Background: Pentosan polysulfate sodium (PPS, Elmiron®) is a widely used medication to reduce pain caused by interstitial cystitis, a bladder disorder most often affecting females. PPS maculopathy is a novel form of toxic retinal disease often not recognized or mistaken for other retinal conditions such as macular degeneration due to the initial similarities on fundus exam. However, when visualized using fundus autofluorescence, affected eyes show characteristic hyper/hypo-autofluorescent patterns.

Case report: A patient taking PPS for 10 years presented with macular changes and complaints of blurry vision and nyctalopia. Clinical, electrodiagnostic, and genetic testing were performed including fundus autofluorescence and …

Purpose. Current approaches lack research toward rehabilitation of the lower extremity following injury regarding progression and the effects of ground reaction forces (GRFs) along the kinematic chain. A typical physical therapy protocol demonstrated the patients initially performing non-weight bearing, or open chain exercises followed by walking and running activities. This initial pilot study entertained the need for a more “seamless” progression of exercises to bridge the gap of GRFs when rehabilitating a weight bearing joint from walking-to-running in healthy individuals. This doctoral capstone project, completed during a 14-week Applied Leadership Experience (ALE) with Georgetown-Scott County EMS (GSCEMS), focused on the ACOTE target areas of Program Development and Advocacy by creating an evidence-based, trauma-informed, occupational therapy support group curriculum for motor vehicle accident (MVA) survivors. A literature review and needs assessment identified a significant gap in post-crash care, as many survivors experience long-term psychological challenges—including PTSD, anxiety, depression, fear of driving, and disruptions in daily roles—despite care often emphasizing physical rehabilitation over emotional recovery. Purpose: Patients with lower extremity amputations are at increased risk of falls due to decreased balance ability between amputated and sound side. The purpose of this study was to examine the usefulness of a reaching tool for practitioners to assess balance in an inpatient rehab facility. It was anticipated that clinicians will find the reaching device useful when administering the Functional Reach Test (FRT) in a clinical setting, given the frequent use of these assessments and the device’s demonstrated reliability.

Hamstring strain injuries are among the most common and recurring injuries in sports, occurring most often during the terminal swing phase of gait. Despite progress in rehabilitation, the incidence of injury has only increased, suggesting a need for a novel intervention. We designed a passive, assistive, wearable device with two specific criteria in mind: to offload the hamstrings during running, and do so without hindering normal kinematic patterns. The device consists of an adjustable elastic band anchored proximally at the waist and distally just above the ankle joint center. Guided by a needs assessment, the program targeted financial literacy, job readiness, professionalism, and soft skills through structured sessions incorporating education, interactive activities, and group discussions. Findings from pre- and post-surveys and qualitative feedback indicated improved knowledge, confidence, and readiness for community reintegration, supporting the value of rehabilitative, occupation-based programming.

The project aimed to collect patient feedback and preliminary qualitative data on the implementation of animal-assisted therapy (AAT) interventions within an inpatient rehabilitation setting. Project activities included a comprehensive literature review, examination of current AAT practices, collaboration with therapy dog teams regarding their work and certification processes, implementation of AAT-based occupational therapy interventions, and administration of patient surveys following sessions. Non-adherence to prescribed home exercise programs significantly undermines the efficacy of physical therapy (PT), with research indicating only approximately 35% of patients fully adhere to regimens. This issue contributes to suboptimal recovery and places a staggering financial burden on the healthcare system, estimated at $100 billion to $300 billion annually. The Physical Therapy Home Exercise Adherence Trial (PT-HEAT) evaluates the effectiveness of a 14-week behavioral education intervention designed to improve patient adherence outside the clinical setting.

Background: Military personnel and veterans are at increased risk of lower-limb amputation, often due to combat-related trauma such as improvised explosive devices (IEDs), as well as secondary factors including tobacco use, peripheral artery disease, and diabetes. This study systematically reviews and compares lower-limb prosthetic technologies in military personnel and veterans, focusing on functional outcomes, mobility, and patient satisfaction.

Methods: A comprehensive literature search was conducted using PubMed MeSH terms to identify studies examining lower-limb prosthetic use and outcomes among military personnel and veterans. The final MeSH-based search yielded 27 results.

Background: Burn injury induces a prolonged inflammatory response that may contribute to long-term metabolic dysfunction. Exercise is known to reduce inflammation in various clinical populations; however, its effect on chronic post-burn inflammation remains unclear. This crossover trial investigated the impact of a 6-week exercise intervention on tumour necrosis factor-alpha (TNF-α) in adults with non-severe burns sustained more than one year prior. Methods: Twenty-one participants were randomised to complete either a 6-week exercise program or a control period first, separated by a 4-week washout. Kate Lewis, a ceramicist and teaching artist based in Grand Rapids, MI, created the artwork for the Winter 2026 issue of the Open Journal of Occupational Therapy (OJOT). The image features a collection of heart-shaped ceramic pinch pots, produced after she was told she may never be able to move her left arm again. She created them on the recommendation of an occupational therapist who understood the value of meaningful occupations for recovery. In this Opinions in the Profession article, we explore remediation treatment strategies using exercise for recovery from post-traumatic brain injury and aim to justify the use of such treatment in the occupational therapy profession to increase skills for functional gain. Spontaneous vertebral osteomyelitis (SVO) is a rare but severe spinal infection that often presents with non-specific back pain, leading to misdiagnosis as a neoplastic disease. A 53-year-old female presented with progressive lower back pain, rapidly developing sudden complete paraplegia and neurogenic bowel/bladder. Initial MRI revealed T4–T6 vertebral lesions mimicking a spinal tumor. Emergent surgery and cultures confirmed Escherichia coli vertebral osteomyelitis.

Following decompression and targeted antibiotic therapy, a cautious, phase-specific rehabilitation program was initiated. Given the persistent infection and spinal instability, all weight-bearing activities were deferred until infection control and structural stability were confirmed following a spinal fixation surgery. Background: After anterior cruciate ligament reconstruction (ACLR), chronic changes in knee joint biomechanics during higher level tasks, such as running, may negatively impact long-term knee joint health. Background: The coracobrachialis (CB) is an important anterior shoulder stabilizer, yet no study has previously reported its electromyography (EMG). The purpose of this study was to validate a novel technique for surface EMG (sEMG) of the CB.   Methods: Thirty-two (21 females) healthy individuals participated in this cross-sectional study. A surface landmark for recording the CB was identified within the medial aspect of the proximal arm using real-time ultrasound imaging and cadaver dissection. Background: Brain-derived neurotrophic factor (BDNF) is a key neurotrophin due to its role in neuron process outgrowth, plasticity, and neuronal survival. Aerobic exercise can induce BDNF release and may ultimately maximize post-stroke recovery. Objectives: This study aimed to determine if a program of moderate-to-high-intensity aerobic exercise increased concentrations of BDNF in subacute stroke survivors compared to usual care. Methods: A parallel-group, RCT was undertaken in people with subacute stroke undergoing rehabilitation. Cette étude explore l’incidence du parcours prélésionnel des travailleuses et travailleurs immigrants (TI) blessés sur l’alliance thérapeutique (AT) en réadaptation au travail. À partir d’une ethnographie longitudinale menée dans une clinique québécoise, sept trajectoires ont été analysées selon leurs dimensions migratoires, socioprofessionnelles et linguistiques. Les résultats montrent que le déclassement professionnel, la précarité de l’emploi, les barrières linguistiques et les expériences de racisme fragilisent la construction du lien de confiance et la définition partagée des objectifs de retour au travail. A Narrative Review Of The Central Neural Control Of Swallowing And Its Implications For Dysphagia Rehabilitation, Kuo-Chang Wei, Tyng-Guey Wang, Ming-Yen Hsiao

Swallowing is controlled by a distributed neural network spanning the brainstem central pattern generator, subcortical modulatory circuits, and cortical regions responsible for volitional planning and sensorimotor integration. Evidence from lesion studies, neuroimaging, and neurophysiology demonstrates that dysphagia arises from disruptions at different nodes of this network, producing lesion-specific impairment profiles rather than a uniform swallowing deficit. This network-based perspective has shifted rehabilitation from predominantly compensatory management toward mechanism-driven interventions aimed at restoring physiological function. Behavioral skill-training and resistance-based exercises harness motor learning and cortical plasticity, whereas neuromodulatory approaches—including repetitive transcranial magnetic stimulation (rTMS), transcranial direct current stimulation (tDCS), pharyngeal electrical …
