Is The Use Of Apixaban Or Enoxaparin, More Effective In The Prophylaxis Of Venous Thromboembolism Post-Orthopedic Surgery?, Jolene Bohensky

OBJECTIVE: The objective of this selective EBM review is to determine whether or not the use of Apixaban or Enoxaparin is more effective in the prophylaxis of venous thromboembolism post-orthopedic surgery.

STUDY DESIGN: Review of three English language randomized, double-blind, placebo-controlled clinical trials from 2009-2010.

DATA SOURCES: Randomized, double-blind placebo controlled, clinical trials comparing the effectiveness of Apixaban and Enoxaparin in the prophylaxis of Venous Thromboembolism (VTE) post-orthopedic surgery were found using PubMed, the Cochrane Database of Randomized Controlled Trials, and the Cochrane Databases of Systematic Reviews.

Studies have shown that CYP2E1, a cytochrome P450 enzyme containing two primary binding sites, plays a substantial role in the oxidative metabolism of many foreign substances, including the detoxification reaction of 4-nitroanisole to 4-nitrophenol. Through the advancements of the computational docking software Tripos Sybyl7.2, it has been possible to investigate the mechanism of oxidation of 4-nitroanisole. Docking modules of Sybyl7.2 software, including Surflext and molecular dynamics, have created the ability to ascertain the likely binding configurations of 4-nitranisole and its constitutional isomers in either the distal or proximal binding sites of CYP2E1. Ewing's Sarcoma belongs in the Ewing's Sarcoma Family of Tumors (EFST) which represents approximately 2% of all cancers found in children and young adults. These tumors are generally treated by chemotherapy along with either surgery or radiation therapy which can themselves have debilitating effects on the patients. Dr. Lori Hensley's research has shown that ajulemic acid (AJA), a synthetic cannabinoid compound, exhibits extraordinary anti-tumor effects. In addition, AJA has not expressed any psychotropic side effects typically seen with other cannabinoids. Nicotine and nornicotine are the two main alkaloids that accumulate in Nicotiana tabacum L. (tobacco), and nornicotine is the N-demethylation metabolite of nicotine. Nicotine is synthesized in the root, and probably primarily in the root tip. Both nicotine and nornicotine exist as two isomers that differ from each other by the orientation of H atom at the C-2' position on the pyrrolidine ring. (S)-nicotine is the dominant form in tobacco leaf and the enantiomer fraction of nicotine (EFnic), the fraction of (R)-enantiomer over the total nicotine, is approximately 0.002. Demand for joint replacements is rising in Australia, driven by a sharp increase in the number of joint problems associated with population aging and obesity. In artificial joints, delamination or failure within the coatings occurs when the stress reaches a critical level, resulting in large wear debris particles appearing on the contact surface between the head and the cup. The process has been described as due to a stress-corrosion-cracking mechanism. Under the same loading, stress increases when the contact area decreases, which happens in the vicinity of wear debris. Methoprene is a Juvenile Hormone (JH) analogue commonly used for the control of mosquito larvae. When applied to a mosquito breeding site, methoprene enters the haemolymph, where it mimics the function of JH and interferes with normal metamorphosis, resulting in larval mortality. Methoprene is commonly used for the control of larvae and has not been used as an adulticide, due to an absence of acute effects. This study evaluated possible chronic effects caused by the exposure of adult Aedes mosquitoes to methoprene. Endocrine disrupting substances (EDSs) comprise a wide variety of chemicals that perturb normal endocrine function including developmental and reproductive processes in vertebrates. The synthetic estrogen 17α-ethynylestradiol (EE2) is a commonly-used model EDS because of its environmental relevance and its effects on the reproductive endocrine system. Early life-stage exposure of fish to estrogenic EDSs causes effects such as intersex (ovotestes in males) and feminization. This study aims to confirm the period of gonadal differentiation in the estuarine killifish or mummichog (Fundulus heteroclitus) and to determine the sensitivity of gonadal development to EE2.
