Unending Mazes: Gendered Inequalities, Drug Use, And State Interventions In Rural Appalachia, Lesly-Marie Buer

Prescription opioids are associated with rising rates of overdose deaths and hepatitis C and HIV infection in the US, including in rural Central Appalachia. Yet there is a dearth of published ethnographic research examining rural opioid use. The aim of this dissertation is to document the gendered inequalities that situate women’s encounters with substance abuse treatment as well as additional state interventions targeted at women who use drugs. These results are based on ethnographic fieldwork completed from 2013 to 2016 and centered around one county seat in rural Central Appalachia. This research utilizes an agency framework to examine the complexities of the participation of indigenous women in local, national, and global spaces of activism. By examining the connections between processes of globalization of indigenous and women’s rights, development agendas, local politics, and gender dynamics in indigenous organizations, this research highlights the connection of ethnicity, gender, and power in an indigenous organization of Cotacachi, Ecuador, and for Ecuadorian and Latin American indigenous leaders and professionals working in national and global arenas.

This research on healthcare strategies of home-based, low-wage, immigrant careworkers contributes to the ways medical anthropology, migration studies and social science understand human-economy-family care relationships and health and carework as commodities in today's global economy. It reveals the consequences for workers as they defray the costs of care for the Italian government and contribute to their home economies.
