Motivational Interviewing In Vocational Rehabilitation: Why It Matters For People With Disabilities, Alicia Wein-Senghas

Motivational Interviewing (MI) is increasingly being trained within and integrated into public vocational rehabilitation (VR) programs based on its efficacy in related fields and yet research specific to the efficacy of MI in public VR programs is almost nonexistent. This study used a quantitative sequential design to explore relationships between MI training, MI proficiency, and VR customer outcomes within the State of Vermont’s Division of Vocational Rehabilitation (VT DVR) program. The study occurred in two phases. BACKGROUND:The success of job seekers with disabilities in achieving their employment goals depends in large part on the quality of employment supports that they receive from employment consultants. OBJECTIVE:To test the effectiveness of data-enabled performance feedback to assist employment consultants in implementing standards of effective employment supports. METHODS:A total of 187 employment consultants in 30 states were randomly assigned to intervention or control groups. The intervention group received data-enabled performance feedback and guidance for 12 months, whereas the control group continued with business as usual. The Social Security Administration (SSA) offers a set of work incentives for Supplemental Security income (SSI) beneficiaries. Work incentive employment supports help SSI recipients go to work by minimizing the risk of losing their SSI or Medicaid benefits (Social Security Administration, 2018). One such incentive, the Plan to Achieve Self-Support (PASS), allows approved individuals to set aside earned or unearned income and resources to achieve an employment goal. Since the introduction of supported employment in the Developmental Disabilities Act of 1984 and the Rehabilitation Act Amendments of 1986, there has been continued development and refinement of best practices in employment services and supports. What?
Analysis to describe the relationship between state employment system characteristics and employment outcomes for individuals with intellectual and developmental disabilities (IDD).

Why?
To better understand higher-performing state employment systems.

How?
Researchers included individual indicators from 2013 for each state from its IDD and vocational rehabilitation (VR) agencies. We also used American Community Survey data that looked at employment outcomes and education for individuals with cognitive disability between the ages of 22 to 30, i.e. individuals who recently aged out of eligibility for services under IDEA. The resulting composite indicator (CI) represents overall state employment system performance.

Interviews with 16 employment consultants-triangulated with job seekers, family members, and supervisors-revealed a model of employment supports aligned with the elements described in the literature, although with an added emphasis on (a) building trust as a key element starting from day one; (b) a circular process converging on the job match; (c) and flexible intensity of supports. The model can be used for improving clarity in communication with employment consultants about effective employment support practices for assisting job seekers with intellectual and developmental disabilities.

In FY2016, an estimated 638,568 individuals received day or employment supports from state IDD program agencies. This number grew from 455,824 in FY1999. The estimated number of individuals in integrated employment services increased from 108,227 in FY1999 to 120,244 in FY2016. Despite the trend to terminate facility-based services in some states, the overall state investment continues to emphasize non-work services, rather than integrated employment services. Figure 1 shows the trends in the percentage of people served in integrated employment and facility-based and non-work settings between FY2008 and FY2016.

Only 23% of adults with intellectual disabilities work, compared to 73% of people without disabilities (statedata.info). To bridge this gap, the vocational rehabilitation (VR) program offers valuable services including assessment, job search assistance, and counseling. In FY 2014, over 46,000 adults with intellectual disabilities exited the national VR program. About 38% of them reported an employment outcome. However, a large proportion of them exited without employment, and were reported as either having lost interest in receiving services (29%), or unable to be located by VR staff (17%). The Ad Hoc Committee on Employment and Community Participation began meeting in the winter of 2004 in an effort to promote integrated employment opportunities for people with disabilities in Colorado. The committee was comprised of representatives from the Division for Developmental Disabilities (DDD) administration; the Division of Vocational Rehabilitation; local Community Centered Boards (private nonprofit organizations responsible for authorizing services); advocacy groups; and self-advocates, parents, and service providers.

In 2010, when the New Hampshire Bureau of Developmental Services (BDS) received grant funds to strengthen multisystem service delivery, its administrators partnered with area agencies; community rehabilitation providers, or CRPs (employment providers); and other stakeholders to improve and streamline the process of collecting employment data. Originally a multi-system process, BDS continued the data-collection effort when other systems withdrew. BDS’s new data-collection system allows unprecedented access to clear data displays, as well as the ability to run a variety of detailed reports to guide the statewide promotion of integrated employment.

Alabama has piloted the Gaining Access to Employment project, a collaborative effort between the state’s Department of Mental Health/ Developmental Disabilities (MH/DD) and its Department of Vocational Rehabilitation (VR). Through this project, known as Project GATE, the two agencies work together to help local service providers use funds to support integrated employment opportunities. MH/DD and VR have a long history of partnering, including joint efforts on a supported employment workgroup, due to the strong relationships between colleagues at each department.

In the mid-1980s, the state of Washington was awarded a five-year federal systems change grant to kick-start their supported employment efforts via the Washington State Employment Initiative. Funding from this grant was used to develop training on best practices and to generate high-quality integrated employment supports among agencies. Wisconsin’s Developmental Disabilities Services agency and Vocational Rehabilitation agency, in conjunction with the Wisconsin Board for People with Developmental Disabilities and Wisconsin’s University Center for Excellence in Developmental Disabilities, implemented a series of community conversations to build dialogue and create a coalition around employment for youth with intellectual and developmental disabilities (IDD). Through this initiative, a range of community members came together in structured forums to discuss ways to improve integrated employment outcomes for youth.

Case management services in Missouri’s Division of Developmental Disabilities underwent a shift from a state structure to a regional structure. Within this structure, 12 regions each serve approximately 12–15 counties. Each region has a technical assistance (TA) position designed to support each of the designated priority work areas. These include self-determination, family supports, individual supports and services, Employment First, and accessible housing. Experiencing paid employment during and immediately after high school is a critical step on the path toward economic self-sufficiency in adulthood. Young adults with disabilities interested in gaining employment experiences may seek support from vocational rehabilitation (VR) programs. In this Data Note, we examine the extent to which young adults with intellectual disabilities engage with their state VR programs.

The following report summarizes available national data on educational, employment and economic outcomes for youth and young adults with intellectual disabilities (ID) over the years 2000-2010. These data can be used to benchmark progress in improving these outcomes for young adult population across the country and within individual states. Data is reported separately for two age groups of young adults (16-21 and 22-30) in order to capture possible differences between youth likely to still be receiving school services (through age 21) and those who have moved on from the education system.

New Hampshire’s Bureau of Developmental Services, Department of Vocational Rehabilitation, other state and local service providers, advocates, and families are committed to increasing the employment rate and the quality of employment outcomes for people with developmental disabilities. These stakeholders met to craft an employment position statement. They framed their discussions according to factors that research has found to be common to “high-performing” states in providing integrated employment opportunities. Then they developed an employment position statement that satisfied all the groups involved, and communicated their shared belief in the importance of integrated employment as a means toward community inclusion.

In 2009, the state of Oregon adopted its Employment First policy. When Oregon’s Office of Developmental Disabilities Services (DDS) decided to promote the implementation of this policy, it began by updating its existing employment website. The redesigned website (http:// www.dhs.state.or.us/dd/supp_emp/) emphasizes the value of integrated employment over other outcomes, and the importance of building community-wide conversations, with the goal of achieving integrated employment for people with developmental disabilities. In 1998, five state agencies formed the Governance Group. Partner agencies included: Iowa Workforce Development, Department of Human Services, Department of Education, Iowa Vocational Rehabilitation Services and Department of Human Rights. The group was developed in response to a Rehabilitation Services Administration (RSA) request for proposals that included an advisory group for systems change efforts to expand employment opportunities to individuals with disabilities. Michigan’s Department of Community Health, Mental Health and Substance Abuse Administration (MDCH) has expressed a strong desire to improve the state’s employment outcomes among people with intellectual and developmental disabilities (IDD). Set against this desire is a major obstacle: Michigan is among the states hardest hit by the continuing economic recession, with the highest unemployment rate in the nation. MCDH delivers supports through local Community Mental Health Services Programs (CMHSPs). Oklahoma’s Developmental Disabilities Services Division (DDSD) realized the need for increased attention towards the goal of community-based employment for individuals they served. Initially, rates were based on a vendor’s costs of providing direct services such as job development and job coaching. It became increasingly apparent that claims for vocational services oftentimes reflected staff activities (e.g., job development, client assessment, and service delivery documentation), which may have been occurring without the direct involvement of the service recipient. In 2002, the Massachusetts Department of Developmental Services (DDS) developed a contractual requirement that employment service provider performance be tracked through outcome measures. As a result, DDS shaped its employment data collection system to focus on what it viewed as key outcomes for measuring success around employment. Tennessee implemented its Employment First initiative in 2002 with a goal of making employment the first day service option for adults receiving supports from the Department of Mental Retardation Services. As part of the Employment First initiative, the state requires a periodic community-based work assessment for all individuals not currently employed in the community. State-contracted and independent individualized support coordinators ensure that individuals who are not in integrated employment participate in a community-based work assessment at least every three years. Since December 2005, employment providers can be trained in conducting situational job assessments and the importance of providing these opportunities.

The state of Florida has implemented a five-year employment initiative for people with ID/DD. The goal is to enable at least 50% of adults (ages 18 to 55) receiving APD-funded day services as of July 1, 2004, to achieve community employment by July 1, 2009. The University of Maine’s Center for Community Inclusion and Disability Studies (CCIDS), along with the Maine Department of Behavioral and Developmental Services (BDS) and the Bureau of Rehabilitation Services, developed the Maine Employment Curriculum (MEC). The comprehensive curriculum fosters best practices in employment supports for people with disabilities statewide by using a cadre of trainers who are supported by the Maine Employment Curriculum project staff. The Maine Employment Curriculum ultimately seeks an increase in the number of integrated, communitybased supports available and builds the capacity of the employment support provider community to achieve this goal.

In 2006, a new Maine law mandated the creation of a waiver program that promotes the expansion of supported employment programs for people with intellectual and developmental disabilities (ID/DD). As a result, state funding for sheltered workshops was reduced for seven workshops and approximately 220 individuals throughout Maine. These individuals received funding under the new waiver program for community supports and supported employment services. As a result, Maine’s Department of Health and Human Service’s (DHHS) Office of Adults with Cognitive and Physical Disabilities (OACPD) developed a comprehensive work plan. King County’s program to employ people with disabilities in county jobs is an example of Washington’s commitment to the use of innovative approaches to increase integrated employment. In 1989, a training resource funded by Washington State and the county Division of Developmental Disabilities, O’Neill and Associates, submitted a grant application to the Rehabilitation Services Administration to develop public sector jobs for people with developmental disabilities within the state.
