outlook. However, it usually takes two to three months of bladder training to reach balanced status. One hundred and fifty nine patients who experienced the first episode of stroke between 15 to 45 year-old is analyzed for possible risk factors. These patients whose diagnosis of stroke were all approved by the computerized tomography, were collected from the National Taiwan University Hospital (NTUH) from 1983 to 1990. They included 79 cases of cerebral hemorrhage (48 males and 31 females) and 80 cases of cerebral infarction (47 males and 33 females). The frequency of male stroke was about 30% higher than female. The Purpose of this study was to compare the stroke patients within day-care center to patients within family-care about the function of activity of daily living. They were selected as twenty groups. Each group had the same hemiplegic side, the same motor stage, and the same score of Barthel Index. They also got the same observation duration. No significant difference at admission were found between the two groups, except that the day-care patient had higher scores in dressing lower body (P=0.04). To investigate the nature of exposure factors, and effectiveness of countermeasures in head injuries in Taiwanese children with head injuries consecutively admitted to Chang Gung Memorial Hospital from Jan. 1985 to Dec. 1989. We analyzed the data by chart review. The analysis confirmed a boy to girl ratio of about 2.3 to 1. The largest number of head injuries was in children under 1 year, 4 to 5 years and more than 15 years of age. This 67 years old, male, DM patient suffered from a falling accident, resulting in traumatic cervical disc herniation with epidural abscess. After discectomy, the patient sustained quadriplegic condition and needed mechanical ventilation due to breathing difficulty. In addition to the usual rehabilitation program, we also used transcutaneous electrical stimulation on this patient's various muscle groups in order to stimulate his muscles and to help his weaning from ventilator. Diurnally fluctuating hereditary progressive dystoria is a very rare disorder. In 1975, Segawa regarded this disorder as an entity, and there are 46 cases reported till 1986 since then. The clinical manifestation of this disease are: normal development, early onset, dyotonia, diurnal change, excellent response to L-dopa. In this report, a 21-year-old female, who had had symptoms onset since 8 years old; became wheel chair bound at 15 years of age; was not diagnosed correctly until 20 years old. The purpose of this study is to quantitatively analyze the effect of reference electrode size on the amplitude, surface area, latency and duration of CMAP during routine NCV studies in median and ulnar nerves. We dilivered supramaximal stimulations over the wrist in the median and ulnar nerve examinations and obtained the data of CMAP parameters from APB and ADQ muscles. Many different types of foot orthotics have been designed clinically. Commercially, insole materials are divided into four classes. In order to survey the epidemiological characteristics and the overall rehabilitation profile of traumatic spinal cord injuries (SCI) in Hualien county, a retrospective study was conducted from January 1987 to December 1990 in four local general hospitals. During this period, 88 traumatic SCI patients were identified. The estimated annual incidence of traumatic SCI in Hualien county was 62.3 per million population. As a result of injury 32 patients were tetraparetic, 29 were tetraplegic, 12 were paraparetic and 15 were paraplegic. Sexual dysfunction with inadequate or poorly sustained erection is a common problem among spinal cord injured patients although reflex erection may be preserved in upper motor neuron lesion and lower motor neuron and psychogenic erection may be preserved in incomplete lesion respectively. From April 1985, to December 1987, 1445 patients were admitted to Chang Gung Memorial Hospital, Keelung under the diagnosis of cerebral vascular accident ( hemorrhagic or infarction type). By random selection, 343 patients were analyzed by the SPSS. Of these 343 patients, 185 had died by the time this study was carried out. Thus the data available for analysis was incomplete for this deceased group. Only the data of those who survivied was analyzed in the results. In order to understand the caring environment of the stroke patients and the utilization, need, demand and congition of the rehabilitation resoures, 225 stroke patients, who admitted to Kaohsiung Chang Gung Memorial Hospital from January 1986 to December 1990, 117 males (52%) and 108 females (48.0%) with the ages from 31 to 89 (mean 63.45 ±10.87) were evaluated by questionaires, telephone and home visit. In this retrospective study, 199 cases of cerebral palsy who lived in South Taiwan and once visited O.P.D of rehabilitation department from July 1986 to Sep. 1991 was collected.The etiology of cerebral palsy were divided into prenatal, perinatal, postnatal factors by time series. The classifications of cerebral palsy were according to type, topography and severity of neuromuscular involvements. Intoxication of lithium carbonate may present as a multiple systematic dysfunction, but central and peripheral nervous involvement is rare and more troublesome. In the past, there was only one case of lithium intoxication with polyneuroparhy reported by Chang et al in Taiwan. We report here an additional 49 year-old man with manic-depressive psychosis. He received lithium carbonate medication treatment for 5 years and developed a severe cerebellar dysfunction and a mild sensori-motor polyneuropathy. Blood lithium level was 1.80meq/l two days after quitting lithium. This is a case of a 64-year-old male with difficulty in ambulation for 40 years. Physical examination revealed a defect in vibration and positional sense over the lower extremities. Muscle power of the upper extremities was normal. The lower extremities could still resist force, but there was a little atrophic change in the quadriceps muscle and weakness over the iliopsoas muscle.Laboratory data were all negative, including CSF, routine blood, urine and stool specimens. Peripheral neuropathy is an important complication of diabetes mellitus. However, neuropathological changes in diabetic neuropathy with axonal degeneration or segmental demyelination are still unclear. By using a newly developing electrodiagnostic method of single fiber electromyography (SFEMG) with a computer-aided program, mean jitter value and muscle fiber density were measured in extensor digitorum communis (EDC) and anterior tibial (AT) muscles from 36 diabetic patients with neuropathy (group 1) and 12 patients without neuropathy (group Ⅱ) proved by nerve conduction study. All cases of home care under the service of the Nurse Assiciation of Taichung City from Oct. 1989 to Dec. 1991 were collected to evaluate sex, age distribution, major caretaker, main disease entities, referral source, service items and prognosis. Questionaire was made for home care nurses of Taichung area to evaluate their training back grounds; disease items of clients and the percentage of the individial items; rehabilitation items that they think they could do, want to learn to do and hope the involvement of rehabilitative specialists; and the major causes interfering the receivement of rehabilitative home care.In 171 cases collected, …
