Background: Electronic health records (EHRs) are commonly used within healthcare systems across the United States. Although EHRs were developed to provide safe delivery of care, poor documentation quality remains a problem. Poor HER documentation can negatively affect many patients’ health outcomes.

Purpose: The purpose of the DNP project was to determine the degree to which using a simulated electronic documentation model to train nurses in an acute care unit would decrease documentation errors and missed care and decrease missed opportunities for patient education in EHRs.

Abstract PROBLEM: Currently healthcare acquired pressure injuries (HAPIs) in the United States result in significant and unnecessary costs along with lost revenue for healthcare facilities. These costs are estimated to be between 9.1 and 11.6 billion dollars annually. Pressure Injuries (PIs) are directly associated with decreases in patient outcomes and are deemed preventable incidents. HAPI prevention can mitigate this clinical and financial problem for patients and organizations. The Reggio Emilia Approach (REA) and Head Start (HS) value high-quality early education, support for families, and community partnerships according to the literature. Exploring principles of REA in relation to the HS program model may reveal alignment that will inform high-quality developmentally appropriate practices that are meaningful and enhance learning outcomes. This multiple case study explores the REA in alignment within three Head Start programs in New Mexico, Massachusetts, and Tennessee. This action research study proposes the need for and examines the usefulness of a Student-Driven Guide for Project-Based Learning. The Guide was designed to give students guidance and structure in carrying out and documenting a project as a part of a class, or when students are working independently from a teacher. Documentation plays a central role in metaliteracy. When individuals engage in metaliterate practices of creating, sharing, and assessing information, they are, in fact, engaging in practices with documents. Yet, while the goals and objectives of metaliteracy implicitly acknowledge documentation, they do not explicitly emphasize the fundamental roles played by it in helping facilitate and enable various metaliterate practices. This article aims to make these roles explicit.

This multiple baseline single-case design study explored the influence that training in observation and interpretation had on teachers’ meaningful conversations with children. Three preschool teachers (1 from public school and 2 from Head Start) were trained using the Cycle of Inquiry System (Broderick & Hong, 2011) that informs of ways to observe and interpret children’s thinking to facilitate developmentally appropriate conversations.

Teachers documented and interpreted observations of children engaged in small group play. Teachers were surveyed pre-training and post-training about observation, interpretation, curriculum, the teacher’s role, and the purpose of teacher interactions with children. Registered Nurses use clinical documentation to describe care planning processes, measure quality outcomes, support reimbursement, and defend litigation. The Connecticut Department of Health, guided by federal Conditions of Participation, defines state-level healthcare policy to include required care planning processes. Nurses are educated in care planning process standards, however no policy-required competency verification processes in academia or employers exists. Guided by the advocacy coalition framework, the purpose of this quasi-experimental study was to determine if the quality of nurse coalition actors’ clinical documentation, a relatively stable parameter, would increase after attending policy-centered structured education. This paper describes the initial results from the Data Information Literacy (DIL) project designed to identify the educational needs of graduate students across a variety of science disciplines and respond with effective educational interventions to meet those needs. The DIL project consists of five teams in disparate disciplines from four academic institutions in the United States. Research conducted over the past two decades indicates the predictive nature of family involvement in children’s education to child school and life success (Jeynes, 2005). Beyond factors such as socioeconomic status and parental education, the extent to which families are involved in children’s education at home and at school has been shown to have the strongest relationship to student academic success (Henderson & Berla, 1994). Literacy is a concept that is understood to be the identifier of an educated populace. In today's world, literacy includes computer literacy, as well as language and quantitative literacy. This paper describes exercises developed to improve first year students' computer literacy through more learner-centered engagement. Exercises are designed to support learner-centered goals of independent and responsible learners, appropriate breadth and depth of content, teacher as facilitator, and assessment woven into learning. Exercise topics include purchase of a personal computer, basic logic via spreadsheets, an annotated bibliography built with electronic resources, and an integrated assignment customized by and for each student.
