The objective of this paper was to determine relationships between patient age and BMI and use of a self-administered dietary assessment website in the primary healthcare setting. Chi- square and ordinal regression models were used to determine the relationships between age and BMI and computer experience, ownership, and usage from 188 patients using a self-administered dietary assessment website over 12 months. One hundred and twenty-five (66.5%) female and 63 (33.5%) male patients used the website. A total of 72.9% were overweight (BMI425 kg/m2). Dietary fat is considered central to the management of obesity and heart disease risk, and more recently with the risk of type 2 diabetes, given its observed role in the development of insulin resistance in mechanistic studies. For changes in lipid profiles, foods delivering substantial amounts of polyunsaturated fatty acids relative to saturated fats are of interest, and walnuts fit this category.As a class of foods, nuts have been shown to provide benefits to health in a number of clinical trials. "Objective: Levels of vitamin E have been reported to be lower in patients suffering major depression, but whether this is due to inadequate dietary intake or the pathophysiology of depression is not known, and was the subject of the present study. Setting: Wollongong, Australia. Methods: Plasma vitamin E (a-tocopherol) was measured in 49 adults with major depression, age (mean7s.d.): 47712 y. In a subset (n¼19) usual dietary intake of vitamin E was determined by diet history. Results: Subjects had significantly lower plasma a-tocopherol (4.7170.13 mmol/mmol cholesterol) than has previously been reported for healthy Australians, and plasma a-tocopherol was inversely related …

Metabolic syndrome is a term linking the clinical profiles of some of the world’s major health problems today: obesity, heart disease and diabetes. It is predicated on dietary patterns, and particularly on the delivery of fuel. The effects may be seen first in the development of abdominal obesity and insulin resistance leading to type 2 diabetes mellitus and coronary heart disease. This review examines the role resistant starch might play in the prevention and management of these conditions. These terms are often used in contexts which encompass mental, physical and emotional health as well as broader more esoteric aspects of life satisfaction such as happiness. Automation of dietary assessments allow participant behaviour to be captured by video observation. They also allow clinicians to identify areas which effect reporting accuracy. This observational study describes the differences in behaviour according to the type of foods selected by participants using a dietary assessment website encompassing diet history methodology.

Objective: To identify the nature, strength and relative importance of influences on intentions to consume foods that are enriched with omega-3 fatty acids using the Theory of Planned Behavior (TPB). Design: A cross-sectional self-administered questionnaire. Setting: Community based residents living in the Illawarra region of New South Wales, Australia. Subjects: Two sub-samples were surveyed via questionnaire: Community members who responded to a local media advertisement (n=79), and subjects in a dietary intervention trial for type 2 diabetes mellitus (n=50). Background Mycobacterium avium subspecies paratuberculosis (MAP) is an infective agent found in ruminants and milk products, which has been suggested to increase the risk of gastrointestinal inflammation in genetically susceptible hosts. It is hypothesized that lactase persistence facilitates exposure to such milk products increasing the likelihood of adverse outcomes. Individuals either homozygous or heterozygous for the T allele of DNA variant, rs4988235, located 14kb upstream from the LCT locus, are associated with having lactase persistence. It has been speculated that high plantar pressures might cause foot pain and discomfort which, in turn, may discourage children from being physically active and result in them spending more time in sedentary activities. The purpose of this study was to determine whether plantar pressure distributions generated by preschool children were correlated with objectively measured time spent in physical activity and sedentary behaviour. Dynamic plantar pressures were measured for 33 preschool children (age = 4.3±0.6 years; height = 1.06±0.1 m; mass = 18.4±2.9 kg; 17 boys) as they walked across an emed AT-4 pressure platform. It has been speculated that the higher peak pressures typically generated beneath the feet of overweight/obese children may result in excessive discomfort of these children’s developing feet (Mickle et al. 2006, Dowling et al. 2001), in turn, acting as a deterrent to them participating in physical activity. Apart from perpetuating the cycle of obesity via decreased energy expenditure, physical inactivity in young children can restrict opportunities for these children to develop basic motor skills and, possibly, proper musculoskeletal development. Background Evidence is growing on the benefit of physical activity to improve well-being following a cancer diagnosis. This study examined changes in physical activity from pre to post diagnosis and explored this relationship with quality of life and depression. Methods Participants were recruited by posters and by letter of invitation. The questionnaire was completed by 59 prostate and 32 breast cancer survivors. Results Physical activity decreased by 72 minutes per week from pre to post diagnosis, although 20.9% reported having increased activity post diagnosis. Over 30% were considered depressed. Breast cancer participants who increased physical activity post diagnosis reported higher …

The benefit of exercise for cancer patients is starting to become recognized. The purposes of this paper were to review the literature to examine whether research findings are being converted into guidelines for patients and survivors and to examine the quality of evidence on which they were based. A computer search of major health databases was conducted for peer-reviewed literature and books on exercise and cancer, and an Internet search was conducted for cancer websites reporting any exercise guidelines/recommendations for cancer patients. Total direct costs of the Women's Health Trial (WHT), alarge multicenter preventiontrial, were reduced by more than 50% by means of research costanalysis conducted during the trial design phase. The unit costs of specific trial activities were estimated so that total direct costs of the trial could be predicted from design parameters. The relative costs of screening, treatment, and follow-up, and the fixed costs associated with each clinical center in a multicenter preventiontrial were taken into account. Objectives: To investigate the relationship between sleep duration and obesity in older Australian adults. Method: Self-reported health data were collected through the 45 and Up Study. Multinomial logistic regression models were used to test the relationship between sleep duration and body mass index, controlling for a range of demographic and health-related variables. Results: The sample included 45,325 Australian adults aged 55 to 95 years. The regression models demonstrated that short and long sleep were associated with obesity in 55- to 64-year-olds but not in those aged 65 years and above. OBJECTIVES. This study examined the implications of annual screening mammography for cost and mortality in American Indian populations with differing baseline breast cancer rates. METHODS. A decision tree compared annual screening mammography and screening clinical breast examination with referral for diagnostic mammography when appropriate. The decision tree was constructed to examine the effect of different base-line cancer rates, stage at diagnosis, and stage-specific survival. Outcomes included 5-year relative survival, deaths prevented at 5 years, cost per death prevented, and total costs. RESULTS. The findings suggest that the total cost of breast cancer is 3.6 times higher with the screening mammography …

The Workgroup on public education was brought together to formulate an international perspective on public education for the important public health challenge of increasing colorectal cancer screening. The focus of the group did not include professional education, which is an important channel for public education, since another Workgroup covered that topic. The results of the Workgroup represent an expression of expert opinion and did not include a comprehensive review of the literature. However, the recommendations for further research may stimulate a more in-depth examination of the literature in some areas and generate ideas for investigation.

Emerging evidence suggests that chronicsleeprestriction contributes to obesity. Targeting short sleep duration may therefore offer a novel and effective method of preventing and treating obesity. Headache is a common symptom in primary care about which surprisingly little is known. Over a 14-month period 3847 patients making 4940 consecutive visits for headache to 38 primary care practices in the USA and Canada were studied. The clinical characteristics of patients, as well as the diagnostic and therapeutic strategies employed by their doctors, were examined. Visits for headache represented 1.5% of all visits during this period. Most patients (72.0%) made only one visit, and nearly half of the headaches reported were new. Although progress has been made in providing cancer prevention and control services to the population, there remain sizeable segments of the population that are either uninvolved or underinvolved with national and state efforts. One method of reaching most of the population, including those segments currently not involved or underinvolved, is through primary care clinicians. Effective, albeit imperfect, strategies to increase the involvement of primary care clinicians in cancer prevention and control are available, and there is emerging evidence that these strategies are making a difference. The avian influenza A (A/H5N1) virus has attracted the attention of governments and health organizations throughout the world because of its pandemic potential. Despite the emerging nature of A/H5N1, there is limited research on public knowledge and perceptions of this disease. This study is based on a computer-assisted telephone interviewing survey conducted in May 2006 to determine the Australian publics knowledge of A/H5N1, their willingness to engage in preventive behaviors, and their acceptance of potential messages for communication campaigns. Little is known about the accuracy of data reported in practice based primary care research. The Ambulatory Sentinel Practice Network (ASPN) undertook a 100% audit of 226 patients included in a study of spontaneous abortion (SAB). The audit was conducted to assess the feasibility of conducting audits in primary care research networks dispersed over large geographic areas, verify that patients met inclusion criteria, and assess the frequency of reporting errors using the medical record as a standard.
