Diagnosis And Treatment Of Stroke-Related Sarcopenia: A Narrative Reviewarticle, Szu-Han Chen, Tieh-Cheng Fu
Rehabilitation Practice and Science
Stroke patients have a high prevalence of sarcopenia due to the presence of several risk factors, including immobilization, impaired feeding, sympathetic activation, inflammation, and denervation. The occurrence of sarcopenia further exacerbates the disability caused by the stroke. Thus, appropriate diagnosis and intervention for sarcopenia is an important health issue for the stroke population. The article discusses the dilemma in diagnostic tests and potential treatment for stroke-related sarcopenia based on the currently available literature evidence.

Dysphagia is a common poststroke complication. It is associated with aspiration, malnutrition, dehydration, and disability, and it may increase the risk of pneumonia, and even death. Early diagnosis and treatment of patients with dysphagia reduces not only the aforementioned risks, but also the length of hospitalization and overall healthcare expenditures. Patients with poststroke oropharyngeal dysphagia exhibit highly variable patterns of swallowing abnormalities, and the related symptoms remain challenging to identify clinically. Kienböck's disease of the wrist is a multifactorial, progressive osteonecrosis of the lunate bone. Patients with this disease often complain of dorsal wrist pain and other wrist dysfunctions. Image studies are usually required for definite diagnosis; however, careful history taking and physical examination are also necessary. However, the application of elastic bandages can cause soft tissue injuries in patients' extremities. Deep vein thrombosis (DVT) of the lower extremity is a multifactorial and potentially fatal complication following stroke. However, repetitive impact on the lower extremity is a relatively uncommon risk factor for DVT. We report an ambulatory hemiparetic stroke patient who, without classic DVT risk factors, developed a lower extremity DVT probably due to repetitive impact on the popliteal fossa after the inappropriate use of a seated leg flexion machine. This case can remind physiatrists and therapists of the importance of preventing patients from using non-ergonomic postures during training sessions.

In the present study, we aimed to investigate the social support status over an 18-month duration; the relationship between health status and social support level; and the predictors of low social support status in informal caregivers of stroke patients. A common symptom that occurs after strokes is impaired contralesional motor function and contralateral limbs weakness. Such weakness of the limbs is often found in a supratentorial stroke. Two patients with ipsilateral limbs weakness after supratentorial stroke events were reported. One was a recurrent ischemic stroke in the right frontal lobe with right hemiparesis; the other was a new ischemic stroke in the right thalamus with right hemiparesis. Bardet-Biedl syndrome (BBS) is a rare inherited disease. Up to date, there have been 14 known BBS-related mutation genes, resulting in ciliopathy with multiple abnormalities such as obesity, pigmentary retinopathy, polydactyly, learning disabilities, poor coordination, renal dysfunction, diabetes and hypertension. This case was a 29-year-old male, who was diagnosed as Bardet-Biedl syndrome since childhood and then was followed up for control of complications. He suffered from stroke at age of 29. This study explores the factors correlated to the use of home-based rehabilitation services by stroke patients for whom care is provided according to the state policy pertaining to the 10-year long-term care plan.During July 2008–June 2010, data were collected from 759 stroke patients by using a long-term care management service evaluation form. These patients had initially applied for services at the long-term care centers of the Bureau of Public Health in Taichung County. Of these, 119(15.7%) eventually used home-based rehabilitation services.
