The European Union Aviation Safety Agency (EASA) continues to promulgate Safety Management Systems (SMS) across aviation organizations when it recently issued SMS requirements for EASA certificated Part 145 maintenance, repair and overhaul (MRO) organizations and Part 21 Design & Manufacturing (D&M) organizations. Currently, the Federal Aviation Administration (FAA) has no such rule in place for these U.S.-based organizations, which may challenge those doing business in countries where EASA policies apply. Flight data monitoring (FDM) began in the flight test community in 1939 and entered the airline industry in 1974. In the 48 years since, however, very few operators have chosen to adopt this practice, which has shown clear safety benefits where it has found acceptance. While technical issues have created some obstacles, cultural issues have proven the greatest hindrance to wider FDM adoption. This research analyzed the perceptions of interpersonal skills on established aviation safety models, Crew Resource Management (CRM), and Threat and Error Management (TEM) using feedback from industry pilots. The flight deck is a sociotechnical system where much research has focused on the technical aspect, whereas we spotlight its socio aspect. The aviation industry must invest in training pilots on interpersonal skills to enhance safety through increased efficacy of safety models integrated throughout existing training programs. Near midair collisions are precursor events to actual collisions and may be an indicator of risk. While previous studies have used reports of near midair collisions to relate factors such as airspace saturation to near midair collisions, the reports relied upon were generally subjective in nature. With the adoption of automatic dependent surveillance-broadcast (ADS-B), the approximate position of every equipped aircraft can be recorded. This study performed a comparative analysis of SMS in both the aviation and ground transportation industries to identify any gaps that may impact aviation safety by incorporating UAS operations within the ground transportation industry. The study reviewed relevant literature and compared the FAA SMS and FMCSA SMC policy components. The study also compared the implementation of OSHA requirements within the aviation and ground transportation industries. There were 7,500 safety events in the NTSB data sets from 2013-2018. These events were analyzed using Chi-square, Cramer’s V, and the odds ratio. Major findings in the study determined that while pilots crash aircraft for the same reasons whether they are in a training environment or not, student pilots are typically less likely to be killed, or seriously injured. The aircraft that student pilots fly however, do not share the same relative safety in some event types. Students destroy and substantially damage more aircraft than their non-training counterparts in abnormal runway contact events. The top five causes of safety …

A method called the best-worst scaling method is proposed to overcome these problems by asking respondents to make tradeoffs among the variables being assessed. This paper raises issues in respect of supplementing and developing a block of disruptive behaviors exhibited by passengers during flights and assesses the impact of these behaviors on other passengers. To illustrate the proposed method and evaluate its performance, we surveyed 240 passengers, and data from 203 survey samples were analyzed. This paper develops an alternative to the ground risk model provided by JARUS SORA. Key inconsistencies in the SORA ground risk model are identified, specifically ground risk continuing to increase when there is no further increase in fatality probability.

Population density is a critical component of UAS ground risk. Definitions of population density adopted by various regulatory jurisdictions are reviewed. A categorisation of population density is developed based on official statistics categories for New Zealand. This categorisation is more granular than that provided by SORA, enabling a more nuanced assessment of risk.

There is a need to develop avionic systems to support pilots when they encounter environmental conditions that compromise their visual capacity. This report discusses the effects of DVEs on aviation, expounds on the advanced avionic systems available and further elaborates how these systems are implemented in countering DVE conditions. Automated Dependent Surveillance Broadcast (ADS-B) provides position and state information about aircraft and is becoming an essential component in the global air traffic management system. ADS-B transponders broadcast this key information on a common frequency to both other aircraft and to secondary surveillance radar systems located at ground stations. Both the aircraft transponders and the ground stations work together to assist in managing the commercial airspace. Eye movement characteristics might provide insights on pilots' mental fatigue during prolonged flight. The visual entropy, eye fixation numbers, and eye fixation durations of ten novice pilots and ten expert pilots were analyzed for a four-hour simulated flight task consisting of four consecutive flight legs. Each flight leg lasted approximately one hour and contained five flight phases: takeoff, climb, cruise, descend, and landing. The pilots maneuvered the simulated B-52 aircraft following instrument flight rules (IFR) in a moderate-fidelity Microsoft Flight Simulator environment. Organizations spend considerable resources building robust Safety Management Systems, yet may struggle to develop a strong Safety Culture. The effect can ripple throughout the organization, particularly affecting employee safety, as well as product quality, productivity, engagement, and retention. This paper introduces the four basic components and most critical cultural preconditions of a successful SMS. The majority of First Officers and Captains feel that Captains establish the tone of the flight deck microculture. However, the majority of these pilots also report that as First Officers, they have hesitated to share safety-pertinent data and/or experience being silenced after sharing safety data due to a psychologically unsafe flight deck. Utilizing direct feedback from industry pilots, we explore ways that Captains can create a more inclusive microculture that elicits safety voice. Flying into hazardous weather can be a cause of aviation incidents and accidents. Accidents involving general aviation (GA) pilots who are not instrument rated who fly into instrument meteorological conditions (IMC) are often fatal. Pilot weather reports (PIREPs) can increase the accuracy and timeliness of current and forecasted weather conditions. They are an essential tool used by pilots to avoid flying into hazardous weather as well as meteorologists to develop and update aviation forecasts.
