Purpose: School settings are a common practice context for rehabilitation professionals; health advocacy is a common and challenging practice role for professionals in this context. This study explored how pediatric practitioners advocate for children with disabilities at school. Specifically, we examined everyday advocacy in the context of school-based support for children with disabilities.
Method: Our theoretical framework and methodological approach were informed by institutional ethnography, which maps and makes visible hidden social coordinators of work processes with a view to improving processes and outcomes. In 1980, DSM-III adopted a descriptive approach to psychiatric diagnosis, creating checklists of unwanted behaviors to define and use as required criteria when posing each of several hundred diagnoses. The objective of this novel approach was to validate psychiatry as a scientifically legitimate branch of medicine, by enabling research into hopefully homogeneous groups of patients to pinpoint the implicit hypothesized physiological causes of the disorders the patients were presumably sharing. In each subsequent revision of the DSM including the DSM-5, however, no physiological criteria of any sort are included for any diagnosis, confirming the empirical failure of this attempt to …

In this article, we examine the causal effect of parental restrictive feeding practices on children’s weight status. An important mediator we are interested in is children’s self-regulation status. Traditional mediation analysis (Baron and Kenny, 1986) applies a structural equation modelling (SEM) approach and decomposes the intent-to-treat (ITT) effect into direct and indirect effects. More recent approaches interpret the mediation effects based on the potential outcomes framework. In practice, there often exist confounders that jointly influence the mediator and the outcome. Inverse probability weighting based on propensity scores are used to adjust for confounding and reduce the dimensionality of confounders simultaneously. …

This article reports on the current state of health disclaimers, online health resources, and credibility markers provided on public library websites (N = 105) for those seeking health information. In light of the Affordable Care Act (ACA), these findings have implications for consumer health reference services in public libraries. Approximately half of the public library websites in the study sample (n = 52, 49.5%) referred users to the ACA Health Insurance Marketplace website (HealthCare.gov). Surrogacy As The Sale Of Children: Applying Lessons Learned From Adoption To The Regulation Of The Surrogacy Industry's Global Marketing Of Children, David M. Smolin

This article will argue that most surrogacy arrangements as currently practiced do constitute the “sale of children” under international law, and hence should not be legally legitimated. Hence, maintaining the core legal norm against the sale of children requires rejecting currently constituted claims of a right to procreate through surrogacy. Given the underlying purpose of all human rights law in maintaining the inherent human dignity of all human beings, a claimed legal right built upon the sale of human beings must be rejected.

Background: In Phase I trials, attribution of adverse events (AE) is a complicated process. The current endpoint of dose limiting toxicity (DLT) assumes attribution is known. Such a simplified endpoint ignores individual-specific or drug-specific information. We propose an approach that allows for personalized scores that account for the uncertainty in AE attribution. Methods: The proposed design takes into account possible errors when attributing AE and allows investigators to explicitly express their degree of uncertainty in whether any AE is drug-related or disease-related. A score of 100% means that the AE is certainly drug-related, whereas a score of 0% means that …

In March 2014, the largest Ebola outbreak in history exploded across West Africa. As of November 14, 2014, the World Health Organization has reported a total of 21,296 Ebola virus disease (EVD) cases, including 13,427 laboratory-confirmed EVD cases reported from the three most affected countries (Guinea, Liberia, and Sierra Leone). As the outbreak of EVD has spread, clinical disease severity and national EVD case-fatality rates have remained high (21.2-60.8%). BACKGROUND: Neisseria meningitidis (Nm) is a leading causative agent of bacterial meningitis in humans. Traditionally, meningococcal meningitis has been diagnosed by bacterial culture. However, isolation of bacteria from patients' cerebrospinal fluid (CSF) is time consuming and sometimes yields negative results. Recently, polymerase chain reaction (PCR)-based diagnostic methods of detecting Nm have been considered the gold standard because of their superior sensitivity and specificity compared with culture. In this study, we developed a loop-mediated isothermal amplification (LAMP) method and evaluated its ability to detect Nm in cerebrospinal fluid (CSF). BACKGROUND: Reported response rates of transitional cell carcinoma (TCC) in dogs to piroxicam in combination with either mitoxantrone or carboplatin are similar; however, it is unknown whether either drug might provide superior duration of response. HYPOTHESIS/OBJECTIVES: To determine if the progression-free interval (PFI) of dogs with TCC treated with mitoxantrone and piroxicam was different than that of dogs receiving carboplatin and piroxicam. The hypothesis was that the efficacy of mitoxantrone is no different from carboplatin. ANIMALS: Fifty dogs with TCC without azotemia. METHODS: Prospective open-label phase III randomized study. BACKGROUND: Atypical hyperadrenocorticism (AHAC) is considered when dogs have clinical signs of hypercortisolemia with normal hyperadrenocorticism screening tests. HYPOTHESIS/OBJECTIVES: To compare cortisol concentrations and adrenal gland size among dogs with pituitary-dependent hyperadrenocorticism (PDH), atypical hyperadrenocorticism (AHAC), and healthy controls. ANIMALS: Ten healthy dogs, 7 dogs with PDH, and 8 dogs with AHAC. METHOD: Dogs were prospectively enrolled between November 2011 and January 2013. Dogs were diagnosed with PDH or AHAC based on clinical signs and positive screening test results (PDH) or abnormal extended adrenal hormone panel results (AHAC). Kisspeptin receptor (KISS1R) agonists with increased half-life and similar efficacy to kisspeptin in vitro may provide beneficial applications in breeding management of many species. However, many of these agonists have not been tested in vivo. These studies were designed to test and compare the effects of a KISS1R agonist (FTM080) and kisspeptin on luteinizing hormone (LH) in vivo.
