The aim of this project was to create a pamphlet of community resources available to seniors in Franklin County, Vermont to aid in connecting them to community involvement and support.

This project aimed to address the presence of both physician burnout and the effect it can have on the care of patients and particularly the most complex ones. A list of resources for complex patient management and physician self care was compiled and presented for physicians at Berlin Family Practice.
