This report documents results of a systematic study of peer-reviewed and “grey” literature which identified studies relevant for understanding barriers to accessing obstetric fistula (OF) care in developing countries. To more comprehensively and holistically understand these barriers in Nigeria and potential enabling factors for alleviation, this study explored the perspectives of those involved in both identifying fistula and accessing care, such as family members and other community stakeholders including patriarchs, matriarchs, community leaders, and other opinion leaders. The 2015 Kenya National Adolescent Sexual and Reproductive Health Policy recognized the importance of addressing adolescent sexual and reproductive health (SRH) needs for achieving the country’s development goals. Although estimates from the 2014 Kenya Demographic and Health Survey (DHS) show the country registered improvements in key reproductive, maternal, and child health outcomes, adolescent outcomes likely remain poor given the challenges associated with adequately addressing their sexual and reproductive health needs. The pilot intervention described in this brief took place at an apparel manufacturing company, Caribbean Island Apparel SA, located in Port-au-Prince, Haiti. The project by the USAID-funded RAISE Health Initiative sought to demonstrate the value of a new management approach to workplace health services and practices in a garment factory. An initial study conducted by Population Council under the Strengthening Evidence for Programming on Unintended Pregnancies (STEP UP) project in 2012 assessed the knowledge and reproductive health needs of adolescents living in selected slums in Brong Ahafo and Greater Accra regions with the view of informing an improvement in adolescent sexual health (ASRH) programming in Ghana. The study concludes that stakeholders generally believed there was the need for enhanced adolescent sexual and reproductive health services in school as the present system of delivering these services were not sufficiently addressing ASRH needs. A formative research study was conducted in Nigeria to better understand the barriers and enabling factors for fistula repair care delivery and access. This brief presents results from a literature review conducted to identify and understand the delays in accessing and receiving treatment and corresponding barriers, to document interventions that help to overcome barriers, and to specify gaps in the literature that require further research. The Indian National Health Insurance Scheme, Rashtriya Swasthya Bima Yojana (RSBY), was launched by the Ministry of Labour and Employment in 2008 to promote equitable access to health services through the private and public sectors. This brief describes a formative research study conducted in Uganda by the Population Council, building on the results of a systematic review, to better understand the barriers and enabling factors for fistula repair care delivery and access. Understanding how Ugandan women living with fistula decide to seek care, identify and reach medical centers, and receive adequate and appropriate care is integral and continues to influence the design of a larger implementation pilot. The objective of this study was to better understand the longer-term effects of a three-day training program offered by Phase III of Pathfinder’s PRACHAR (Promoting Change in Reproductive Behaviour) program among adolescents in rural areas of selected districts of Bihar, India. This program focused on addressing adolescents’ need for information, contraceptive supplies, parental and community support, and a youth-friendly health system. Findings confirm that the training program was acceptable and useful to the young people exposed to it, and that it had a number of notable longer-term effects, observed even four years following its implementation; and they demonstrate the promise …

Pakistan is one of the six countries that account for more than 50 percent of the world’s maternal deaths. According to Population Council estimates, each year nearly 8.6 million women become pregnant in the country. Of these, 1.2 million women are likely to face obstetric complications. This formative research builds upon the results of a systematic review by the Population Council for better understanding of the barriers and enabling factors for fistula repair care and access in Uganda. The study focuses on Fistula Care Plus project-supported treatment facilities where fistula camps are routinely held. Results reveal the nuances of women’s experiences, along with the additional perspectives of their spouses, family members, community stakeholders, and fistula camp care providers including nurses/counselors, surgeons, and facility and district managers.
