Health care providers collect an array of documentation and specimens to support criminal investigations. Such documentation and specimens are referred to as “medico-legal” or “forensic” evidence, and in the case of rape, include a survivor’s documented injuries and emotional state at the time of examination, as well as samples and specimens from the survivor’s body or clothing. For survivors of rape and defilement who want to obtain legal justice, medico-legal evidence is critical. In African countries, health providers and the police are depended upon to ensure such evidence is collected, but medico-legal evidence collection can present challenges. Implementers of sexual and gender-based violence programs in refugee settings are typically eager to learn about the extent to which their programs are making a difference in the lives of those they serve. Basic monitoring information can help address this imperative without unduly burdening program implementers with time-consuming evaluations. The Sauti/VOICE project is guiding adaptation and implementation of evidence-based SGBV interventions in refugee contexts. These interventions are implemented by UNHCR partners in eight countries in the East, Horn, and Great Lakes region of Africa. Domestic work is characterized by excessive working hours, extremely low pay or no pay at all, and conditions that make the profession exploitive in many cases. Evidence is emerging that domestic work is a feeder profession for sex work. One study of more than 2,000 commercial sex workers in Ethiopia found that 42 percent were former domestic workers. However, little is known about transitions to commercial sex work or commercial sexual exploitation in Tanzania. Among practicing communities, female genital mutilation/cutting (FGM/C) is a highly sensitive and emotionally charged issue, and open discussion is often considered taboo. Not only may it be difficult to motivate people to discuss the topic, but the validity of their responses may be questionable. These factors make research on FGM/C challenging and hence the need for careful consideration of research methodologies. Female genital mutilation/cutting (FGM/C) is recognized globally as carrying risks to both the physical and psychological health of girls and women and is widely considered a violation of human rights. An estimated 200 million girls and women in 30 countries have undergone FGM/C and as many as 30 million girls younger than 15 years of age are at risk. A common policy response to FGM/C is to call for the enactment and enforcement of criminal prohibitions on the practice. Sexual and gender-based violence (SGBV) is a global public health concern disproportionately affecting women, with negative consequences for their health, well-being, human rights, and equality. SGBV includes any act perpetrated against an individual that is against that person’s will and is predicated upon gender norms and unequal power relationships. SGBV occurs in crisis settings where vulnerability is increased, and social and legal sanctions are reduced. Globally, more than 200 million women and girls have undergone FGM/C. More than half of these girls and women live in three high-prevalence countries: Egypt, Ethiopia, and Indonesia. Given the increase in population growth, recent estimates show that the number of girls and women subject to FGM/C will continue to rise in the coming years unless sufficient efforts are undertaken to encourage abandonment of the practice. A descriptive analysis of changes in FGM/C prevalence over time in Ethiopia shows that the percentage of women aged 15–49 years who report undergoing FGM/C has been declining. Gendered norms and discriminatory practices often limit women’s decision-making power, which over time can lead to social norms that systematically subordinate women. Aspects of empowerment were explored in a global evaluation of Demographic and Health Survey data that measured how gendered social norms influenced maternal health-seeking behaviors. Analysis specifically explored associations of women’s autonomy and acceptability of intimate partner violence against women (IPVAW) on antenatal care (ANC) use and facility delivery in 63 low- and middle-income countries. Service utilization is positively associated with increased autonomy and negatively associated with increased acceptability of IPVAW, but variability exists across countries and regions. …

For over a decade, the Population Council and its partners (the Africa Regional SGBV Network) have provided technical assistance and conducted research to strengthen the evidence base of sexual and gender-based violence (SGBV) programming in Africa’s East, Horn, and Great Lakes regions. Under the Africa Regional SGBV Network, the Council and partners developed and tested an intervention including police response to sexual violence survivors, with police provision of emergency contraception (EC) for survivors and referrals to health facilities. Acceptability and experience of sexual and gender-based violence is alarmingly high among adolescent girls in Zambia. Even more striking is the very young age from which notions of violence are ingrained and experience with violence begins. This brief summarizes the Adolescent Girls Empowerment Program (AGEP) evaluation which demonstrated that in the Zambian context, a program focused on changing norms among girls themselves is not enough to impact attitudes toward and experience of violence. In April 2016, the Population Council, the World Health Organization (WHO), and the International Consortium for Emergency Contraception (ICEC) convened a three-day regional technical meeting aimed at helping participating countries meet their obligations under the Maputo Protocol to protect and promote the reproductive health rights of women and girls, with a special emphasis on survivors of sexual and intimate partner violence. This study builds on previous interventions by the Population Council and the Zambia Police Service to mitigate unintended pregnancy and HIV among survivors of sexual violence by involving trained police in task-sharing interventions. These previous efforts have involved police providing emergency contraceptive pills and HIV post-exposure prophylaxis to survivors reporting to police stations, in addition to referrals to health facilities. The objective of the current study was to assess the feasibility of implementing a transportation intervention within police stations, coupled with training and sensitization for police officers to perform their government-mandated roles in post-rape care. The Government of India in 2014 signaled its commitment to adolescent health through introduction of a national strategy, Rashtriya Kishor Swasthya Karyakram (RKSK). Targeting adolescents aged 10–14 and 15–19, the program aims to ensure universal coverage of health information and services for all adolescents—those in and out of school, married or unmarried, and in vulnerable groups. According to the Survey of Young People in Egypt 2014, a significant proportion of young women residing in rural Upper Egypt were reported to be married before 18. Early marriage deprives a girl of education and employment opportunities and places her at risk of early and repeat pregnancy, gender-based violence, and sexually transmitted infections. The present study examines the sexual and reproductive health needs of married adolescent girls (MAGs) in rural Upper Egypt and identifies key contextual and cognitive factors that could mitigate or exacerbate the effects of early marriage on their sexual and reproductive health. The Population Council, together with partners, the Centre for Catalyzing Change, and the London School of Hygiene and Tropical Medicine, with support from UKaid, implemented the Do Kadam Barabari Ki Ore (Two Steps Towards Equality) project in rural areas of Patna district in Bihar, India. The project engaged frontline workers (FLWs) to screen women for their experience of marital violence, inform them about their options in case of such an experience, and provide basic counseling and referral to women reporting the experience. A key challenge underlying the gap between policy and program commitments in India and the reality of women’s lives is the dearth of evidence on what works and what does not work to change notions of masculinity and femininity, reverse norms at the community level that condone marital violence, and reduce women’s experience of intimate partner violence. In response to a global policy effort to increase school enrollment, in 1994 Malawi became one of the first low-income countries to eliminate primary school fees. Since then, Malawi has achieved nearly universal primary enrollment, however enrolling young Malawians in school has not translated into keeping them in school. This policy brief describes the nature and consequences of school violence in rural Malawi—a common experience for both girls and boys. There is little evidence that school violence disrupts schooling as expected, with the exception of sexual violence experienced at school by boys. Violence at home is also common, and may …
