The deminer was investigating the signal and excavating by sitting/squatting position and his prodder/bayonet detonated the PMN mine. The evidence on site shows that the excavation of the signal started from the first mark, and not 30cm back from the indication. The bayonet directly touched the mine causing a detonation. This indicates he was not conducting operations with the correct procedure.

On 10/12/2007 as the team was busy with normal demining activities, suddenly a dog of the villagers run harshly into the minefield and as the dog reached to 6m distance from a deminer's clearance lane, suddenly an explosion happened to the dog which perished it on the spot. As a result of the explosion [the Victim] deminer who was busy on marking of his cleared lane got slight injuries on his both hands and as he was dressed fully with the PPE, he did not receive any more injuries to other parts of his body.

After the routine break at 1140 – 1150 hrs, the deminer was advised by his Section Leader (SL) to mark his lane clearly so that the SL could conduct his own QC checks. The victim deminer was walking around and wanted to cross the boundary of the MF using unclear area that suddenly he stepped on an AP mine and caused it to explode. MF AF/0308/01634/MF163 is located at Jan Qadam village of Bagram province. This area is contaminated by anti personnel mines laid by Russians over the period from 1982-1985. These mines have been laid as belt to secure Bagram airbase from Mujahidin attacks. The density of mine contamination in this area is very high. [The Victim] deminer was working in a bushy area with MIL-D1 mine detector and after he pinpointed a signal he put the detector away and started excavation of the signal by using a pick, and within a few minutes the pick stroke on the top of an AP mine and the explosion happened. On 18 June 2007 at 1140 hrs (local time), [the Victim] a (Commercial demining group) Deminer, inadvertently caused an item of explosive ordnance (EO), which is considered to be a No. 4A anti-personnel mine, to activate, while conducting the manual mine clearance full excavation procedure. As a result of the explosion [the Victim] was thrown backwards by the blast wave and suffered a traumatic amputation of the right hand, a fractured right humerous and critical head and eye injuries.
