According to the deminer statement: He starts his work. At 6.45 AM. After short break for 15 minutes from 7.30 to 7.45 he restart his work and found an AP mine approximately at 7.55 AM .The mine was RSP by the team Leader and he restart his work after few minutes his detector indicate a signal and after pin pointing the signal he use the prodder and start excavating the ground with his shovel. While he was excavating his visor fall down and simultaneously he activate the buried mine with his shovel. (

The 26th of January 2007, approx 12.25 (local time SL), the HQ in Jaffna was called up by the Team leader of Team 5, working in a heavy dense (P4s) minefield, LK-23B.The message he gave was that one of his deminers had set one mine of in his lane while conducting mine clearance. The mentioned deminer was taken out of work for the rest of the day. The minefield consists of rocky ground with both loose rock and solid/fixed rock.

Deminer [the Victim] started demining in his lane no. 47. He set off a P4 MK1 antipersonnel mine with his right foot by stepping on it 07:10 am. Explosion occurred 2 meters from the end of the 27 meter long cleared lane in already cleared area. Deminer fell down to his front. He was not injured except for small scratch in his little finger due to falling. His boots did not suffer any damage.

Deminer [the Victim] used his rake in normal manner. He set off a P4 MK1 antipersonnel mine with his 2-tooth heavy rake. Heavy rake bended but remained in one piece in the blast. Deminer was in perfect health and didn’t get bruises nor other minor injuries.

At 09.10hrs on the 22nd of September 2004, deminer [the Victim] was carrying out manual clearance using the prodding method on the northern side of Minefield 0011 at Ina Guuxaa. Whilst excavating earth at the front of his lane, he inadvertently detonated what is suspected to have been a P-4 anti personnel mine.

There is no definite explanation for how the incident happened. According to deminer, he used his tools slowly and correctly letting the rake to come towards himself by its own weight while pulling. Visual appearance of the incident lane supports this claim. Deminer’s raking tracks are visible, 50 cm in length and symmetrical.

Experienced deminer uses his rake slowly and in a correct manner according to SOP. Suddenly he sets off an antipersonnel mine with his heavy rake. Heavy rake breaks into two pieces in the blast. Deminer remains in perfect health and doesn’t have bruises nor other minor injuries.

Experienced deminer sets up an antipersonnel mine with his heavy rake. There are two one inch thick bush roots lying horizontally in depth of four centimetres, twenty five centimetres from each other. Antipersonnel mine was situated between the roots. Instead of prodding the spot between the roots, deminer continued raking with heavy rake.

Antipersonnel Mine model P4 mark 1 exploded during mine-clearance in minefield LK-257, Katkovalam. Explosion took place 1,5 meters in front of [Demining group] deminer while he was raking the uncleared lane with heavy rake. The BOI could not establish conclusively why the accident took place, as no eyewitnesses saw the incident at the time of the explosion. However, it was obvious that the deminer had his visor up at the time of the explosion. Damage and blast marks to the inside of the visor could only have accrued if the visor was up at the time of the explosion. The lane followed a P4 AP blast mine strip where [the Victim] had found three mines the previous day. I am the section leader of Team 07 – 2 Southern Group at 10.15 while we all were at rest area on rest suddenly heard an explosion and saw smoke coming from close to bund. Details of this accident have been withheld by the demining NGO that employed the Victim. A spreadsheet including the Victim’s name and very brief details of the accident was made available in 2007. Some details can be inferred from the information released. For example, the limited injury implies that the victim’s PPE was being worn in the correct manner.

Narrative: The deminer [Name excised] was carrying out one man one lane manual demining drills in accordance with [Demining group] Sri Lanka manual demining SOPs for 100% excavation. He was using the axe hammer tool to excavate the face of his lane down to a depth of 15cm, when the detonation occurred. The mine detonated on the left hand side of his lane as he was using the axe hammer.

The victim was working normally and got a reading from his detector which the investigators believe must have been from a fragment beyond the mine (they considered the mine undetectable with a Schiebel AN/19 detector). He marked the reading and while prodding towards it detonated the mine at 08:15. The victim received "superficial injuries on his right hand, forearm, fingers and neck".

The investigators determined that the victim was working normally and registered a detector reading, so marked the spot and began prodding towards it. Before he reached the marked point an undetected mine detonated at 11:15. They found that he was working in a squatting position without wearing his helmet. They also found that he "ignored" correct prodding procedure and so applied pressure directly onto the pressure plate of the mine.

This accident involved a “P2MK2” minimum metal blast mine. [P2MK1 and P2MK2 mines are frequently given the same last number.] A letter explaining why it was not identified at the time was found on file in September 1999 - stating that the same mine occurred in two accidents at the site and that survey had not thrown up the presence of minimum metal mines.
