Identifying Risk Factors For Ptsd Symptom Clusters In Maltreated, Multiracial Youth Using Nonparametric Modeling, Mallory Constantine

Multiracial youth are one of the most at-risk racial groups for child maltreatment. Multiracial individuals are also more likely to report mental health concerns than other racial or ethnic groups. This study aimed to identify demographic and psychological risk factors that are unique to multiracial, maltreated youth with respect to PTSD symptoms (i.e., re-experiencing, hyperarousal, avoidance) through classification and regression tree (CART) analyses. Participants included 99 multiracial, maltreated youth directly following their placement in an emergency group shelter due to substantiated maltreatment. BackgroundInsomnia impacts 30% of people and is associated with increased risk for medical and psychiatric illness. However, pharmacological treatments for sleep yield mixed results and often result in undesirable side effects indicating a need for novel insomnia interventions. Sleep patterns also demonstrate high intra-individual variability which may be associated with greater daytime impairment, and sleep-wake cycles are strongly affected by the timing of exposure to blue-green wavelength light. Virtual darkness, a novel intervention, blocks blue wavelength light from reaching the eye to simulate actual darkness on retinal cells sensitive to light. Racial/ethnic marginalized youth are more likely to receive a misdiagnosis of a mental health disorder than their White peers. Misdiagnosis contributes to existing disparities for racial/ethnic marginalized groups by leading to longer durations of untreated and worsening mental health symptoms. For maltreated youth, receiving a misdiagnosis can lead to a failure to connect with appropriate mental health services, medication regimens, therapeutic foster homes, and academic and developmental resources. To date, very few empirical studies have identified disparities in mental health diagnosis among racial/ethnic marginalized maltreated youth. Ethnically and racially diverse youth athletes underutilize mental health services, resulting in social and economic costs to society (Gudiño et al., 2008; Snowden & Yamada, 2005). Almost half of all children and adolescents participate in sport, and sport participation is especially common in youth from ethnically and racially diverse neighborhoods (Holt, 2008; NSCH, 2017). Schizophrenia presents as a heterogenous disorder characterized by diverse expression of symptoms between individuals. Thus, clinicians and researchers rely on comprehensive symptom measures for diagnosis and treatment. This dissertation investigated two symptom measures, the Positive and Negative Syndrome Scale (PANSS; Kay, Fiszbein, & Opler, 1987), a commonly used scale that assesses a broad range of symptoms of schizophrenia, as well as the Brief Negative Symptom Scale (BNSS; Kirkpatrick et al., 2011; Strauss et al., 2012), a novel scale that focuses on the assessment of five negative symptom domains (i.e., anhedonia, asociality, avolition, blunted affect, and alogia). Research suggests the presence of a non-relational aspect of women’s sexuality wherein a focus on oneself may be linked to elevations in sexual desire/arousal. Some theorists have even asserted that women may be, in some ways, their own sexual objects. Building on a previous exploration of what is now termed Erotic Self-Focus (ESF; Fertel, 2015), the present study developed a refined measure of ESF to further assess the validity and nature of this new construct. An exploratory factor analysis revealed four factors: Arousal to Own Body, Self/Own-Gender Focus vs. Partner/Other-Gender Focus, Self-Focused Arousal – Physical, Self-Focused Arousal – Mental. Fewer cases of COVID-19 and hospitalizations have been reported in children. This has impacted the prioritization of pediatrics in understanding the infection, transmission, and treatment of SARS-CoV-2 in children. Unfortunately, COVID-19 rates are higher among racial and ethnic minoritized children. Simultaneously unfolding during this pandemic is a national outcry to address systemic injustice, including institutional racism in healthcare which are driving these disparities. Selective mutism is a persistent and debilitating psychiatric disorder in which a child fails to speak in situations where speaking is expected. Although listed as an anxiety disorder, the multifaceted and heterogeneous nature of selective mutism indicates that a more accurate conceptualization may be as a neurodevelopmental disorder. Adolescent athletes with Attention Deficit Hyperactivity Disorder (ADHD) and Oppositional Defiant Disorder (ODD) experience unique challenges that impact their sport performance, such as making errors due to poor concentration and adverse relationships with referees due to poor anger management. Pharmacological treatments have shown to be successful in treating ADHD comorbid with ODD. However, there are negative consequences for pharmacological treatments for adolescent athletes (e.g., banned by sports organizations). Well-established behavioral interventions for ADHD and ODD in non-athlete populations have included family-based, cognitive-behavioral approaches.
