The number of commercial spaceflights will be increasing by orders of magnitude over the next several decades. The current volume of space traffic can be managed on a case-by-case basis and there is little impact to the National Airspace System (NAS). The Space Data Integrator (SDI) Project is the initial step to satisfy the Federal Aviation Administration (FAA) strategic initiative to integrate commercial space launch and reentry vehicles into the National Airspace System. The project addresses the needs for greater situational awareness and monitoring, and increased response capability during non-nominal and catastrophic incidents during space operations.

Post-market surveillance is a collection of processes and activities used by product manufacturers and regulators, such as the U.S. Food and Drug Administration (FDA) to monitor the safety and effectiveness of medical devices once they are available for use “on the market”. These activities are designed to generate information to identify poorly performing devices and other safety problems, accurately characterize real-world device performance and clinical outcomes, and facilitate the development of new devices, or new uses for existing devices. Tools that predict dangerous working conditions empirically in the cockpit could greatly improve aviation safety. In an effort to lay the groundwork for such tools, this study will investigate whether a link exists between task load and pilots’ control strategies, their biometric data, and their planes’ position data. Using X Plane simulation software, an experiment will be run in which licensed pilots will fly a 3-nautical-mile approach into Mineta San Jose International Airport 4 times, under different task load conditions. During the flight, participants’ pupil diameters will be recorded along with the simulated aircraft position data and cognitive control modes. …

In recent decades, commercial aviation accidents have occurred due to human-machine interaction (HMI) problems known as “mode confusion.” This is caused, in some cases, by a lack of understanding of onboard systems by pilots. Large amounts of Flight Operational Quality Assurance (FOQA) data are available, analysis of which could assist in identification of safety risks in daily operations. Through analysis of pilot reactions and recorded aircraft data, methods to detect mode confusion are developed to improve aviation safety. NASA's Human Exploration Rover Challenge, held annually in at the Marshall Space Flight Center in Huntsville, Alabama, is an engineering design challenge that asks teams of student engineers to design a human-powered vehicle capable of traversing a simulated lunar surface. The rover must be able to be transported in a 5x5x5 foot cube, echoing the design constraint faced by the engineers who built the Lunar Roving Vehicles used by the astronauts of the later Apollo missions.

Historically, high risk industries such as aviation have attempted to respond to accidents by fixing what they find after the investigation is complete and therefore represents a reactive approach to an accident event. For many years, the industry has tried to become more proactive to these events by seeking out problems and correcting them before an accident occurs. A Safety Management System is a detailed and regimented process by which any high risk industry can try to become proactive in accident prevention.
