Comparison of Push-Pull Air Curtain to Overhead Downdraft Systems on Aerosol Containment and Distribution in Controlled Environments.

This dissertation looks at the ongoing issue of tiny airborne particles created during medical and dental procedures and evaluates ways to reduce the risk of exposure for healthcare workers and patients to these particles. The first study focuses on the size, how long particles stay in the air, and how far they travel from common medical procedures that create aerosols, showing that tiny particles (less than 5 µm) can stay in the air long enough to escape basic droplet controls. Ultra-fine particulate matter (UFP), particularly from diesel emissions, poses significant health and environmental risks due to its small size and potential to penetrate deep into the respiratory system. Accurate measurement of UFP concentrations is essential for assessing exposure and mitigating health impacts. However, current methods for measuring UFP can be complex, time-consuming, and costly. Exploring the routes of exposure and biological fate of 4,4’-methylene diphenyl diisocyanate (MDI) has been a challenge for many years. Occupational exposure to MDI can result in the development of occupational asthma (MDI-OA) which results in loss of productivity, large economic costs, and costs to the individual worker. Initially, it was thought that respiratory exposure to MDI was the most likely route leading to MDI-OA. Industries utilizing MDI have been successful at controlling airborne concentrations according to industrial hygiene assessment and newly diagnosed cases of MDI-OA decreased and plateaued. This study aimed to assess the effectiveness of different types of respiratory protection, including National Institute for Occupational Safety and Health (NIOSH) Approved respirators and surgical masks in healthcare settings, as well as barrier face coverings (BFCs) used by the public as source control. The assessment was based on three components: total outward leakage (TOL), particle size distribution, and exhalation airflow velocity. To achieve this, the study utilized headform testing systems to measure TOL and airflow velocity. Allergic airway diseases such as asthma continue to increase in incidence in industrialized nations like the United States. These diseases are complex inflammatory processes involving numerous cells and mediators and are strongly influenced by fungal exposures. Recent developments in fungal detection methods have highlighted the contribution of Basidiomycota yeast species in indoor environments such as Vishniacozyma (syn. Cryptococcus) victoriae. Introduction: Minimal research has been conducted on healthcare utilization among injured workers residing in rural areas. These workers are often at high risk for work-related injury due to the industries in which they work and commonly experience barriers to accessing healthcare. The first study in this dissertation series explored general characteristics of injured workers receiving care at occupational medicine clinics in West Virginia (WV). The second and third studies explored the moderating effects of rurality, pain medication, and psychiatric medication on sociodemographic and health status factors and number of medical encounters.

Using evacuated canisters for whole-air sampling allows for sampling without specific knowledge of the environment and potential analytes. Evacuated canisters are able to tolerate concentrations ranging from parts per trillion (PPT) to parts per million (PPM). The U.S. Environmental Protection Agency (EPA) has used evacuated canisters for years to sample volatile organic compounds (VOCs) from ambient air using their method TO-15 (LeBouf, et al., 2019). The GC-MS system used for the development of NMAM 3900 used liquid nitrogen to cool sorbent traps in the preconcentrator. Fused filament fabrication (FFF), a three-dimensional (3-D) printing process, is an emerging technology that has recently gained wide popularity among both consumers and manufacturers. Introduction - Patient mobility performed in the hospital setting places healthcare workers at high risk for WMSDs. The healthcare industry has responded through the creation of SPHM programs that are successful in reducing work-related injuries broadly. However, less is known about the distinct experiences of rehabilitation professionals. Therefore, an in depth exploration of the perceptions, participation, and needs pertaining to lift equipment usage among rehabilitation professionals was performed.

The role of powered air- purifying respirators (PAPRs) in healthcare settings during infectious diseases outbreaks and highly contagious pathogens (e.g., SARS, HINI, Ebola, COVID etc.) has attracted much interest based on the level of respiratory protection they provide and their many positive user features. A common practice among healthcare workers to minimize airborne contaminant exposure has been to use surgical masks or N95 filtering facepiece respirators (FFRs). Introduction: Hand grip strength (HGS) has been negatively correlated with both biologic, cognitive, and mental health markers. Aim one investigated how occupational metabolic equivalents (OMETs), impacted HGS. Aim two investigated lost time from work and the association with HGS. Finally, aim three investigated the health outcomes of workers’ compensation claimants who have lost time from work, following diagnosis of CTS.

Results:HGS was positively associated with OMET (rs = 0.078, p = 0.009). Linear regression model demonstrated a statistically significant association between HGS and OMETs, F(1, 31) = 9.735, p = .001. Aim two found that all occupation groups had …

In the United States, under OSHA regulations, all respirators used in the workplace must be certified by the National Institute of Occupational Safety and Health (NIOSH). Under current NIOSH standards, loose-fitting powered air-purifying respirators (PAPR) must provide a minimum operational flow of 170 liters per minute. The objective of this study was to characterize the breathing flow rates of nine health care workers during patient care activities typically performed in a hospital's intensive care unit. This characterization will be used to determine the feasibility of lowering the minimum operational flow rate of PAPRS used in health care settings.

The unique physicochemical properties of multi-walled carbon nanotubes (MWCNT) have led to many industrial applications. Due to their low density and small size, MWCNT are easily aerosolized in the workplace making respiratory exposures likely in workers. The International Agency for Research on Cancer designated the pristine Mitsui-7 MWCNT (MWCNT-7) as a Group 2B carcinogen, but there was insufficient data to classify all other MWCNT. As the demand for multi-walled carbon nanotube (MWCNT) incorporation into industrial and biomedical applications increases, so does the potential for unintentional pulmonary MWCNT exposure, particularly among workers during manufacturing. Healthcare workers (HCWs) are the first responders when infectious pathogens occur. Powered air-purifying respirators (PAPRs) are one of the most widely used personal protective equipment for healthcare workers. Currently, few studies have been performed with HCWs in the actual healthcare work environments to assess discomforts, acceptance and protective efficiency towards wearing and use of PAPRs. Background: The cardiovascular effects of pulmonary exposure to engineered nanomaterials (ENM) are poorly understood, and the reproductive consequences are even less understood. Inflammation remains the most frequently explored mechanism of ENM toxicity. However, the key mediators and steps between lung exposure and uterine health remain to be fully defined. The purpose of this study was to determine the uterine inflammatory and vascular effects of pulmonary exposure to titanium dioxide nanoparticles (nano-TiO2). Introduction: Toxicology and epidemiology studies have observed an association between ultrafine particles (UFPs) and respiratory, cardiovascular, and neurological health effects. While there is a paucity of data in the literature on the potential toxicity and health effects from indoor UFP exposure, more exposure assessment studies and research evaluating the efficacy of controls is merited.
