Nurses have identified a need for improving their knowledge and skills in providing end-of-life care. Critical care nursing textbooks can serve as an important source of information on end-of-life care for critical care nurses. Hence, an analysis of end-of-life content in 14 critical care nursing textbooks was conducted. Critical care nursing textbooks used for review were published in 1995 or later and identified from the libraries at the University of Wisconsin-Madison and Brigham Young University. The accreditation criteria of the Commission for Collegiate Nursing Education (CCNE) direct faculty to select nationally developed standards for professional practice to guide curricula and to articulate curriculum outcomes. Recently, the CCNE proposed that all undergraduate programs should adopt the American Association of Colleges of Nursing (AACN) Essentials of Baccalaureate Education as one set of national standards for undergraduate and prelicensure programs. Standards of Holistic Nursing Practice, developed by the American Holistic Nurses’ Association, is an additional set of standards that assists in defining quality for undergraduate nursing education. The purpose of this study was to discover associate degree nursing students' perception of their summer clinical practicum. The university was located in rural southeast Tennessee. A qualitative method of phenomenological design was used with an unstructured interview to ask the question: "please tell me about your summer clinical practicum and how it affected you?" The framework was Imogene King's Systems of Transaction in Gqal Attainment. Themes identified were communication, mentoring, interpersonal relationships, prior learning, and growth and development. The Perceived Value Of Certification By Certified Perioperative Nurses, Kathleen B. Gaberson, Kathryn Schroeter, Aileen R. Killen, Wendelny A. Valentine

We conducted this study to determine the perceived value of certification in perioperative nursing. Following development and pilot-testing, we mailed the 18-item Likert-type instrument, the Perceived Value of Certification Tool (PVCT), to a sample of 2750 perioperative nurses who had earned the CNOR or CRNFA credential or both. A total of 1398 surveys were returned (50.8% response rate). Factor analysis extracted three factors, accounting for 61% of the variance: personal value, recognition by others, and professional practice. Internal consistency reliability testing (Cronbach's α) identified a standardized α of .924. Over 90% of respondents agreed or strongly agreed with statements about …

The purpose of this article is to describe the process of creating a faculty development program for online instruction. Both educational and technological issues are discussed. Factors that will facilitate and barriers that will impede the implementation of online courses are included. Many faculty concerns regarding online courses relate to the issues of quality and student learning. Faculty development activities are directed to ensure that courses are developed using sound educational theory and principles. Online courses are first about student learning; using the technology is second. Thus, faculty development programs must be two-pronged, involving instructional design and technology. It is …

The Health Enhancement of the Rural Elderly (HERE) project is a federally funded grant project designed to empower the rural elderly in Butler and Edmonson counties to maximize their use of the health care system. This project was developed after identifying the needs of approximately 25% of the population in the U.S. that reside in rural areas. Of this population in rural Kentucky, 14-15% is 65 years or older (U.S. Bureau of Census, 2000). A higher incidence of chronic diseases, disabilities, difficulties with daily living activities, and cutbacks in social services have been identified as factors causing this population to …

Delirium is one of the most serious and prevalent cognitive disorders among hospitalized elders. Fourteen elders participated in this phenomenologic study describing the "lived experience " of delirium from the patient's perspective. Implications for nursing practice are derived from their reality and insight.

It is important to understand constructs essential to low-fat-diet and physical-activity behaviors of low-income culturally diverse middle-school-age students, because ethnicity and socioeconomic status influence body weight status, and these groups are most at risk for obesity later in life. In this descriptive study, constructs important in low-fat diet and physical activity in low- to middle-income, culturally diverse middle school students were examined. Revised for cultural/developmental appropriateness and reading level, instruments incorporating constructs from the Health Promotion and Transtheoretical Models were tested with 221 youth. The purpose of this study was to determine the feasibility of a flexible multiple daily insulin (FMDI) regimen in routine pediatric diabetes care by comparing HbA1c, body mass index (BMI), and episodes of severe hypoglycemia (SH) before and after initiation of FMDI therapy. Data from 44 patients (2-16 years old), on a conventional insulin (CI) regimen, were collected during quarterly diabetes clinic visits. These patients were transitioned from CI to FMDI regimen: pre-meal lispro (bolus) and once or twice daily Humulin Ultralente with or without bedtime Humulin NPH as the basal insulin. The article discusses institutional issues related to the development and implementation of an undergraduate student exchange program in nursing. A consortium of four universities in the United States and six nursing schools in three European countries developed an exchange program to teach a common community health module. Thirty-one students from the United States and 30 students from Europe participated in this program, which was by the U.S. Department of Education and the European Union. The project commenced with a 3-week meeting of faculty and students to design the 8-week exchange program and cultural preparation module. This article describes institutional issues …

The effectiveness and safety of hormone replacement therapy (HRT) have been studied since the 1980s. However, much of the available evidence was contradictory, and practitioners were uncertain how to counsel peri- and postmenopausal patients. In this article, recent evidence, both for and against use of HRT in peri- and postmenopausal patients, is compared to past studies with recommendations for practice.

OBJECTIVES: To examine the effect of organizational characteristics on physical restraint use for hospitalized nursing home residents.
DESIGN: Secondary analysis of data obtained between 1994 to 1997 in a prospective phase lag design experiment using an advanced practice nurse (APN) intervention aimed at reducing physical restraint for a group of hospitalized nursing home residents.
SETTING: Eleven medical and surgical units in one 600-bed teaching hospital.
PARTICIPANTS: One hundred seventy-four nursing home residents aged 61 to 100, hospitalized for a total of 1,085 days.

Cancer cachexia is a poorly understood syndrome of anorexia, weight loss, and muscle wasting that negatively impacts quality of life and survival in cancer patients. Research has clearly implicated proinflammatory cytokines in the biology of cancer cachexia. More recent research implicates products of arachidonic acid and suggests that cachexia may be a chronic inflammatory condition rather than a nutritional aberration. To date, nutritional support to slow weight loss has focused primarily on increasing calorie intake. Alternatively, many foods contain factors that can modulate the synthesis or activity of proinflammatory mediators, especially the synthesis of prostaglandin E2 from arachidonic acid. These …

Although combination therapies slow progression of HIV disease, HIV-infected women face new challenges that must be overcome to increase their life expectancy. One challenge is adherence to strict, and often unpleasant, drug regimens. Peplau's Theory of Interpersonal Relationships provides a framework for identifying and examining factors that influence medication adherence, in particular difficulties in taking medication. A descriptive, correlational design was used to test associations between difficulty taking HIV medications and a woman's relationship with her primary healthcare provider (PHCP), depression, HIV-specific emotional and tangible support, present life satisfaction, family functioning, HIV-related symptoms, AIDS beliefs and perceptions, medication side-effects, and …

Medical errors have received a great deal of attention in recent years. The phrase medical errors is an umbrella term for all errors that occur within the health care system, including mishandled surgeries, diagnostic errors, equipment failures, and medication errors. This article is a review and discussion of the literature on the scope of medical errors, with a focus on drug-related problems and medication errors. Cost and quality ideas for addressing these issues are provided.

The purpose of this study was to evaluate stable DNA transfection of M-21 human melanoma cells with particle-mediated gene transfer (PMGT) with B7-1 cDNA and to identify sites of gene integration. Stable B7-1 transfectants (M-21-B7) were obtained with PMGT using a plasmid vector containing cDNA for both B7-1 and neomycin phosphotransferase, with subsequent selection with G418. The transfected cells were flow sorted by B7-1 expression into two populations, bright and dim. The bright population had 85%–90% of cells expressing B7-1; the dim population had less than 50% of cells with B7-1 expression. Chromosome analysis with fluorescence in situ hybridization (FISH) …

The landmark Institute of Medicine Report, To Err Is Human: Building a Safer Health Care System. stated that medical error causes 44,000 to 98,000 deaths per year. There is no question that the report raised awareness of patient safety and stressed the importance of patient outcomes. Heightened awareness has produced a patient safety industry of sorts, with solutions that range from technology to outcomes measurement.

Rh Negative Status And Isoimmunization Update: A Case-Based Approach To Care, Kathryn Shisler Harrod, Lisa Hanson, Leona Vandevusse, Patricia Heywood

Prior to the 1970s and the advent of Rho (D) immune globulin (RIG) for Rh negative women, hemolytic disease of the newborn led to morbidity, long-term disabilities, and mortality. Antepartum RIG administration has been a standard of practice since 1983. Yet, Rh isoimmunization (sensitization) and its sequelae have not been completely eradicated. Rh-related issues remain clinical challenges facing perinatal and neonatal nurses. Evidence for the administration of RIG prenatally and during the postpartum period is presented including controversies and challenges. Current information about fetal and neonatal care of erythroblastosis fetalis and immune hydrops is also presented.
