The four Explosive Ordnance Disposal Marines were clearing the range of unexploded ordnance and consolidating it as part of an annual training exercise, according to a Marine report about the results of the investigation. The Marines moved 40-millimeter rounds into one area surrounding an M60 105-millimeter white phosphorous round that could not be safely moved, the investigation found. They were moving rounds from the consolidation point to the demolition pit when one of the rounds exploded in the demolition pit, causing the other 40- millimeter rounds at the consolidation point and the demolition pit to explode, the investigation found.

Deminers reported that the Victim was in a particularly happy mood that day. The Victim went to the grenade without telling anyone and began to dismantle it. His actions can be reliably inferred because the black cap of the grenade was undamaged at the site (and featured in the first site photographs after the accident). So it seems that the Victim got some way into the grenade before it went off in his hands. The deminer was instructed by his Section Leader to move the UXO and place the item concerned in the demolition pit, which was located 21 metres away from the deminer’s working area / lane. On the way to the demolition pit the deminer accidentally dropped the UXO and according to the Section Leader, the deminer dropped the UXO directly on to stone that was in the ground and subsequently the UXO detonated. On 1st Nov 2009, at 02:45 pm during the rest time, [the Victim] a deminer of mentioned team found an unknown item and tried to touch it. According to the investigation report, the item was a rifle Grenade of 40mm which was exploded as a result of irresponsible action of the victim deminer. As it was the break time of the team and the deminer was without visor and PPE, therefore, he lost his right hand, left eye and sustained jaw and leg fracture.

Based the [International demining NGO] OPS Officer verbal report the injured Deminer was during the prodding /excavation at Task No 1144 in Khoja Kamal e Wali area to initiating the unexploded Garnet when the prodder touched with a sensitive part of this buried fuse and blow up, as per the explanations of OPS Officer he was recognized the initiation of fuse for explosion when take a special sound for its readiness so Deminer started to gotten away when the garnet exploded and the fragments beached to his body specially in his hand , shoulder and leg.

The accident occurred in a mined area that had been worked on at the provincial government request since January 10th 2000. The method used was conventional manual demining. At 06:51 “while working in a dense vegetation area, the deminer used a machete to cut some branches “… He dragged the cut branches out and heard a “strange click”. He ran about “15 metres into a cleared area and took cover a few seconds before the F1-grenade exploded”.

The investigators determined that the victim got a detector reading and marked it, then put on his helmet and started to prod. He struck the fuse of a grenade with his bayonet and heard a "fuse detonation sound" [click of a delay mechanism arming] so was scared. He stood up.

At 08:30 the team started work. The victim was clearing the central lane of three lanes 25 metres from each other. Vegetation was sparse where he was working and surface munitions or mines could have been easily seen. The accident area had been cleared in the previous year and various UXO had been found, including half-buried mortars and M77 grenades. A seasonal wind had buried them. The victim was walking over the cleared area at 17:15 from his "base camp" when he initiated the device. He "lost" both legs and suffered superficial injuries on his abdomen and hands.

The investigators determined that the victim got a reading on a "hand grenade or its wire" and his partner came to investigate the reading with a long handled shovel. When he started to "cut/remove the bushes, safety pin of the hand grenade got out". The deminer ran away and after a few seconds the grenade exploded. The device was identified as a hand grenade from "found fragments".
