On November 8, 2005, a 26-year-old male laborer who was a passenger in a semi-truck died when the driver swerved to avoid hitting a cow that was standing in the parkway lane. The two employees had been traveling westbound for approximately one hour when the driver noticed a cow in his lane. He swerved to avoid hitting the cow but hit it anyway. The tractor and two trailers jack-knifed, traveled through the median with the cab and trailers flipped onto their right sides, slid across the eastbound lanes, then the cab slammed through the guardrail. Dump Truck Driver Dies After Unintentional Release Of Asphalt, Kentucky Injury Prevention And Research Center

On June 6, 2005 a 55-year-old male dump truck driver (Driver 1) died when he was buried waist deep in 400-degree Fahrenheit asphalt. Driver 1 stopped along the highway to help another dump truck driver (Driver 2), (both were independent truck drivers), who had experienced brake problems. As the two men were examining the pneumatic brake system at the rear of the disabled dump truck, the tailgate opened unexpectedly, spilling hot asphalt onto the decedent who was under the rear of the truck.

To prevent similar incidents, the following recommendations have been made:

On November 11, 2004, a 33-year-old male dump truck driver died when he was crushed between the driver’s side of the dump truck and the swinging counter-weight of an excavator (trackhoe). His dump truck became stuck in mud after the trackhoe loaded dirt into it so he walked to the on-site office and informed the owner. The owner located a bulldozer on site and drove it to the dump truck while the driver retrieved a chain from his personal pickup truck. On Monday, March 31, 2003, a 39-year-old male (decedent) highway traffic technician, working alone, received fatal head injuries after falling out of the bucket of an aerial boom truck. Injuries sustained in the fall resulted in his death four days later. He was changing a burned-out green traffic bulb in a traffic signal suspended over 2 lanes of traffic. At approximately 8:30 am on April 16, 2003, a 48-year-old male (decedent) highway construction laborer working in an interstate highway median, died when he was struck by an out-of-control double trailer (a semi-truck pulling two trailers). The laborer was speaking with the job foreman and another laborer in the median next to a grading machine. A 32-year-old woman (the decedent) was killed when the asphalt compactor she was operating slid over an embankment and dropped 17 feet to a roadway below. At the time she was pressing an asphalt joint joining a roadway and a shoulder/emergency lane. She was two miles behind the asphalt truck and 1000 feet in front of a flat bed truck. The location of the incident had been identified as a possible hazardous location and her supervisors had instructed her to use extra caution in that area. Since retiring from the mining industry several years ago, he had been working part-time driving a semi-trailer truck owned by a logger. As was his usual practice, the victim drove the truck alone, and on the day of the incident hauled a large load of logs on the trailer bed. The logs were approximately 12 inches in diameter and 20-25 feet long. He operated the business for about 23 years, employing various part time helpers. On the day of the incident, two sons-in-law were operating chain saws at the scene when they heard a loud "pop." A 52-year-old part-time farmer was killed when a pick-up truck overturned and crushed him. The victim, along with a neighbor (the witness), was loading the neighbor's tractor on a flatbed trailer to transport it to the victim's tobacco field for plowing. The trailer, attached to the pick-up truck, was parked on a gradual downward slope in preparation for tractor loading. The victim was standing on the front of the trailer while the tractor was being driven onto the trailer by the witness.
