Extract:To the Editor: When an elderly patient has a first-time presentation of psychiatric symptoms, a physical cause should be suspected. In geriatric practice, these symptoms are often due to delirium, cognitive decline, or a mood disorder. The present case history illustrates the importance of further investigation when geriatric assessment alone cannot explain the symptoms sufficiently.

This paper presents a 3D finite element upper arm model, validated by experiments as well as clinical data, used to study the error introduced in blood pressure measurements due to variability of arm tissue mechanical properties. The model consists of three separate cylindrical parts: soft tissue, bone and brachial artery. The artery volume changes under the cuff are used to represent the cuff pressure oscillations for analyzing blood pressure measurements. Transient Ischaemic Attack (TIA) and stroke are clinical syndromes characterised by acute neurological deficits with vascular causes. People experiencing TIA or first stroke are at significant risk of subsequent stroke. Risk factors, which are associated with 90% of risk of all stroke, have been identified, and include factors associated with lifestyle i.e. tobacco use, diet, obesity, alcohol consumption, physical activity and stress. Targeted therapeutic interventions have the potential to substantially reduce the burden of stroke. Prediction studies testing a thorough range of psychological variables in addition to demographic, work-related and clinical variables are lacking in lumbar fusion surgery research. This prospective cohort study aimed at examining predictions of functional disability, back pain and health-related quality of life (HRQOL) 2–3 years after lumbar fusion by regressing nonlinear relations in a multivariate predictive model of pre-surgical variables. Before and 2–3 years after lumbar fusion surgery, patients completed measures investigating demographics, work-related variables, clinical variables, functional self-efficacy, outcome expectancy, fear of movement/(re)injury, mental health and pain coping. We evaluated a tele-education programme for primary care staff in Pernambuco State, Brazil. During 2008 and 2009, tele-education sessions occurred four times each week for one hour per day. The topics included public health, child and adolescent health, mental health and nursing. After each session, participants completed an evaluation questionnaire. A total of 73 municipalities and 141 health centres participated in the programme. There were 254 tele-education sessions scheduled during the 20-month study period; of these, 224 sessions were successfully performed and 30 were cancelled. Background: Axial rotation of the trunk is important to many vocational tasks and activities of daily living, and may be associated with back injuries. The influence of spinal postures on trunk rotation appears conflicting. This study investigated the influence of forward trunk inclination, spinal posture and pelvic fixation on maximum trunk rotation.

This paper reviews the physiological, biomechanical and health impacts of load carriage on the female soldier and extends to include issues acknowledged as impacting on the female athlete and hence the female soldier. Physiological factors such as fat mass, strength, aerobic endurance and biomechanical factors such as stride length and forward lean have the potential to increase the energy cost of load carriage and cause injury. The female athlete triad, iron deficiency, pelvic floor muscle dysfunction and military equipment issues can likewise reduce female soldier performance and increase injury risk. Background: Low back injuries are common and costly, accounting for 15 to 25 percent of injuries covered by workers’ compensation and 30 to 40 percent of the payments made under that program. The high costs of injury, the lack of effective treatment, and the evidence that there are behavioral risk factors have led to widespread use of employee education programs that teach safe lifting and handling. The effectiveness of those programs, however, has received little rigorous evaluation. When factors that influence exercise behavior are known, health care professionals can more likely design and modify patient education materials targeted to promote exercise behavior. This study aimed to identify predictors of exercise behavior in patients with rheumatoid arthritis 6 months after a visit with their rheumatologist.
Subjects and Methods. Twenty-five rheumatologists and 132 patients with rheumatoid arthritis participated. One hundred thirteen patients (85.6%) completed the 6-month follow-up. Rheumatologists and patients completed baseline questionnaires and were audiotaped during a subsequent visit. Background: The prevalence of imaged pathology in primary care has received little attention and the relevance of identified pathology to symptoms remains unclear. This paper reports the prevalence of imaged pathology and the association between pathology and response to diagnostic blocks into the subacromial bursa (SAB), acromioclavicular joint (ACJ) and glenohumeral joint (GHJ).

There is lack of in depth data on health needs of diverse Asian American communities. We conducted 19 focus groups in 13 Asian American communities in Maryland in 2007. We developed a moderator’s guide to collect qualitative data on health needs from 174 participants, and used MAX QDA to analyze data and code emergent themes. Cardiovascular disease related conditions, diabetes, and mental health were the top three health concerns. Weight concerns, cancer, arthritis, smoking, osteoporosis, and hepatitis B followed next. Current guidelines advocate the use of R.I.C.E., the acronym for Rest, Ice, Compression and Elevation, for patient self-management of soft tissue injury. The purpose of this study was to investigate the efficacy of R.I.C.E. applied as part of patient self-management compared (R.I.C.E.) to R.I.C.E. incorporated with multimodal physiotherapy management (Physiotherapy). A randomised control trial was carried out in a Physiotherapy Clinic. Twenty eight participants with an acute ankle sprain were randomised into either R.I.C.E. (n=12) or Physiotherapy (n=16) groups. Acceptable reliability is a prerequisite for inclusion of physical examination tests in clinical examinations of the painful shoulder. The aim of this study was to establish the intraexaminer and interexaminer reliability of measures of shoulder range of motion (ROM) and muscle force using a new hand-held dynamometer with the ability to standardize overpressure force during passive ROM tests. Purpose: To investigate the within-day and between-day reliability of 3D lower extremity kinematics during five lower extremity functional screening tests and to assess the association between these kinematics and those recorded during jogging.
Methods: Peak three-dimensional lower extremity kinematics were quantified in 25 uninjured participants during five lower extremity functional tests and jogging. A nine camera motion analysis system (Qualysis Medical AB, Sweden) was used to capture three trials of all tests. All functional tests were repeated by 10 participants one to two days later. PURPOSE: The present case study examined the potential use of the AlterG Anti-Gravity Treadmill as treatment modality for extreme obese individuals. The special effect of the treadmill reduces ground reaction forces during walking through the use of lower body positive pressure. METHODS: One extremely obese (BMI 69.2) Caucasian 44 year-old female utilized the AlterG Anti-Gravity Treadmill during a 14 wk walking program to pilot test the protocol. RESULTS: At the conclusion of the study, exercise tolerance time increased 3-fold (initially 20 min increased to 65 min) while caloric expenditure increased 4-fold (initially 91.0 kcal/session increased to 442.7 kcal/session) without an …
