Protecting Communities In Research: Current Guidelines And Limits Of Extrapolation, Charles Weijer, Gary Goldsand, Ezekiel Emanuel

As genetic research increasingly focuses on communities, there have been calls for extending research protections to them. We critically examine guidelines developed to protect aboriginal communities and consider their applicability to other communities. These guidelines are based on a model of researcher-community partnership and span the phases of a research project, from protocol development to publication. The complete list of 23 protections may apply to those few non-aboriginal communities, such as the Amish, that are highly cohesive. Although some protections may be applicable to less-cohesive communities, such as Ashkenazi Jews, analysis suggests substantial problems in extending these guidelines in toto …

The purpose of this work is to determine the most physically effective radiation energy for K-edge absorption of x- or gamma-rays by iododeoxyuridine (IUdR) on Chinese hamster ovary (CHO) cells. Brachytherapy sources (Sm-145, I-125, Yb-169 and Am-241) and x-ray beams (30 kVp, 100 kVp and 100 kVp with gold, gadolinium, lead or tungsten filtration) were investigated for their preferential absorption qualities by IUdR sensitized DNA. The 30 kVp, 100 kVp and 100 kVp with tungsten filtration were then used to irradiate CHO cells, with or without IUdR incorporation (i.e. 10(-5) M of IUdR for 3 days). The current controversy as to the proper role of the placebo control in the evaluation of new treatments for schizophrenia requires an analysis that is sensitive to both ethical and scientific issues. Clinical equipoise, widely regarded as the moral foundation of the randomized controlled trial (RCT), requires the use of best available treatment as the control in RCT. Scientific criticisms of the use of an active control are examined and none present an insuperable barrier to the use of an active control. Indeed, scrutiny of the most recent argument for the use of placebo controls, 'assay sensitivity', suggests that the …

The purposes of this study were threefold: (a) to describe self-care agency, (b) to describe self-care practice, and (c) to describe the relationship between self-care agency and self-care practice in adolescents. The usefulness of Orem's Self-Care Deficit Nursing Theory (SCDNT) with adolescents also was evaluated. Participants in this study included 173 adolescents, 14 to 19 years of age. Data were collected in classroom settings in two diverse high schools. Denyes Self-Care Agency Instrument-90 and Denyes Self-Care Practice Instrument-90 were administered. Results revealed the presence of self-care agency and self-care practice. Although self-care practice was lower than self-care agency, a significant …

Theoretical accounts of the nature and purposes of clinical ethics consultation are disappointingly superficial and diffuse. Attempts to illuminate the goals, the forms, the substance, and the criteria for the success of ethics consultations need to focus on detailed reports of cases and the contexts in which they occur. The uncommonly rich description of the consultation surrounding Mrs. Roses plight provides a splendid opportunity to explore such matters. The ethics consultant pursues a number of ventures providing and clarifying information, improving communication, educating and counseling, and being a friend with variable degrees of success. Movement disorders include both psychiatric illnesses with neurologic features and neurologic illnesses with frequent psychiatric complications. During 1998, neuropsychiatric progress was made on several fronts, notably in parkinsonism and tic disorders. This remains an exciting area for research, with many unsolved, clinically important problems, but with increasing neurobiologic knowledge and sophistication.
