Many immunization programs in developing countries arr now undergoing a period of rapid utilization. Innovative strategies, such as immunization holidays, are being tried. Political commitment and enthusiasm are high. There is a strong desire by health professionals to protect the lives of many children from the untimely death and/or disability resulting from infectious diseases preventable by vaccination.
Evaluation of an immunization program can benefit other elements of the health system, particularly those that are not amenable to objective evaluation as immunization. In this case, immunization can serve as an indicator of the overall performance of the health system. The problems …

Expert system technology has proven itself to be able to improve productivity and decision making in a variety of fields of endeavor. The purpose of this study was to determine if an expert system for medical evaluation boards (MEBs) could improve upon the processing of medical evaluation board cases. Research was conducted to develop an expert system and measure the capabilities of the system, both in terms of accuracy and timeliness of case completion of the medical evaluation administrators/clerks who were assisted by the system. There has been considerable uncertainty in estimates of past and current human immunodeficiency virus (HIV) infection rates in the United States. Statistical estimates of historical infection rates can be obtained from acquired immunodeficiency syndrome (AIDS) incidence data and the incubation period. However, this approach is subject to a number of sources of uncertainty and two other approaches, epidemic models of HIV transmission and surveys of HIV prevalence, are used to corroborate and refine the statistical estimates. Analyses suggest the HIV infection rate in the United States grew rapidly in the early 1980s, peaked in the mid-1980s, and subsequently declined markedly. …

This paper analyzes the medical knowledge required for formulating decision models in the domain of pulmonary infectious diseases (PIDs) with acquired immunodeficiency syndrome (AIDS). Aiming to support dynamic decision-modeling, the knowledge characterization focuses on the ontology of the clinical decision problem. Relevant inference patterns and knowledge types are identified.

The findings of recent investigations earmarked chronic daily stressors as being highly related to stress levels. In addition, researchers are promoting a more integrative model of stress. The stress response is no longer thought of in strictly linear terms, i.e. an event causes stress. Rather, cognitive factors, existing within the human mind itself, strongly influence our coping processes and the chronic stress we experience.
Canonical correlation analysis was used to study how dysfunctional attitudes, a negative cognitive factor, were related to individuals' daily hassles and coping processes. An increase in dysfunctional attitudes was found to correspond to an increase in …

Before being introduced to wide use, health status instruments should be evaluated for reliability and validity. Increasingly, they are also tested for responsiveness to important clinical changes. Although standards exist for assessing these properties, confusion and inconsistency arise because multiple statistics are used for the same property; controversy exists over how to measure responsiveness; many statistics are unavailable on common software programs; strategies for measuring these properties vary; and it is often unclear how to define a clinically important change in patient status. Using data from a clinical trial of therapy for back pain, we demonstrate the calculation of several …

Radiation due to x-ray exposure is a concern, and the development of the Dolphin™ DigiGraph represents an attempt to limit this exposure. The purpose of this paper was to compare measurements (using Ricketts analysis) obtained from conventional radiographic cephalometric tracings with those obtained by computerized digitizing with the DigiGraph. Forty-six patients from the Loma Linda University School of Dentistry graduate orthodontic clinic were digitized, and the results were compared to the x-ray tracings of these patients. The results showed that seven of the twenty-one variables examined exhibited a statistical similarity between the two methods (DigiGraph and x-ray tracing). An examination …

The purpose of this study was to compare the fracture strength of ceramic brackets which are currently on the market with ceramic brackets tested in a previous study at Loma Linda. Fracture strengths of standard edgewise ceramic brackets vs. pre-torqued ceramic brackets and bicuspid ceramic brackets vs. central incisor ceramic brackets were also compared.
A total of 160 Polycrystalline brackets, 8 categories in groups if 20, were tested for fracture strength. Each bracket was tested using the same testing model that was used in the previous study at Loma Linda. The brackets were tested by torquing a wire within the …

The pulps of mandibular molars of 15 athymic and 15 conventional rats were surgically exposed and left open to their oral flora. Each group was divided into 3 subgroups of 5 animals each. The rats were sacrificed after their pulps were exposed for 2, 4, or 8 weeks. After fixing, decalcifying, and embedding, the specimens were sectioned and stained with H and E. They were then examined under a microscopic grid and quantified by percentages of surface areas of bone, connective tissue, bone marrow, intrabony spaces, periapical lesions, and numbers of osteoclasts. Two-color flow cytometric analysis was performed on paired samples of peripheral blood (PB) and pleural effusions (PE) of patients with metastatic malignancies. The purpose of these analyses was to test the hypothesis that there are significant differences in the distribution of immunocyte populations in the PE compared to that of the PB.
The majority of the pleural fluid immunocytes were CD3+ cells. The ratio of CD4+ to CD8+ cells was higher in the PE compared to PB. Levels of CD8+, CD19+ and CD14+ cells were not significantly different in the PE compared to the levels in the PB. CD16+CD56+ cells …

A series of speech recognition experiments was conducted to investigate time-dynamic speech recognition of stop consonants invariant of vowel environment using data from six talkers. The speech preprocessing was based on previous studies investigating acoustic characteristics which correlate to the place of articulation (Blumstein and Stevens 1979). The place of articulation features were statistically abstracted using four moments and the energy level of the speech sample.
Both statistical and neural network pattern recognition methods were used. Statistical methods included linear and quadratic discriminant functions, maximum likelihood estimator (MLE) and K-nearest neighbors (KNN). The neural network approach used was Time Delay …

Estimating County Percentages Of People Without Health Insurance, Paula Diehr

County data on the percentage of people without health insurance are seldom available, although state program planning requires such information. As part of an evaluation of Washington's Basic Health Plan (BHP), we conducted a telephone survey in nine Washington counties to estimate the percentage of people under the age of 65 who were uninsured. We used regression analysis to estimate the percentage uninsured in a county as a function of the percentage unemployed. Two validation approaches yielded very good results, suggesting that the equation could be used to estimate the percentage uninsured in unsurveyed counties. The variation ranged from 15% …

Substance abuse in the U.S. has been estimated to cost $99 billion annually through lower productivity. Yet the authors urge caution in attempting to reduce these costs and health and safety Risks. In doing so, they cite commonly high frequencies of false negatives and false positives in employee drug tests - the latter having the potential to do great injustice to many drug-free employees.
