Objective: Do pre-injury and early hospital admission characteristics help predict patient discharge disposition? Background: Total one-year treatment cost of adult major trauma in the United States is estimated at $30 billion annually, with 58% of the cost due to the index hospitalization. Increased length of stay (LOS) increases morbidity and delays rehabilitation. Prediction of discharge to a location other than home through univariate risk factor methodology and development of a multivariable binary logistic regression model allows early discharge planning. Methods: A one-year Level 1 trauma center registry dataset was used to develop a predictive model of discharge disposition using 2836 …

Because the general public is beginning to encounter molecular genetics during the course of their everyday lives with things like genetic screening, genetically modified food, and stem cell research, among others; scientific literacy in molecular genetics is becoming increasingly important in society today. Two learning progressions have recently been published by Roseman, Caldwell, Gogos & Kurth (2006) and Duncan, Rogat & Yarden (2009). Our study aims to test the upper bounds of the Duncan et al. Background: Cholera outbreaks result in significant morbidity worldwide. Transmission of the disease may be influenced by weather fluctuations, such as El Niño-Southern Oscillation (ENSO) and excessive rainfall, and socioeconomic status (SES), including income and education. El Niño events may influence many aspects of the climate, including the amount of rainfall occurring in a given year and surface water temperature. This study aimed to assess the influences of extreme weather and SES factors on cholera incidence in India during the seventh cholera pandemic (1961-2008).

Since the 1950s the number of major disaster declarations has more than tripled. These disasters cause an increase in the number of sick and injured individuals. In order to handle this increased patient load, health professionals must be brought in from outside the area, often from surrounding states. Current health professional licensing is maintained by each state individually, with post-disaster assistance made available through the Robert T. Stafford Disaster Relief and Emergency Assistance Act, the Pandemic and All-Hazards Preparedness Act (PAHPA), the Model State Emergency Health powers Act, the Nurse Licensure Compact, and the National Disaster Medical System. The purpose of this study was to determine whether day of the week, season, federal holidays, and severe weather conditions affected emergency department (ED) volume in Montgomery County, Ohio. A retrospective analysis of ED visits from July 1, 2010 to July 1, 2011 was conducted. Meteorological data in Montgomery County was obtained from the National Oceanic and Atmospheric Administration (NOAA). A two tailed Analysis of Variance (ANOVA) was used to determine if the total number of ED visits were significantly associated with these variables. The average number of ED visits per day was 722.5. Reproductive isolation is the basis of the Biological Species Definition and can be a driving force of speciation. Theoretical studies have provided models of how reproductive isolation can arise within individual species. Genetic tests of these models are limited to populations in which reproductive isolation is present but not complete. Here, reproductive isolation in the Elgans-Group of the nematode genus Caenorhabditis is reviewed. Pre-mating barriers, assortative fertilization and post-zygotic barriers all have been observed in this clade. In some combinations of species, fertile F1 hybrids can be obtained. Therefore, the Elegans-Group of Caenorhabditis is poised to become an important experimental …

The population requiring hemodialysis (HD) in the United States continues to grow, with recent studies reporting over 370,000 Americans with end stage renal disease (ESRD) who are HD-dependent. The creation of functional HD access is often the limiting step in utilization of renal replacement therapy (RRT). Since the 1960s, the creation of hemodialysis access has become one of the most commonly performed procedures in the United States with over 500,000 vascular access procedures performed per year. In 2011 the first of the baby boomers turned 65 years old. By 2030 a total of 78 million baby boomers, 19% of the population, will be 65 or more years old and eligible for Medicare (U.S. Census Bureau 2010). BACKGROUND:

Increased blood levels of ammonia (NH3) and ammonium (NH4(+)), i.e. hyperammonemia, leads to cellular brain edema in humans with acute liver failure. The pathophysiology of this edema is poorly understood. This is partly due to incomplete understanding of the osmotic effects of the pair NH3/NH4(+) at the cellular and molecular levels. Cell exposure to solutions containing NH3/NH4(+) elicits changes in intracellular pH (pHi), which can in turn affect cell water volume (CWV) by activating transport mechanisms that produce net gain or loss of solutes and water. Ischemic stroke makes up 87% of all hospital-admitted stroke cases annually; the primary treatment for these cases is intravenous administration of tPA within a 3.5 hour window from stroke onset. A long-term delayed ischemic stroke treatment proposed by this study was a combination of the pharmaceuticals Fluoxetine (SSRI), Simvastatin (statin), and ascorbic acid (Vitamin C). 51 adult rat subjects (10-12 months of age; 44 Sprague Dawley, 7 Long Evans) were given a combination of the drugs for 31 days. Drugs were given through voluntary oral administration via sugar cookie-dough balls to reduce inhibition of neurogenesis through stress-related glucocorticoid production. Drug …

Air traffic management personnel manage traffic flows into, out of, and through the area for which air traffic controllers in their facilities are responsible. We report on a cross-domain, artifact-based, work analysis of air traffic management that employed a hierarchical modeling methodology centered on the artifacts used in the operational setting and the information and decision-making support they provided. Many tools support traffic managers in planning for, managing, and monitoring traffic flows, but information and decision support functions are rarely integrated across tools. This study describes effects of a series of simulations of an airline flight operations center and related functions such as pilots and airport ramp control. It is continuation of a multi-year project designed to develop an effective training program for entry level aviation professionals and to develop insights into teamwork processes. The debate regarding the effectiveness of computer-based software for pilot training and proficiency has been ongoing since such software first became available. While studies on the efficacy of such software have been and continue to be conducted, pilots are in large number utilizing such packages. A nationwide survey was conducted to determine how the Microsoft Flight Simulator (MFS) software package is being used by pilots for both initial private pilot training and for maintaining proficiency once certificated. As part of an ongoing collaborative project, a high fidelity simulation lab was developed to address teamwork deficiencies exhibited by newly minted aviation professionals. This lab, referred to as the Flight Operations Center-Unified Simulation (FOCUS) lab, was enhanced with greater fidelity, realistic and complex scenario triggers, and robust after action reviews. Student teams participate in a simulated work shift to further improve problem solving skills and coordination among senior-level undergraduate students.

Effective intelligence, surveillance, and reconnaissance (ISR) relies heavily on both technological and human analytical capabilities. Intelligence analysts must be able to detect, interpret, process, and perform other critical tasks to turn data into meaningful information for decision-makers. The ability to aggregate massive data sets into operationally relevant information is challenging due to issues such as information overload, team coordination, time constraints, tunnel vision, and limited or vague guidance. This report describes research and development efforts to enhance training for geospatial intelligence analysts. We conducted an exploratory data analysis as a step toward modeling components of SA. It was based on data collected from situation awareness probe questions that were used in large-scale air traffic control simulations over 5 semesters of an ATC radar internship in the Center for Human Factors in Advanced Aeronautics Technologies (CHAAT). Three components of SA were generated by a principal component analysis that we label “Action Relevant,” “Distance R and “Low Priority.” The analyses provide a data-driven scheme for categorizing probes relate SA that can be used in future evaluations of NextGen concepts and technologies.

A preliminary examination of the effectiveness of a simulator-based training program for pilots was conducted. Thirteen students of varying backgrounds, with limited flight experience enrolled in an intensive, simulator-based flight training program. Six students completed their FAA Private Pilot certificates in an average of five weeks, while five completed within four weeks. They completed instrument ratings within three or four weeks. These qualifications were completed with fewer flight hours than the US average. One student stopped training, but four remaining students have completed their FAA commercial, multi-engine and instructor qualifications in a timely and cost-effective manner.
