On the 13th Dec 2010 at 10:37, the deminer [Victim No.1] was operating in his clearance lane excavating a detected signal, his prodder touched on a subsurface tripwire of POMZ mine which caused it to explode. According to the investigation report the signal was not pinpointed correctly and the de-miner has used pick in contrary to set procedure, during the excavation, he pulled the subsurface tripwire connected with POMZ mine. The accident resulted in superficial injuries to deminer’s hands, left thigh and left side of abdomen.

The clearance operation on mentioned task was started by [Demining group] DT-12 on 1st of November 09. According to [the Victim] deminer that he was searching (removing the soft soil) which backhoe has prepared with a small shovel and he pulled out the bush where barbed wire existed, while he pulled the bushes suddenly an accident occurred.

The investigators reported that the victim was working normally and looking for tripwires. He found a tripwire and wanted to find whether it was connected to a mine "so he pulled it with his hand and the mine went off". He received injuries to his right armpit, both thighs and suffered hearing loss in his left ear.

On the day of the accident the victim was working in a bushy area when at 09:00 he pulled a bush out of the way. In doing so he pulled a tripwire and initiated a POMZ. He suffered hand and chest injuries, and hearing loss. The victim was treated at the site, then taken to the ICRC hospital in Ghazni for treatment.

The investigators determined that at around 11:00 the Section Leader instructed the deminers to pack up and rest until the end of the day. While they were resting Victim No.1 went into the uncleared area for no known reason. When he was 15m into the mined area he noticed a tripwire and "started wrapping it into the stick of a POMZ". A POMZ detonated at 11:30.

The victim said that he had cleared a 30cm section ahead of him and then moved the end-of-lane marker. He suddenly felt dizzy and accidentally put his hand into an uncleared area. The investigators determined that Victim No.1 was scratching a mark to show the end of the breaching land outside the area actually cleared (he should have left a 50cm safety margin the other way) when his bayonet pulled the tripwire attached to the mine. The mine was three metres away from the deminer when it was initiated. They thought that Victim No 2 did not maintain the proper safety distance and so was hit by some fragments.

The investigators determined that the victim was in a known POMZ minefield but neglected to use a tripwire feeler before advancing beneath a big bush with the detector, so pulled a tripwire with the detector head. It was recorded that there was "No damage to equipment", but photographs of a damaged Schiebel detector were included in the report.

The investigators determined that the victim got a detector reading and started prodding in a prone position. As he removed loose soil he pulled a tripwire with his hand. A POMZ six metres away was initiated. The device was identified as a POMZ from "found fragments".

The investigators determined that the minefield was old and the mines had fallen and become buried. The victim did not mark the detector reading point before investigating with a pick, so triggered the mine which was identified as a POMZ from "fragments found". The victim's pick and helmet were damaged.
