The deminer was investigating the signal and excavating by sitting/squatting position and his prodder/bayonet detonated the PMN mine. The evidence on site shows that the excavation of the signal started from the first mark, and not 30cm back from the indication. The bayonet directly touched the mine causing a detonation. This indicates he was not conducting operations with the correct procedure.

On 10/12/2007 as the team was busy with normal demining activities, suddenly a dog of the villagers run harshly into the minefield and as the dog reached to 6m distance from a deminer's clearance lane, suddenly an explosion happened to the dog which perished it on the spot. As a result of the explosion [the Victim] deminer who was busy on marking of his cleared lane got slight injuries on his both hands and as he was dressed fully with the PPE, he did not receive any more injuries to other parts of his body.

On 01 November 2007 at 09:50 am an accident happened on [the Victim] deminer of section # 2 of the mentioned team. The victim deminer was walking around and wanted to cross the boundary of the MF using unclear area that suddenly he stepped on an AP mine and caused it to explode. The accident happened during rest time of the team and the deminer was without PPE, so as a result of the accident he lost his left leg, sustained right foot ankle joint fracture and some other minor injuries to other part of his body.

[The Victim] was part of an [Demining group] Mine Clearance Team was clearing an area of ground that they knew was definitely mined with PMN anti-personnel mines, laid very close together, at intervals of less than one metre between one mine and the next. On 04 June 2007 at 1103 hrs while the deminer was working in his clearance lane, his trowel struck the top of a PMN anti personnel mine, causing the mine to detonate. During the accident his visor was not worn. This lack of face protection caused the serious injuries to his eyes. The accident occurred near a corner of a ruined mud wall. The mud wall over a period of time had gradually disintegrated and fallen on ground surface, thus increasing the ground depth over the mine. Activity being performed when accident occurred: [the Victim] was performing excavation with a bayonet probe (prodder) from left to right (left handed) and hit a PMN mine. It is not clear why was he using his bayonet prodder for excavation, or if he probed the soil before excavating. No MDD or MDU were in support of DT #4 on the day the accident occurred.

Activity being performed when accident occurred: According to [Demining group] Field Officer, Mr. [Name removed], both deminers were together on the base stick when the accident occurred because acting SL [Victim no.1] was verbally instructing Deminer [Victim no.2] how to perform excavation. Deminer [Victim no.2] was performing excavation with a bayonet probe (prodder) and hit a PMN mine. On 19th April-07 at 08:10 hrs while the Deminer was excavating the area by pickaxe suddenly an anti personal mine has hit by pickaxe and exploded caused the accident and Deminer got superficial injuries on his right arm and neck. During our observation from the accident point it was seemed that the Deminer was working with pickaxe not by bayonet/prodder, because the pickaxe handle was available in the accident point.

The deceased deminer detonated the PMN mine with an unidentified object. He was in a kneeling position with his right side facing towards the mine. The blast struck him on the chest and right hand side, causing a fatal wound to the right under arm (armpit). He suffered extensive bleeding from this wound and also from the mouth and nose at the time. He was evacuated by ambulance but died en route to the hospital.

At 11:12hrs on Wednesday 7th February 2007 [Demining group] MCT-6, a deminer was involved in an accident at Islam Qala Kalat-e-Nayab Ghafor MF-0069 Kohsan district Herat province. The accident occurred while the deminer was excavating with shovel.

The deminer was tasked to clear across the slope before clearing up the slope and it appears that he has carelessly moved into an unclear area before physically clearing it and has stepped on a PMN Mine. [The Victim suffered] Lower left leg amputation and injury to upper right arm and lower right leg.
