Serum Creatinine Concentrations In Healthy Newborns: Reference Ranges During The First Five Days Of Life, Jennie Andreson, Jaime Alfredo Zegarra, Carlos Alberto Estrada, Francis Dumler

Serum creatinine concentrations were studied in 52 healthy, full-term infants (29 males and 23 females) during the first five days of life. At birth, mean serum creatinine concentration was 0.76 ± 0.13 mg/dL. At 6 hours of life, values increased to 0.97 ± 0.11 mg/dL (P < 0.001) and remained elevated for 24 hours. Values then returned to baseline so that serum creatinine concentrations were 0.81 ± 0.15 mg/dL at 48 hours and 0.6l ± 0.15 mg/dL at 3 to 5 days of age. At birth, serum creatinine concentrations were higher in males than in females (0.80 ± 0.13 versus 0.71 ± 0.11 mg/dL, respectively; P < 0.009). However, the increases in serum creatinine concentration observed after birth were similar in both sexes. This is the first report of a prospective longitudinal study of serum creatinine concentrations between birth and 3 to 5 days of age in a Hispanic population. In addition, data were analyzed by sex. The results are a useful reference for normal serum creatinine concentrations in early life.

Ovarian Failure In A Chromosomally Competent Adolescent Female: A Case Report, T. Jann Caison-Sorey

Primary ovarian failure in adolescence is uncommon. This report depicts such a case in a 17 1/2-year old girl whose investigation revealed elevations in the follicle-stimulating hormone and luteinizing hormone levels in association with a low estrogen level. The blood leukocyte chromosome analysis with Giemsa banding revealed a 46,XX chromosomal complement. Pelvic ultrasound disclosed a rudimentary uterus and a small left ovary. Hormonal replacement has resulted in advancement of secondary sexual characteristics and monthly withdrawal bleeding.

Electronic rectal thermometry is performed routinely in most medical centers. While electronic thermometry has been validated at the oral site, rectal measurement has not. We performed a controlled study on 84 children comparing mercury-in-glass and electronic thermometry at the rectal site. No significant difference occurred in temperatures between the two methods. We conclude that the electronic thermometer is a valid instrument to measure rectal temperature.

Pediatric Emergency Care: A Survey Of Michigan Emergency Departments, David J. Treloar, Gregory Preston, Fred Lamb

Recent studies suggest a lack of preparation in emergency medical systems for pediatric patients. Michigan emergency departments were surveyed to determine their degree of preparedness regarding equipment, space, and pediatric on-call coverage. More than 50% of responding emergency departments did not have a pediatrician on call and lacked a pediatric "crash cart," and 34% had to send their personnel to other areas in the hospital to obtain pediatric equipment during resuscitations. This survey suggests a lack of preparation for pediatric emergencies in Michigan.

Food allergy occurs in as many as 5% of infants less than 1 year old. Most food sensitivities disappear by age 3 to 4. Most immunologically mediated food allergy reactions are due to type I IgE mechanisms, with skin, gastrointestinal, and respiratory signs and symptoms the most common manifestations. U sing the double-blind, placebo-controlled food challenge (DBPCFC) technique and atopic dermatitis as a model, the most common foods to which children are allergic are cow's milk, eggs, peanuts, wheat, fish, and soy proteins. Anaphylactic sensitivity in older children often involves crustacean seafoods (eg, shrimp), nuts, peanuts, fish, and eggs. Such …

Among the many challenges of providing care for children with multiple physical and developmental handicaps is the proper management of their respiratory difficulties. This study was designed to evaluate the prevalence of respiratory symptoms in children attending a multidisciplinary clinic for handicapped children. We also used several methods to estimate the prevalence of allergic sensitivity in children with respiratory complaints. Questionnaires concerning respiratory symptoms were completed by the parents of 104 patients. The most frequent complaint reported was snoring in 41% of the children. More than 1 million teenagers become pregnant every year in the United States. Nearly one-half of the pregnancies are carried to term, while a similar number end in abortion. In Michigan this amounts to approximately 34,000 teenage pregnancies (ages 15 to 19). These adolescent patients have a high incidence of sexually transmitted diseases (STDs). Of the pregnant teens at Henry Ford Hospital. 60% present with one or more STDs. We discuss the case of a 13-year-old pregnant girl who presented at five weeks estimated gestation with gonorrhea, chlamydia, and appendiceal perforation, resulting in subsequent spontaneous abortion. Teenagers in the United States experience significant mortality and morbidity because of high-risk behaviors. Adolescents need routine health supervision aimed at reducing these risks. A Teen Clinic within the Pediatrics Clinic at the Henry Ford Hospital Fairlane Center offers separate facilities, longer appointments, and an emphasis on adolescent issues. This plan is a model for practitioners in various office settings who seek to improve adolescent care.

Of 35 tuco-tucos (Ctenomys opimus) collected in Bolivia, South America, 31 (88%) had eimerian oocysts in their feces at the time they were examined. Eighteen (58%) of the 31 infected animals were concurrently infected with 2 or 3 eimerian species. Four species of Eimeria were recovered and are described as new species based on the characteristics of sporulated oocysts. Oocysts of Eimeria granifera n. sp. were ellipsoidal, 21.1 x 17.2 (15-26 x 11-20), [micro]m with sporocysts ovoidal, 11.3 x 7.1 (8-14 x 5-9), [micro]m. Ellagic acid (EA), a naturally occurring plant phenol, at concentrations of 5 to 50 pg/plate, inhibited rat liver S9 protein dependent benzo[a]pyrene (B[a]P)-induced mutagenesis in Salmonella typhimurium TA 100 by 30-81% and B[a]P 7,8-dihydrodiol (DHD)-induced mutagenesis by 29 to 75%. EA (5-40 juM) did not significantly affect the metabolism of B[a]P or B[a]P 7,8-DHD as determined by high performance liquid chromatographic analysis of the organosoluble fraction and by the quantification of watersoluble conjugates. Prior work at East Canyon Reservoir, Utah indicated that adult trout are important predators of stocked juvenile rainbow trout. Brown trout, in particular, were exclusively piscivorous when th~y exceeded 185 mm standard length. Fish became increasingly important in the diet of cutthroat trout greater than 330 mm. The purpose of this study was to further document the effect of adult trout on survival of juvenile rainbow trout in mid-elevation reservoirs. Causey Reservoir was chosen because it was thought to contain large brown and cutthroat trout and had not been intensively studied during the last eight years.

There were 65 people who attended part or all of the 1988 Fall Meeting at Halsey Forest 8-9 October. This included Dr. Jan Skoudin, of Prague, Czechoslovakia. Saturday night Dr. Julie Ann Savidge spoke on “The Extinction of an Island Forest Avifauna by an Introduced Snake”, a summary of her investigation of the brown tree snake’s effect on Guam’s avifauna. There was also a short slide show. The rest of the time was available for birding. ADDITIONAL REPORTS FROM THOMAS COUNTY, SPRING 1988. Mike Dwyer, then working on a grouse telemetry project at Halsey Forest for the Game and Parks Commission, reported a Northern Saw-whet Owl 10 May. Mr. Dwyer's comments were: Most likely territorial male. Constant repetitions whistle. First vocalizations heard at 2345 h. in mixed hardwood/conifers. On 13 June he reported a Chuck-will's-widow, with these remarks: Was close enough to detect the "chuck" syllable of the call. Calling occurred at dusk in mixed hardwoods/conifers.

This study describes in a quantitative manner the relationships between coating structure and optical properties. Scattering coefficient, gloss and pore volume were measured after applying different blends of polystyrene spheres using various amounts of binder to transparent Mylar film. The coatings were prepared using four proportions of two sizes of polystyrene spheres and varying amount of two binders, starch and latex respectively. Optical properties of coatings were related to changes in coating structure. A scanning electron microscope was used to observe the structure and to facilitate void size measurement. Survival of Escherichia coli and Salmonella spp. in estuarine waters was compared over a variety of seasonal temperatures during in situ exposure in diffusion chambers. Sublethal stress was measured by both selective-versus-resuscitative enumeration procedures and an electrochemical detection method. E. coli and Salmonella spp. test suspensions, prepared to minimize sublethal injury, were exposed in a shallow tidal creek and at a site 7.1 km further downriver. Bacterial die-off and sublethal stress in filtered estuarine water were inversely related to water temperature. Salmonella spp. populations exhibited significantly less die-off and stress than did E. coli at water temperatures of <10°C. Although the most pronounced reductions (ca. 3 log units) in test bacteria occurred during seasonally warm temperatures in the presence of the autochthonous microbiota, 102 to 104 test cells per ml remained after 2 weeks of exposure to temperatures of >15°C. DNA polymerase alpha, the enzyme involved in nuclear DMA replication, and DNA polymerase gamma, the enzyme involved in organellular DNA replication, were isolated and purified from soybean and turnip. The enzymes were characterized following ammonium sulfate precipitation, DEAF-cellulcse, phosphocelluloca, and hydroxylapatite chromatography, and by non-denaturing polyacrylamide gel electrophoresis. Protein banct were electropluted and the enzymes characterized using kinetic studies and sensitivity to divalent cations and inhibitors. Molecular weight and subunit composition studies indicated a molecular weight for the catalytic subunit of DNA polymerase alpha in soybean and turnip to be 46kDa.
