The purpose of the research was to create and validate a safety performance decision-making tool to transform a reactive safety model into a predictive, decision-making tool, specific to flight training organizations, to increase safety and aid in operational decision-making. Using Monte Carlo simulation, the study conducted simulation runs based on operational ranges to simulate the operating conditions with varying levels of controllable resources in terms of personnel (Aviation Maintenance Technicians and Instructor Pilots) and expenditures (active flight students and available aircraft). Four What-if Scenarios were conducted by manipulating the controllable inputs. Changes to the controllable inputs are reflected by variations …

This phenomenological study aimed to understand and interpret corporate trainers' perspectives when developing disaster or pandemic-driven training content for remote situations and suggest a baseline response to identified deficits. Using the disaster risk management theory by Kim and Sohn (2018), with specific emphasis on Petak's (1985) and McLoughlin's (1985) framework, data from the participants was collected using semi-structured individual interviews, document analysis, and observations. While corporate trainers have used years of experience and seasoned pedagogy to enhance learning for their participants to achieve corporate objectives, almost no content exists regarding the process. The paradigm shift to a knowledge economy, predicted by Drucker, is currently reflected in a knowing-to-doing gap in healthcare, potentially threatening the lives of long-term care (LTC) residents and sustainability of LTC organizations. The purpose of this grounded theory study was to seek a substantive conceptual theory to explain how LTC uses knowledge management (KM) to improve performance by probing the a priori views and lived experiences of 11 LTC knowledge creators, managers, and users. Two electromagnetic phenomena have the potential to create continental-scale disasters. The first, nuclear electromagnetic pulse (EMP), results from a nuclear detonation high above the tropopause. The second, a major solar storm, or "solar tsunami" occurs naturally when an intense wave of charged particles from the sun perturbs the earth's magnetic field. Both phenomena can debilitate electrical and electronic systems necessary for the operation of infrastructure systems and services. The Commission to Assess the Threat to the United States from Electromagnetic Pulse Attack has provided a compelling case for protecting civilian infrastructure against the effects of EMP. As with protecting infrastructure against any hazard, it will be important to take a risk-based priority approach for EMP, recognizing that it is fiscally impracticable to protect everything. The Institute of Medicine’s seminal report, To err is human: Building a safer health system, established the national patient safety framework and initiated interest in changing the traditionally punitive healthcare culture. This paper reviews a multidisciplinary literature and offers an attribution framework to explicate the organizational processes that contribute to an industry-wide culture where clinicians are routinely blamed for adverse patient events. Attribution theory is concerned with the manner in which people explain the behaviors of others or themselves by assigning causality for events. Conducting an effective postconference continues to be challenging because of low levels of student participation. Many students are exhausted at the end of the clinical day, which lessens their participation in postconference. In my experience, students often omit important information, such as patients’ age, sex, race, and other medical problems, during initial reports. Some have trouble providing patients’ information in an organized manner as well. The SBAR (situation-background-assessment-recommendation) communication tool can be used as a strategy to conduct clinical postconference.

No critical infrastructure is self-sufficient. The complexity inherent in the interdependent nature of infrastructure systems complicates planning and preparedness for system failures. Recent wide-scale disruption of infrastructure on the Gulf Coast due to weather, and in the Northeast due to electric power network failures, dramatically illustrate the problems associated with mitigating cascading effects and responding to cascading infrastructure failures once they have occurred.
The major challenge associated with preparedness for cascading failures is that they transcend system, corporate, and political boundaries and necessitate coordination among multiple, disparate experts and authorities. Iatrogenesis is a term typically reserved to express the state of ill health or the adverse outcome resulting from a medical intervention, or lack thereof. Three types of iatrogenesis are described in the literature: clinical, social and cultural. This paper introduces a fourth type, technological iatrogenesis, or emerging errors stimulated by the infusion of technological innovations into complex healthcare systems. While health information technologies (HIT) have helped to make healthcare safer, this has also produced contemporary varieties of iatrogenic errors and events. This standard establishes minimum requirements and design objectives for high-altitude electromagnetic pulse (HEMP) hardening of transportable1 ground-based systems that perform critical, time-urgent command, control, communications, computer, and intelligence (C4I) missions. Systems required to fully comply with the provisions of the standard will be designated by the Joint Chiefs of Staff, a Military Department Headquarters, or a Major Command.
The standard prescribes minimum performance requirements for low-risk protection from mission-aborting damage or upset due to HEMP threat environments.
