Hallus Rigidus: A Technique For Reliable, Safe, And Reproducible Cheilectomy Excision, Jory Wasserburger, Amber Price, Matthew Wharton, Richard Miller

Hallux rigidus is a common cause of foot pain involving the hallux; however, the etiology is yet to be elucidated. There are many different treatments that have been described, including cheilectomy. Taking too much bone can cause joint instability and altered joint mechanics, whereas taking too little bone may not relieve pain. We present a reliable, safe, and reproducible technique for cheilectomy in the hallux rigidus.

Abstract Loose bone and cement can promote polyethylene wear that may lead to failure in total knee arthroplasty. The authors propose a technique to enhance the debridement of loose bodies by distracting the cemented components with a lamina spreader following standard debridement with pulsatile lavage. Cement and bone/soft tissue debris was recovered from 51 consecutive primary total knee arthroplasty patients. This technique facilitated the removal of hidden debris in 80% of patients (41/51). On average, 2.6 fragments measuring 158 mm3 were removed per patient. The authors advocate that distraction with a lamina spreader be used to reduce potential sources of …

After performing open reduction and internal fixation (ORIF) for treating malleolar fractures of the ankle, surgeons typically use plaster splints during postoperative recovery of patients. Use of regional anesthesia during ORIF has been noted as a risk factor for burns in patients using plaster splints, possibly owing to inability to feel pain after undergoing regional block. We describe a successful postoperative splinting technique used for preventing thermal injuries in this patient population. Syndactyly is the most frequently occurring congenital malformation of the limbs. Although common, few studies have reported treating syndactyly using Z-plasty with and without full-thickness grafting. We present a 2-year-old boy who presented to our clinic with ectrodactyly of the feet and simple complete syndactyly of two fingers on each hand. After ectrodactyly reconstruction, we used the excess foot skin to cover finger defects. The patient recovered well postoperatively, with matching skin and no trouble with hair growth. Use of a spare-parts technique can help avoid complications associated with harvesting grafts from other donor sites. The results reinforce the importance …

Fractures of the proximal humerus are common in older patients, and the incidence of these fractures in the United States is expected to notably increase with the aging population. Nonoperative procedures have been preferred in elderly patients with stable fracture patterns to avoid complications associated with osteoporotic bone. However, more complex and unstable fracture patterns often necessitate operative techniques to allow for more anatomical healing.
