The following is a quantitative study with an explorative element conducted at Southern California University with a convenience sample of 23 social worker participants with experience in child welfare. A Qualtrics questionnaire was administered during the Fall 2023 semester and was used to assess the social workers perception, preparedness, and knowledge in providing services to transgender youth in foster care. The study aimed to gain an understanding of the overall perception of the effect in available transgender resources and training within the child welfare system.

The current climate for trans folks in the U.S. remains increasingly hostile and many researchers have called attention to the "joy deficit" within the existing trans literature (Shuster & Westbrook, 2022). This study investigates what trans individuals are currently doing to survive, thrive, and resist in a belligerent socio-political climate. To answer this, five community conversations with 25 participants were held using a semi-structured conversation guide. Within the analysis, the central theme that emerged was that trans individuals are using their communities to create radical futures. Social work education has traditionally used frameworks, such as cultural competency, to guide implicit and explicit curricula that shape how we think about communities that live and thrive outside of white supremacist and cis/heteronormative norms and values. While the cultural competency framework intends to promote a level of consciousness and attention that is required to practice with diverse individuals, families, and communities whose identities differ from that of the social worker, it instead inadvertently creates a knowledge base that reinforces harmful power dynamics between social worker and client/community. Educational settings have been found to be challenging arenas for transgender and gender expansive (TGE) youth and young adults due to misgendering, lack of affirming bathrooms, systemic exclusion (e.g., legal names and lack of inclusive gender identity demographic options), and frequent silence or avoidance related to TGE issues. Though studies of TGE adult experiences in higher education are emerging, most explore disaffirming experiences. Social work programs have been largely silent in the face of legislative at- tacks on transgender/nonbinary (TGNB or “trans”) communities across the U.S., which signals to TGNB students that they may not be supported in their respective programs. Consequently, TGNB students, staff, and faculty shoulder the burden to advocate for change within social work academic institutions and to speak out about violence perpetrated against TGNB communities. Understanding factors influencing mental health and substance use in transgender and gender diverse people is critical to reducing disparities in this population. We sought to investigate whether a history of sex work was associated with increased prevalence of poor mental health, substance use, and a negative experience within drug and alcohol treatment facilities. We conducted a secondary analysis of the data of 25,204 transgender respondents of the 2015 United States Transgender Survey. We draw on previous studies regarding the entire LGBTQ+ community and the minimal research on the TGD community. The study conducted interviews to measure the level of competency social workers have assisting members of the lesbian, gay, bisexual, transgender, queer and/or questioning, intersex, and asexual community (LGBTQIA+) using a qualitative exploratory research design. Data was collected from nine social workers from the United States of America. Key findings indicated the participants generally did not feel competent to work with this community, particularly transgender persons. Training at work, through school, or learned on their own about serving LGBTQIA+ clients were generally limited in scope. Despite increasing attention to cultural humility and inclusivity for financial therapy, mental health, and financial practitioners, little is written about the best practices for working with transgender and gender non-conforming (GNC) clients. This theoretical paper will review these best practices and will present a hypothetical financial therapy case study that utilizes the Inclusive Financial Well-being Empowerment Model (IFWEM). The goal of this work is to introduce practitioners to the best practices for working with transgender and GNC clients in an inclusive and empowering way and to promote future data-driven research for this population.

Transgender and gender diverse individuals face a variety of barriers when attempting to access healthcare, from discrimination to lack of access to lack of knowledgeable providers. Using data from the 2015 United States Trans Survey (N = 27,715), this study looks at the differences within the TGD population regarding having seen a doctor in the past year, having a primary care provider, and having a primary care provider who is knowledgeable about trans health. This study examines past-year unmet healthcare need due to cost experienced by transgender and gender-expansive (TGE) adults in the United States in the context of the Patient Protection and Affordable Care Act (ACA). It also aims to estimate the importance of having health insurance among TGE Americans (transgender men, transgender women, nonbinary/genderqueer people, and cross-dressers). Data were from the 2015 U.S. Transgender Survey (N ¼ 19,157 adults, aged 25 to 64 years). The enforcement of the gender binary is a root cause of gender-based violence for trans people. Disrupting gender-based violence requires we ensure “gender” is not presumed synonymous with white cisgender womanhood. Transfeminists suggest that attaining gender equity requires confronting all forms of oppression that police people and their bodies, including white supremacy, colonialism, and capitalism (Silva & Ornat, 2016; Simpkins, 2016). Using data from the 2015 United States Transgender Survey, this study investigates which patient sociodemographic characteristics and psychosocial risks are associated with likelihood of transgender mistreatment in healthcare and how patterns vary for patients of color. Numerous predictors, including alignment of identity documents, were associated with healthcare mistreatment. Among subgroups of transgender patients of color, psychosocial risks were more consistently significant than sociodemographic characteristics in predicting mistreatment. National and international health organizations are called to enact clear policies that affirm transgender patients and patients of color and establish a commitment to effectively serving these populations within their ethical codes.

Transgender people encounter interpersonal and structural barriers to healthcare access that contribute to their postponement or avoidance of healthcare, which can lead to poor physical and mental health outcomes. Using the 2015 U.S. Transgender Survey, this study examined avoidance of healthcare due to anticipated discrimination among transgender adults aged 25 to 64 (N ¼ 19,157). Multivariable logistic regression analysis was conducted to test whether gender identity/expression, socio-demographic, and transgender-specific factors were associated with healthcare avoidance. Almost one-quarter of the sample (22.8%) avoided healthcare due to anticipated discrimination. Transgender men had increased odds of healthcare avoidance (AOR ¼ 1.32, 95% CI …

As part of a larger online survey, we conducted an Internet-based study that included both qualitative and quantitative data from a national non-probability sample to examine how sexual minority women and gender non-conforming individuals described their experiences and concerns after the 2016 election. The current study explores responses in relation to local social and political climates. Quantitative analysis of survey responses (N = 969) examined changes in participant concerns relative to state policy context (number of positive policies offering equal rights and protections for LGBTQ communities) and size of community (e.g., urban and rural). Native Hawaiian people, and especially lesbian, gay, bisexual, transgender, māhū and/or queer (LGBTQM) Native Hawaiians, face health and mental health disparities that are disproportionate when compared with other racial/ethnic minorities in Hawai`i, and when compared to the United States as a whole. Native Hawaiians have the highest mortality rates for numerous biomedical diseases, including higher rates of substance abuse, diabetes, and even asthma, of any ethnic group in the state of Hawai`i (Andrade et al., 2006; Liu & Alameda, 2011). Social work students must be equipped to confront injustice and oppression. Effectively challenging oppression necessitates attention to the ideological origins and subsequent systematic marginalization of oppressed populations. This article critically examines social work education as it relates to trans people and communities. We propose two interconnected pedagogical shifts for consideration: moving from the social work classroom as “safe space” to the social work classroom as “brave space,” and broadening the commonly used educative method of cultural competence to structural competence. Profound Health Care Discrimination Experienced By Transgender People: Rapid Systematic Review, Luisa Kcomt

Transgender people experience interpersonal and structural barriers which prevent them from accessing culturally and medically competent health care. This rapid systematic review examined the prevalence of health care discrimination among transgender people in the U.S. and drew comparisons with sexual minority samples and the general U.S. population. Eight primary studies with 35 prevalence estimates were analyzed. The transgender population experience profound rates of discrimination within the U.S. health care system. Compared to sexual minorities, transgender participants appear to be more compromised in their access to health care. Service providers must change structural inequities which contribute to transgender people’s invisibility
