The immunological paradox of nurturing a fetus with paternal antigens poses some perplexing questions. Peter Medawar, an immunologist, asked at a lecture, “How does the pregnant mother contrive to nourish within itself, for many weeks or months, a fetus that is an antigenically foreign body?” Researchers have since then struggled to answer this question. Before 1990, options were limited for couples who were at risk for transmitting a genetic disease or a structural chromosomal abnormality to their children. Couples traditionally underwent invasive procedures such as amniocentesis and chorionic villus sampling, after which termination was offered if the fetus was found to be affected. Many couples chose not to have children at all. Since then, technological advances have allowed preimplantation genetic testing (PGT) to be offered to these couples. The United States has been noted as the country with the greatest caffeine consumption in the world. More than half of all Americans are said to consume caffeine on a daily basis. Caffeine is easily available in a variety of dietary products including coffee, tea, soft drinks, and chocolate. Many pregnant women are unaware of the potential risk that excessive caffeine consumption can have on their unborn child. Although the common effects of gestational diabetes on the fetus are known, the outcome of a diabetic gestation is ambiguous. There is concern for complications in the fetus at delivery due to enlarged size of the fetus, as well as concerns for birth defects, fetal distress, diabetes and obesity. Yet, although there is a correlation in the previous disorders and gestational diabetes, many of the mechanisms that cause these complications are unknown.
