Racial/Ethnic Disparities In Mental Health Diagnosis Of Maltreated Youth, Shadie Burke

Racial/ethnic marginalized youth are more likely to receive a misdiagnosis of a mental health disorder than their White peers. Misdiagnosis contributes to existing disparities for racial/ethnic marginalized groups by leading to longer durations of untreated and worsening mental health symptoms. For maltreated youth, receiving a misdiagnosis can lead to a failure to connect with appropriate mental health services, medication regimens, therapeutic foster homes, and academic and developmental resources. To date, very few empirical studies have identified disparities in mental health diagnosis among racial/ethnic marginalized maltreated youth. Research suggests that some fathers and birth partners can experience post-traumatic stress disorder (PTSD) after witnessing a traumatic birth. Birth-related PTSD may impact on many aspects of fathers’ and birth partners’ life, including relationship breakdown, self-blame and reducing plans for future children. Despite the potential impact on birth partners’ lives there is currently no measure of birth-related PTSD validated for use with birth partners. The current study therefore adapted the City Birth Trauma Scale for use with birth partners. Research on childhood posttraumatic stress disorder (PTSD) has increased over the past decade. Youth who experience maltreatment are more likely to exhibit symptoms of PTSD. At present, the literature largely focuses on youth who have experienced general trauma or PTSD symptoms, while maltreated youth are understudied. To date, no studies have examined the psychometric properties of the Adolescent Dissociative Experiences Scale (A-DES) or the Posttraumatic Cognitions Inventory (PTCI) in a sample of maltreated youth. Schizophrenia (SZ) and Posttraumatic Stress Disorder (PTSD) are often comorbid psychological disorders. Separately, research has previously demonstrated deficits in social cognition, including affect recognition impairment in both disorders; however, less is reliably known about the specific deficits unique to a comorbid diagnosis of SZ and PTSD. This study examined the ability to correctly identify facial expressions in individuals diagnosed with SZ (n = 38) and PTSD alone (n = 20), comorbid SZ and PTSD (n = 26), and healthy controls (n = 28). Certain individual factors may heighten a youth’s risk for PTSD symptoms following maltreatment (Kearney et al., 2010). This study aimed to identify maltreated youth at highest risk for re-experiencing, avoidance, and hyperarousal symptoms of PTSD. This study evaluated interactions among several demographic, affective, and cognitive variables to determine which subgroups of maltreated youth are most likely to manifest diagnostic PTSD symptom presentations in the aforementioned clusters. Participants included 400 ethnically diverse youth aged 7-18 years from Department of Family Services (DFS)-related sites in Las Vegas. Resilience following exposure to adverse life situations is an ongoing process that reduces the impact of traumatic experiences and opens potential for posttraumatic growth (Fincham, Altes, Stein, & Seedat, 2009; Luthar, Cicchetti, & Becker, 2000). The relationship between resilience and risk for PTSD in maltreated youth, however, remains unclear. Childhood maltreatment is associated with increased risk for lifetime and current PTSD (Davis & Siegel, 2000; Widom, 1999). This study examined a model of PTSD in which dissociation, depression, and posttraumatic cognitions related to heightened PTSD symptoms for maltreated youths. This study then evaluated the model's fit across variations in intelligence, gender, age, ethnicity, and maltreatment type. Participants (n=360) included youths from Department of Family Services-related sites in Las Vegas. The first hypothesis was that the model would display goodness-of-fit across various indices.
