The Influences Of Medicaid On Mental Health Practitioners, Matthew Mohan

Medicaid is the largest publicly funded health insurance program which influences the mental health practitioners’ treatment of low income individuals (Altman & Frist, 2015; Rowland et al., 2003). To understand how Medicaid influences mental health practitioners, this study utilizes institutional theory by using the five propositions from Lammers and Barbour (2006) to analyze how Medicaid’s communicative practices influence mental health practitioners who treat those who are in poverty. Semi-structured interviews were conducted with 10 mental healthcare practitioners who accepted Medicaid: two psychologists, five social workers, one psychiatrist, and two psychiatric nurse practitioners. Background: Nevada has had consistently high rates of youth suicide in the past decade with 21.2 deaths by suicide per 100,000 population (ages 15-24), over the national rate of 14.5 deaths per 100,000 in 2019. National strategies, state agendas, and best practices in the field prioritize research on upstream strategies to prevent suicide. One such strategy is the use of social emotional learning (SEL) programs for youth to help increase and enhance protective factors against suicide, helping them to cope with negative life events. Over 46 million adults in the United States (U.S.) live with a mental illness. Locally in Nevada, the psychiatrist to patient ratio is approximately 700:1, and patients are seen primarily in the inpatient setting, if seen at all, due to the wait average of 85 hours in the hospital emergency rooms for access into mental health hospitals or specialty units of a general hospital. Moreover, costs for mental health mood disorders alone are estimated at over seven million dollars annually. Depression is a serious illness characterized by persistent low mood, reduced cognitive capacity, and fatigue. Although treatable, depression is the leading cause of disability and ill-health worldwide and a significant contributor to suicide, the second leading cause of death for young Americans. In any given two-week period, 8.1% of adults in the United States had moderate to severe depression (2013-2016). The Sport Interference Checklist (SIC) is a psychometrically validated instrument designed to assess how often cognitive and behavioral factors interfere with athletes performance during training and/or competition as well as the extent to which athletes are interested in pursuing sport psychology to address these problems. The success of this scale inspired an interest in developing new items that assess the influence of specific mental health concerns on sport performance using the SIC format. Athletes at the collegiate level frequently experience unique stressors that cause them to be at risk for a number of mental health difficulties, including depression, anxiety, and substance use. Current research in the field suggests that athletes are not as likely as their non-athlete peers to seek out psychological services for mental health difficulties. Social supports have been shown to impact athletes’ mental health and sport performance. Specifically, family relationships appear to have an influence on athletes’ level of stress and motivation, with positive family relationships showing decreases in athletes’ worry as well as faster recovery following injury. There is limited research on Latino LGBQ individuals and their coming-out experiences. To understand the coming out process of Latino LGBQ individuals, interviews were conducted with 10 Latino LGBQ young adults between the ages of 18 and 30 years old. Nevada’s competitiveness in receiving federal funding has been a serious and contentious topic that has steered recent efforts to ascertain ways the state can increase and utilize federal dollars efficiently. Nevada is positioned 53rd out of 53 U.S. states and territories in bringing federal dollars to the state, missing out on $1.5 billion annually (Gustafson, 2012). Nevada spends far less on Medicaid (per capita) than any other state. Medicaid is a key factor in improving Nevada’s position; however, there are additional non-formula funding opportunities that are missed every year.
