In Kenya, recent trends in contraceptive use among currently married women aged 15–49 years show that after experiencing a stall between 1998 and 2003, the contraceptive prevalence rate (CPR) increased between 2003 and 2008–2009. Although the increase in CPR after 2003 is encouraging, there is limited understanding of how these changes affected adolescents, especially girls, given the unique sexual and reproductive health challenges that they face. Young adolescence is a critical life-cycle stage marked by a dynamic combination of opportunity and risk. During this time, children undergo tremendous physical, emotional, social, and cognitive changes as well as socialization into prevailing sexual and gender norms. For the majority, young adolescence is characterized by relatively good health and stable family circumstances, but it can also be a period of vulnerability due to a number of rapid transitions that force some young people into adult roles. The Population Council’s extensive work on adolescent safe spaces and livelihoods programs has demonstrated the pathways of empowering adolescent girls and the substantial benefits for society. Monitoring and evaluation data from these initiatives have produced empirical evidence about threats to girls’ health and well-being that relate to both longstanding and emergent social forces. In developing countries, where the risk of adverse sexual and reproductive health outcomes is high and young girls are at high risk, it is important to address issues related to sexual and gender-based violence (SGBV) and gender norms that relate to these risks. The international development community’s desire to alleviate poverty and improve health outcomes presents an extraordinary opportunity to transform the lives of young people, particularly girls. Cycles of illiteracy, poor job prospects, and social isolation can be broken but will require concerted efforts to reach large numbers of vulnerable adolescent girls with robust asset-building programs. Scaling-up effective pilot programs will be critical to achieving these goals. As a pioneer in the relatively young field of adolescent livelihood programs, the Population Council has implemented and evaluated a range of projects that test vocational training, savings, financial education, and credit initiatives. Since the 1990s, the Council has documented the importance of adolescents’ livelihoods and worked to adapt adult livelihood models to fit adolescents’ unique needs. Guatemala’s population is poorer, less educated, faster growing, and more ethnically diverse than most others in Latin America. The country’s fertility and infant mortality rates are among the highest in the region. Nearly half of the Guatemalan population is indigenous, and more than 20 Mayan ethnic groups live in isolated rural areas with limited access to basic services such as water, sanitation, schooling, and health care. Mayan girls are the country’s most disadvantaged group, leading lives characterized by limited schooling, early marriage, frequent childbearing, social isolation, and chronic poverty. In the next ten years, if current patterns continue, more than 100 million girls will marry before the age of 18, according to Population Council analyses of United Nations country data. Child marriage—any marriage that occurs before age 18—is considered a human rights violation by international convention. Marriage transforms virtually all aspects of girls’ lives. Typically, girls who marry are moved from their familial home and village, lose contact with friends, initiate sexual activity with someone they barely know, and soon become mothers. Education, particularly at the primary level, has long been regarded as an essential input of social and economic development. In addition to its role in increasing the cognitive abilities of students, schooling is recognized as a facilitator of labor force participation, female empowerment, improved health outcomes, and greater civic engagement. As one of the early pioneers in sub-Saharan Africa of free primary education, Malawi’s enrollment figures compare favorably with those of its neighbors in southern Africa. Ghana’s population of 24 million is projected to reach 31.8 million in 2025. This population growth is coupled with rapid urbanization. Urban adolescent females experience a rapid decline in school enrollment, often because they need to provide economic support to families. Early marriage occurs frequently in rural areas of Burkina Faso and curtails girls' ability to complete their education and fulfill their potential. Once married, girls tend to be less mobile and have limited social networks compared to unmarried girls, and married girls are expected to begin childbearing almost immediately. Given their young age, pregnant adolescents may be at risk of complications during childbirth. Despite the large number of married girls and girls at risk of early marriage in Burkina Faso’s rural areas, few programs address the issue of early marriage directly. In 2001, the Population Council and partners designed and implement an intervention program to address the needs of out-of-school adolescent girls in rural Upper Egypt. As noted in Promoting Healthy, Safe, and Productive Transitions to Adulthood Brief No. 12, the pilot, referred to as Ishraq (“Sunrise”), was launched in four rural villages of el-Minya governorate in Upper Egypt, the country’s least developed and most disadvantaged region. In 2001, UNICEF initiated a pilot intervention to test whether livelihoods opportunities could ameliorate early marriage and other adverse outcomes for girls in rural Bangladesh. Kishori Abhijan (“Adolescent Girls’ Adventure”) aimed to lower school dropout rates, increase girls’ independent economic activity, and raise the age at which girls marry. Life-skills training consisted of enhancing self-esteem and leadership skills and providing education related to gender roles and discrimination, health and nutrition, and legislation and legal rights, particularly early marriage and girls’ and women’s rights.
