In the world of medicine and disease interventions, exercise is too often underlooked as a viable form of treatment or complementary treatment to medication. This is especially true with Parkinson’s Disease, a disease that attacks the basal ganglia of the brain and impairs neuromuscular function. In my analysis, I read a perspective from a PD patient, Gerry Hill, who has struggled with the disease himself and how he used physical activity to aid in his battle with the disease for both physical and psychological aid. Oxygen deprivation masks are being advertised towards athletes as a piece of fitness equipment that can simulate the effects of high altitude, as well as a respiratory muscle training device. Studies that investigated the claims as to whether ODMs increased VO2max and improved endurance were looked into. Experimental data did not find that the use of these masks increased VO2max, nor was there any supporting data to prove that it improved endurance. Muscle builders frequently consume protein supplements, but little is known about their effect on the gut microbiota. This study compared the gut microbiome and metabolome of selfidentified muscle builders who did or did not report consuming a protein supplement. Twenty-two participants (14 males and 8 females) consumed a protein supplement (PS), and seventeen participants (12 males and 5 females) did not (No PS). Participants provided a fecal sample and completed a 24-h food recall (ASA24). The PS group consumed significantly more protein (118 ± 12 g No PS vs. 169 ± 18 g PS, p = 0.02). Fecal metabolome and …

ผลของการฝึกโยคะร่วมกับการฝึกกระตุ้นระบบรับรู้การทรงตัวในหูชั้นในต่อการทรงตัวในผู้สูงอายุเพศหญิง, อาทิตย์ งามชื่น

จากการศึกษาที่ผ่านมา ไม่ได้มีเพียงแค่การฝึกเสริมสร้างความแข็งแรงของกล้ามเนื้อและร่างกายเท่านั้นที่ส่งผลต่อการเพิ่มความสามารถในการทรงตัว แต่ยังพบว่าการฝึกกระตุ้นระบบรับรู้การทรงตัวในหูชั้นในส่งผลต่อความสามารถในการทรงตัวที่เพิ่มขึ้นได้เช่นกัน เพื่อให้ครอบคลุมถึงกลไกการควบคุมการทรงตัว ผู้วิจัยจึงสนใจการฝึกกระตุ้นการทำงานของระบบประสาทสั่งการ vestibulo-ocular reflex pathway โดยมีวัตถุประสงค์เพื่อศึกษาเปรียบเทียบผลของการฝึกโยคะร่วมกับการฝึกกระตุ้นระบบรับรู้การทรงตัวในหูชั้นใน (YOGA+VSE) และ การฝึกโยคะเพียงอย่างเดียว (YOGA) ต่อความสามารถในการทรงตัวในผู้สูงอายุเพศหญิง ผู้เข้าร่วมวิจัยจะถูกสุ่มแบ่งออกเป็น 2 กลุ่ม คือ กลุ่ม YOGA+VSE (n=19) และ กลุ่ม YOGA (n=15) ทั้งสองกลุ่มจะได้รับการฝึกออกกำลังกาย 3 วัน/สัปดาห์ เป็นระยะเวลา 8 สัปดาห์ ซึ่งผู้เข้าร่วมวิจัยจะได้รับการทดสอบ Berg Balance Score, Time Up and Go, การควบคุมจุดศูนย์กลางแรงดันร่างกาย (center of pressure) ขณะยืน และการควบคุมจุดศูนย์กลางแรงโน้มถ่วงร่างกาย (center of gravity) ขณะเดิน ก่อนเข้าร่วมการฝึก หลังเข้าร่วมการฝึก 4 สัปดาห์ และ 8 สัปดาห์ ผลจากการศึกษา ภายหลังการฝึกออกกำลังกาย 8 สัปดาห์ พบว่าการควบคุมจุดศูนย์กลางแรงดันร่างกายขณะยืนกลุ่มที่ฝึกโยคะร่วมกับการฝึกกระตุ้นระบบรับรู้การทรงตัวในหูชั้นใน (YOGA+VSE) มีค่าความเร็วที่ใช้ในการเคลื่อนที่ของจุดศูนย์กลางแรงดันร่างกายลดลงอย่างมีนัยสำคัญทางสถิติเมื่อเปรียบเทียบกับกลุ่มที่ฝึกโยคะเพียงอย่างเดียว (YOGA) และภายหลังการฝึกออกกำลังกาย 8 สัปดาห์ พบว่าการควบคุมจุดศูนย์กลางแรงโน้มถ่วงร่างกายขณะเดิน 1 gait cycle และขณะเดินในช่วง single limb stance phase ของทั้งสองกลุ่มมีค่าระยะทางการเคลื่อนที่ของจุดศูนย์กลางแรงโน้มถ่วงร่างกายลดลงอย่างมีนัยสำคัญทางสถิติ และผลจากการฝึกออกกำลังกายทั้งสองรูปแบบช่วยเพิ่มความสามารถในการทรงตัวขณะทำกิจกรรมเคลื่อนไหวร่างกายได้จากค่าคะแนน Berg Balance Score ที่เพิ่มขึ้น และระยะเวลาการทดสอบ Time Up and Go ที่ลดลง สรุปผลการวิจัย กลุ่มที่ฝึกโยคะร่วมกับการฝึกกระตุ้นระบบรับรู้การทรงตัวในหูชั้นในมีค่าความเร็วที่ใช้ในการเคลื่อนที่ของจุดศูนย์กลางแรงดันร่างกายขณะยืนลืมตาบนพื้นเรียบดีกว่ากลุ่มที่ฝึกโยคะเพียงอย่างเดียว อย่างไรก็ตาม การฝึกออกกำลังกายทั้งสองรูปแบบให้ผลไม่แตกต่างกันในการฝึกเพื่อเพิ่มความมั่นคงของร่างกายจากการควบคุมการทรงตัวขณะยืนและขณะเคลื่อนไหวร่างกาย ช่วยส่งเสริมความสามารถในการทรงตัวขณะทำกิจกรรมต่างๆ อีกทั้งยังช่วยลดปัญหาและป้องกันความเสี่ยงจากการหกล้มในผู้สูงอายุเพศหญิงได้

เพื่อศึกษาการตอบสนองของระบบหายใจต่อการออกกำลังกายที่ความหนักระดับสูงสลับเบา (HIIE) และความหนักระดับปานกลาง (MICE) ในผู้ที่มีภาวะอ้วน โดยอาสาสมัครจะได้รับการทดสอบออกกำลังกายทั้งสองรูปแบบข้างต้น คือปั่นจักรยานที่ความหนักสูง 90% peak power output (PPO) 1 นาทีสลับกับความหนักเบา 15% PPO 1 นาที เป็นระยะเวลา 20 นาที (HIIE) และปั่นจักรยานที่ความหนักปานกลาง 50% PPO เป็นระยะเวลา 20 นาที (MICE) วัดความแข็งแรงของกล้ามเนื้อหายใจก่อนและหลังออกกำลังกายและวัดการตอบสนองของระบบหายใจขณะออกกำลังกาย ผลการศึกษาในอาสาสมัคร 27 ราย พบว่าค่าความแข็งแรงกล้ามเนื้อหายใจเข้า (MIP) และออก (MEP) มีค่าลดลงหลังออกกำลังกาย (p < 0.05) แต่ไม่พบความแตกต่างระหว่างกลุ่ม ค่า VO2 ของ HIIE ช่วง high intensity มีค่ามากกว่า MICE (p<0.01) ค่า VCO2 ของ HIIE ตลอดการออกกำลังกาย 20 นาทีมีค่ามากกว่า MICE อย่างมีนัยสำคัญ (p<0.05) ค่า VE มีแนวโน้มสูงขึ้นขณะออกกำลังกายทั้งสองรูปแบบ โดยใน HIIE มีค่ามากกว่า MICE (p<0.001) และมีความแตกต่างกันหลังเสร็จสิ้นการ cool down (p<0.05) ไม่พบความแตกต่างกันของค่า IC ขณะพักและหลังเสร็จสิ้นการ cool down ในการออกกำลังกายทั้งสองรูปแบบ แต่เมื่อเทียบภายในกลุ่มพบว่าค่า IC หลังเสร็จสิ้นการ cool down มีค่าสูงกว่าค่าขณะพักอย่างมีนัยสำคัญ สรุปได้ว่าการออกกำลังกายทั้งสองแบบส่งผลต่อความแข็งแรงของกล้ามเนื้อหายใจ ทำให้ความแข็งแรงกล้ามเนื้อหายใจลดลงหลังออกกำลังกาย และการออกกำลังกายแบบ HIIE มี respiratory demand มากกว่าการออกกำลังกายแบบ MICE

Tendon injuries are a common athletic injury that have been increasing in prevalence. While there are current clinical treatments for tendon injuries, they have relatively long recovery times and often do not restore native function of the tendon. In the current study, gene electrotransfer (GET) parameters for delivery to the skin were optimized with monophasic and biphasic pulses with reporter and effector genes towards optimizing underlying tendon healing. The current study aimed to investigate the differences in running demands between the National Hurling League (NHL) and the Championship, and within playing positions. GPS (10 Hz, STATSports Apex GNSS) were used to analyse the running demands during 34 games (2017–2020 seasons) of the Championship and the NHL. The running demands (total-, relative-, high-speed- [>17 km·h−1] and sprint [≥22 km·h−1] distance, number and length of sprints, and peak speed) were compared between competitions. การออกกำลังกายท่าดันพื้นเป็นท่ามาตรฐานในการออกกำลังกายแบบ compound movement ของกล้ามเนื้อช่วงบน การประเมินความล้าของผู้ปฏิบัติให้รวดเร็วและแม่นยำจะช่วยป้องกันการบาดเจ็บได้ จุดประสงค์ของการวิจัยนี้เพื่อเปรียบเทียบว่าความสัมพันธ์ระหว่างสัญญาณกระแสประสาทและการรับรู้ขณะเกิดภาวะกล้ามเนื้อล้าขณะออกกำลังกายท่าดันพื้นในกลุ่มคนที่มีประสบการณ์ออกกำลังกายในท่าดันพื้น (Well-trained) และกลุ่มคนที่ขาดประสบการณ์ออกกำลังกายในท่าดันพื้น (Un-trained) เพศชายอายุ 20-35 ปี โดยแบ่งกลุ่มละ 30 คน ทำการดันพื้นจนเกิดอาการล้าไม่สามารถดันพื้นต่อได้ ผลการศึกษาพบว่า กลุ่ม Well-trained มีจำนวนครั้งที่ดันพื้นมากกว่ากลุ่ม Un-trained อย่างมีนัยสำคัญ โดยมีค่าเฉลี่ย 27.3±6.3 ครั้ง และ 18.2±4.3 ครั้ง (p=0.001) ตามลำดับ โดยในกลุ่ม Well-trained ความล้าที่วัดได้จากการลดลงของค่าความถี่มัธยฐาน (Delta median frequency, ∆MDF) ในกล้ามเนื้อ Pectoralis Major มีความสัมพันธ์กับ Visual numeric scale of fatigue (VNS-F) ในระดับสูง (r=-0.98, p<0.05) ส่วนกลุ่ม Un-trained การลดลงของ ∆MDF ในกล้ามเนื้อ Triceps Brachialis และกล้ามเนื้อ Upper Trapezius มีความสัมพันธ์กับ VNS-F ในระดับสูง r = -0.93, p<0.05 และ r = -0.86, p<0.05 ตามลำดับ โดยทั้ง 2 กลุ่มพบว่า ∆MDF ลดลงอย่างมีนัยสำคัญเมื่อค่า VNS-F มากกว่า 6 สรุปว่า ในการออกกำลังกายท่าดันพื้น ความสัมพันธ์ของภาวะกล้ามเนื้อล้าที่วัดได้จากกระแสประสาทกล้ามเนื้อกับความรู้สึกล้ามีความสัมพันธ์ในระดับสูงทั้งสองกลุ่ม แต่จะแตกต่างกันที่กลุ่มกล้ามเนื้อ โดยกลุ่ม Well-trained วัดได้ที่กล้ามเนื้อหลักมัดใหญ่ที่ใช้ในท่าดันพื้น ส่วนกลุ่ม Un-trained วัดได้ที่กล้ามเนื้อหลักมัดเล็กและกล้ามเนื้อมัดรองในท่าดันพื้น ดังนั้นในกลุ่ม Un-trained ควรเสริมสร้างความแข็งแรงของกล้ามเนื้อ Triceps Brachialis และ Pectoralis Major ให้แข็งแรงก่อนเพื่อป้องกันการบาดเจ็บจากการใช้กล้ามเนื้อผิดมัดในการออกกำลังกายท่าดันพื้น

To demonstrate that a 12-week exoskeleton-based robotic gait training regimen can lead to a clinically meaningful improvement in independent gait speed, in community-dwelling participants with chronic incomplete spinal cord injury (iSCI).
Setting:
Outpatient rehabilitation or research institute.

Multi-site (United States), randomized, controlled trial, comparing exoskeleton gait training (12 weeks, 36 sessions) with standard gait training or no gait training (2:2:1 randomization) in chronic iSCI ( > 1 year post injury, AIS-C, and D), with residual stepping ability. This project intends to use Tiger Woods as a case study to further study the physiological and biomechanical effects of a spinal fusion. In 2017, and at age 42, Woods received an Anterior Lumbar Interbody Fusion (ALIF) surgery. Less than two years later, Woods was able to return to professional golf. The goals for this research project are to; explain the medical issues that led Woods to receiving this surgery, describe in depth the surgical technique that was used, detail the recovery process that is associated with this procedure, and to examine the change in biomechanics of Tiger Woods’ golf …
