The Qualitative Report
In this article, we share insights regarding an arts-based research project where carers of people with dementia conveyed their experiences in cloth. Carers face high rates of mental ill health and burnout, while forming a largely undervalued and unrecognised workforce. Through this project, carers’ knowledge was valued and amplified using an innovative methodology – craftivism. During a series of five workshops in 2021, a small group of carers, researchers and artists gathered online to develop an exhibition of craftivist textile works. They evoked the complexity of their makers’ journeys supporting loved ones at the end of life, finding joy and …

Understanding The Perspectives Of African American Gay And Bisexual Men Toward Hiv, Victor Onwezi , Ikechukwu Nwanguma
Walden Dissertations and Doctoral Studies
There is an urgent need to stop the HIV epidemic among African American gay and bisexual men in the United States. The purpose of this basic qualitative study was to identify the health issues responsible for the HIV epidemic among African gay and bisexual men. Two theoretical frameworks that guided the study were the transtheoretical model and the syndemic theory. The study's research involved understanding African American gay and bisexual men's perspective toward HIV infections and their understanding of possible reasons HIV rates appear to increase in their community. Semi-structured telephone interviews and field notes were used to collect primary …
Exposed Intimacies: Clinicians On The Frontlines Of The Covid-19 Pandemic, Ellen Block
Exposed Intimacies: Clinicians On The Frontlines Of The Covid-19 Pandemic, Ellen Block
Sociology Faculty Publications
COVID-19 has overwhelmed health-care providers. The virus is novel in its prevalence, severity and the risk of asymptomatic infection. In order to reduce the risk of infection and stop the spread of COVID-19, clinicians in hospitals across the United States are taking measures to limit exposure to infected patients by reducing the frequency of visits to patients’ rooms, touching patients less, and adopting new protocols around the use of personal protective equipment (PPE). While these newly adopted practices are helping to reduce transmission risk of COVID-19, they are producing a habitus of infection; an acute shift among clinicians that is …
A Case Study Of Barriers To Dental Care In Nebraska, Kaitlin Buhler
A Case Study Of Barriers To Dental Care In Nebraska, Kaitlin Buhler
Honors Theses
An important aspect of overall health is access to dental care, which is, unfortunately, oftentimes difficult to obtain. This study used data collected from a case study of three registered dentists in Nebraska to investigate the barriers to dental care, and identified the lack of dental insurance, limited access to free or reduced-price dental care, and dental anxiety as perceived barriers to dental care. Factors that did not seem to have an impact were language or ethnicity differences between patients and workers, distance to travel, comfortability with receiving free or reduced-price dental care, knowledge of how often to visit the …
Comparison Of Advance Care Planning Interventions And Older Adults’ Advance Directive Completion Rates, Tammy Biehler
Comparison Of Advance Care Planning Interventions And Older Adults’ Advance Directive Completion Rates, Tammy Biehler
Walden Dissertations and Doctoral Studies
Despite the benefits of advance care planning (ACP), only one third of older adults in the United States have completed it. To address this gap, health care organizations are implementing ACP programs to engage older adults in the process. These programs can be delivered in various ways, including 1:1 conversations, classes, and web-based tools. The purpose of this quantitative study was to investigate which ACP option was associated with highest participation and advance directive (AD) completion rates in older adults. Social cognitive theory provided the framework to understand the impact the ACP environment and personal characteristics’ have on the odds …
From The Human To The Planetary: Speculative Futures Of Care, Miriam Ticktin
From The Human To The Planetary: Speculative Futures Of Care, Miriam Ticktin
Publications and Research
This is largely a theoretical, speculative essay that takes on the question of what ‘care’ looks like at a moment when climate change is increasingly taking center stage in public and political discussions. Starting with two new practices, namely, humanitarian care for nonhumans and One Health collaborations, I seek to determine what forms of political care can incorporate the well-being of future generations and future iterations of the earth. After an exploration of One Health as an approach to planetary care, I ask what its parts enable us to think, despite its limitations; I focus on the new human-nonhuman assemblages …
“We Make Death Look Pretty”: A Qualitative Study Analyzing Reported Effects Of Compassion Fatigue On Hospice Nurse-Patient Communication, Katherine Harville
“We Make Death Look Pretty”: A Qualitative Study Analyzing Reported Effects Of Compassion Fatigue On Hospice Nurse-Patient Communication, Katherine Harville
Masters Theses, 2010-2019
Rationale: Compassion fatigue is present in multiple nursing fields, but hospice poses a significant threat to nurses working within that specific environment. This is due to their consistent proximity with patient suffering, death and dying, and constant communication with patients regarding their death. The hospice nurse-patient relationship requires a deeper connection between nurse and patient which often results in consistent emotional labor for hospice nurses, further amplifying the threat of compassion fatigue. While the effects of compassion fatigue can manifest in many forms, it is typically characterized by a nurses’ decline in job satisfaction, which they do not tend to …

Activating Primary Care Copd Patients With Multi-Morbidity (Apcom) Pilot Project: Study Protocol, Sameera Ansari, Hassan Hosseinzadeh, Sarah May Dennis, Nicholas Zwar
Faculty of Science, Medicine and Health - Papers: part A
Chronic obstructive pulmonary disease (COPD), third leading cause of mortality worldwide,1 is primarily caused by cigarette smoking in Australia.2 COPD often occurs in the presence of multi-morbidity, which is the simultaneous occurrence of two or more chronic conditions; this is a growing concern in a health system focused on single-disease management.3 Around 80% of older Australians have multi-morbidity, average prevalence of chronic respiratory disease being 9.5%.4 A systematic review of COPD education programs suggests that equipping patients with self-management skills is as important as disease knowledge.5 Studies exploring positive effects of chronic disease-management interventions6 also underline the lack of studies …

Changes In Medical Education To Help Physicians Meet Future Health Care Needs, Judith N. Hudson, Kathryn M. Weston, Elizabeth Farmer
Faculty of Science, Medicine and Health - Papers: part A
Health care needs are changing due to the rapidly ageing population and the increasing number of patients with long term conditions and comorbidities.1 This has occurred at a time of continuing maldistribution of the medical workforce in Australia and increased specialisation and subspecialisation within the medical profession and the medical education system. As the next generation of doctors will need to serve an older population and those with more than one condition, a more useful focus would be "much less on narrow disease silos and . more on the breadth of possible permutations of co-morbidity". 1 Long periods of training …

Background Key Information Summaries (KIS) were introduced throughout Scotland in 2013 so that anticipatory care plans written by general practitioners (GPs) could be routinely shared electronically and updated in real time, between GPs and providers of unscheduled and secondary care. Aims We aimed to describe the current reach of anticipatory and palliative care, and to explore GPs' views on using KIS. Methods We studied the primary care records of all patients who died in 2014 in 9 diverse Lothian practices. We identified if anticipatory or palliative care had been started, and if so how many weeks before death and which …

Faculty of Science, Medicine and Health - Papers: part A
The aim of this study was to develop delirium care pathways (DCPs) useable and relevant for registered practitioners in all care settings: community; acute; and nursing homes. A qualitative approach was adopted to develop the pathways inductively. Focus groups and one-to-one interviews with registered practitioners (n = 45) working as managers, practitioners and clinical nurse consultants were undertaken to develop draft versions of the pathways, which was pilot trialled across 19 clinical settings. The publication of the DCPs was a concise and easily readable document for registered practitioners who required immediate guidance on how to implement evidence-based delirium care for …

Faculty of Science, Medicine and Health - Papers: part A
Background: Migrant travellers who return to their country of origin to visit family and friends (VFR) are less likely to seek travel-related medical care and are less likely to adhere to recommended medications and travel precautions. Through this study, we aimed to get an understanding of the views of stakeholders from community migrant centres and primary care providers on barriers for migrants, particularly from non-English speaking backgrounds, in accessing travel health advice and the strategies that could be used to engage them. Methods: A qualitative study involving 20 semi-structured interviews was undertaken in Sydney, Australia between January 2013 and September …
Undergraduate Student Attitudes On Concierge Medicine, Sushane Gupta
Undergraduate Student Attitudes On Concierge Medicine, Sushane Gupta
Honors Theses
The purpose of this thesis was to examine the opinions of undergraduate students on a field of personalized primary care known as concierge medicine, as well as assess their satisfaction with their current non-concierge healthcare providers. Concierge medicine aims to provide patients with a high level of customer service and satisfaction, and in exchange for an annual fee, they receive benefits such as lower waiting times for appointments, access to the physician by phone or e-mail, and a stronger patient-physician relationship focused on preventative care. The current literature on non-concierge healthcare reveals several deficiencies including poor insurance coverage, poor patient …
The Impact Of Nursing A Significant Other In The Course Of Employment, Tania Arnold
The Impact Of Nursing A Significant Other In The Course Of Employment, Tania Arnold
Theses: Doctorates and Masters
This thesis describes the experiences of nurses and midwives working in metropolitan hospitals who cared for a significant other in their role of employment.
The absence of research directly related to this topic guided the exploration of individuals’ experiences to establish base line knowledge relating to this phenomenon.
This study used descriptive research to provide information relating to the personal and professional effects on nurses who were required to, chose to or had no choice but to care for a significant other in their role of employment. An on-line survey gathered demographic, Likert scale responses to evaluate impact on care, …

Research Brief: "Preferences For Gender-Targeted Health Information: A Study Of Male Veterans Who Have Experienced Military Sexual Trauma", Institute For Veterans And Military Families At Syracuse University
Institute for Veterans and Military Families
This brief is about male veterans' preferences for health information provided to them after experiencing military sexual trauma and the effect of that information on use of mental health services. In policy and practice, veterans who have experienced military sexual trauma should seek military sexual trauma care from the VA, health practitioners should learn more about military sexual assault to better help servicemembers, and gender-targeted literature should be distributed to servicemembers who have experienced military sexual trauma; the VHA should include gender-specific resources for male veterans who experienced military sexual trauma. Suggestions for future research include the addition of a …

Malnutrition is common in long-stay elderly hospitalized patients and their dietary intakes are often poor, despite the provision of adequate quantities of food to meet patient needs. The aim of this study was to identify environmental factors that were associated with achieving adequate food consumption in a hospital context. This study observed the daily routines of 30 elderly patients over 2 days in rehabilitation wards in three Australian hospitals. All activities associated with mealtimes were recorded, from the commencement of breakfast to the conclusion of supper at the end of the day. Four key themes emerged: the eating location; assistance …

Beyond The Rhetoric: What Do We Mean By A 'Model Of Care'?, Patricia Davidson, Elizabeth Halcomb, L Hickman, J Phillips, B Graham
Elizabeth Jane Halcomb Professor
Background: Contemporary health care systems are constantly challenged to revise traditional methods of health care delivery. These challenges are multifaceted and stem from: (1) novel pharmacological and non-pharmacological treatments; (2) changes in consumer demands and expectations; (3) fiscal and resource constraints; (4) changes in societal demographics in particular the ageing of society; (5) an increasing burden of chronic disease; (6) documentation of limitations in traditional health care delivery; (7) increased emphasis on transparency, accountability, evidence-based practice (EBP) and clinical governance structures; and (8) the increasing cultural diversity of the community. These challenges provoke discussion of potential alternative models of care, …

Aims & rationale/Objectives Population ageing poses major challenges for health systems. Additionally, training future general practitioners in the management of older and chronically ill patients is potentially hampered by the reluctance of these patients to consult trainees for chronic care. This paper reports a cross-sectional study investigating the attitudes of older patients to trainees, to inform strategies to improve older patient-trainee interaction. Methods The survey instrument was distributed to 1900 patients aged 60 and over from 38 training practices from five Australian states using a stratified, randomised cluster sampling process. Generalised estimating equation models were used for analysis. Principal findings …
