The Communicative Capacities Of The Medical Discourse In Authoritarian Societies : The Case Of Aids In Iran, Elham Pourtaher

This study explores the role of medical discourse in the Iranian formal public sphere. It examines how an epidemiological wave of HIV/AIDS—known as "the third wave"—highlighted nontraditional sexual behaviors in public and enabled a shift in policy and discourse by the Islamic Republic State. Through analyzing published content on HIV/AIDS from five major Iranian newspapers between 2009 and 2013, this study identified four competing narratives of the third wave which coexist and have a dynamic relationship with one another. In recent years, the increased availability of public data dashboards provided opportunities to democratize data access by summarizing large amounts of data into accessible snapshots to facilitate educational efforts and encourage evidence-based decision-making. However, many current practices have not reached their full potential because the dashboard design has not accounted for users’ needs and preferences. This dissertation employed the user-centered design framework with three studies. Study 1 investigated the usability problems of existing public health data dashboards through an expert review of existing dashboards on government websites. Phthalates have been implicated as reproductive toxicants in animal models and in human populations. This study examined associations between potential exposure sources and urinary phthalate concentrations among couples undergoing infertility treatment. Women (n=56) and their male partners (n=43) undergoing in vitro fertilization (IVF) were enrolled in a prospective cohort investigation of environmental chemicals and IVF outcomes at an academic medical center. On the day of oocyte retrieval, participants provided urine samples and completed questionnaires detailing use of personal care products (PCPs), and consumption of medications, foods, and beverages in the preceding 24-hours. Obesity is a critical health and social concern. Almost 40% of American adults are obese, creating more than $300 billion dollars in annual medical costs. While obesity is a public concern that has been on the policy agenda for some time, obesity policies have proven difficult to formulate, adopt and implement in practice. This thesis is a study of popular data resources in healthcare and their applications toward health information system design and development. The first section reviews the way data is collected with a list of websites where health datasets can be obtained; along with a summary of the recognized vocabulary standards that make it easier to understand and share this data. The next section discusses ways of data categorization for better analysis, recognizing that a significant proportion of health data is unstructured. Essays On Health Disparities And Income-Related Health Inequality In Sub-Saharan Africa, Marshall Makate

This dissertation comprises of five empirical essays. Essay one assesses the impact of prenatal care quality and its components on child mortality using nationwide data from the Zimbabwe Demographic and Health Surveys (ZDHS), 1999-11. Results indicate that increasing prenatal care quality by one unit improves neonatal, infant and under-five survival by about 42.33, 30.86, and 28.65%, respectively and all statically significant at the 1% level.

This set of essays addresses issues related to income inequality, public healthcare supply and population health. Income inequality has been found to affect health in a number of studies. Using data from Behavioral Risk Factor Surveillance System (BRFSS), a telephone survey of adults in the United States, the first chapter of my dissertation studies the effect of state level and county level income inequality on health status and the pathway via public health spending. Some patients with chronic disabilities and diseases are able to adapt to their health states and, as a result, rate their quality of life higher than hypothetical patients imagining themselves to be in such states. Due to this phenomenon of adaptation, there is much controversy surrounding the effect of adaptation on patient preferences and the role that these adapted preferences ought to play in health care resource allocation decisions. X-ray fluorescence (XRF) spectrometry is a well-established analytical technique for determining the elemental content of many different materials. XRF has long been used for public health applications, particularly for identifying lead-based paint hazards. The primary advantage of XRF over other atomic spectrometric techniques is that it is nondestructive. In addition, little sample preparation is required, so results are obtained rapidly and at low cost. The principal aim of this study was to characterize the utility and reliability of current XRF instrumentation for use in the environmental health sciences.
