Background and Objectives The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) extension for scoping reviews (ScRs) (PRISMA-ScR) was published in 2018 to provide reporting guidance. A substantial increase in published ScRs in recent years coupled with the omission or lack of engagement of interest-holders, including patients and public partners, as research partners throughout its development necessitate updating PRISMA-ScR. Important methodological advancements have emerged since PRISMA-ScR 2018, including automation and data extraction, which need to be incorporated into this update and reflected in new reporting items. Aligned with PRISMA 2020, this ScR underpins the update by identifying potential new …

Cardiovascular Health And Life Expectancy In Chronic Kidney Disease, Peipei Liu, Di Liu, Yulong Lan, Qi Zhang, Qiao Lu, Kang An, Jingyu Wang, Shuohua Chen, Shouling Wu, Youxin Wang

Research outputs 2022 to 2026

Chronic kidney disease (CKD) affects approximately 674 million people worldwide and accounted for 35.6 million years of life lost in 2021.1 Premature mortality in CKD is driven largely by cardiovascular disease (CVD),2 and management of modifiable cardiometabolic risk factors remains essential alongside pharmacological renoprotection. Life's Essential 8 (LE8),3 a validated composite measure of cardiovascular health (CVH), has been associated with lower mortality in CKD,4 yet prior studies have relied on relative risk estimates, leaving the absolute survival benefit and the extent to which high CVH can close the life-expectancy gap between patients with and without CKD unclear. We therefore examined …

Okiro, Natalie Strobel, Bibian N. Robert, Emily Odipo, Peter M. Macharia, Anthony Danso-Appiah, David C. Goodman, Queen Dube, Gagan Gupta, Hema Magge, Karen Edmond

Research outputs 2022 to 2026

Background: Evidence-based global guidance on safe travel time for small or sick newborns who require transfer to health facilities after birth is lacking. A two-hour threshold is frequently cited in low- and middle-income countries (LMICs), while 30 minutes is commonly used in high-income countries (HICs). Although these thresholds are widely referenced, their empirical basis and consistency across levels of newborn care and journey types have not been systematically examined. This study synthesises the evidence linking travel time and perinatal outcomes. Methods: We conducted a systematic review with narrative synthesis and meta-analysis to assess the impact of travel time or distance …

Remnant Cholesterol And Atherosclerotic Cardiovascular Disease Risk In Populations With Different Low-Density Lipoprotein Cholesterol Elevations: A Prospective Cohort Study, Hong Zheng, Guanlin Chen, Zhenyu Huo, Yulong Lan, Yuxian Wang, Peng Fu, Weiqiang Wu, Haixiang Zheng, Kuangyi Wu, Zegui Huang, Dan Wu, Shouling Wu, Youren Chen

Research outputs 2022 to 2026

Background: Low-density lipoprotein cholesterol (LDL-C) and remnant cholesterol (RC) are risk factors for atherosclerotic cardiovascular disease (ASCVD). However, the extent to which differences in RC levels affect ASCVD risk in populations with varying degrees of LDL-C elevation remains unclear. This study aimed to investigate whether RC can provide additional risk stratification value across different sexes, ages, and elevated LDL-C statuses. Methods: This study included 12 743 elevated LDL-C participants (LDL-C ≥3.4 mmol/L) and 50 073 age- and sex-matched non-elevated LDL-C controls from the Kailuan Study. Elevated LDL-C participants were categorized by RC levels into <0.5, 0.5 to <1.0, and ≥1.0 mmol/L subgroups. Kaplan-Meier curves and Cox proportional hazards models were used to assess the relationship between RC levels and ASCVD risk across different sexes, ages, and high LDL-C statuses. Results: During a median follow-up of 12.8 years, 1686 elevated LDL-C participants (13.2%) and 5252 non-elevated LDL-C participants (10.5%) developed ASCVD. In the borderline-high LDL-C group (3.4 ≤ LDL-C < 4.1 mmol/L), those with the lowest RC levels showed no significant risk difference compared with controls (hazard ratio [HR], 1.03 [95% CI, 0.93-1.13]), and this pattern remained consistent across different sexes and ages. In contrast, in the high LDL-C group (LDL-C ≥4.1 mmol/L), even when RC was at the lowest level, ASCVD risk remained significantly higher than that of controls (HR, 1.20 [95% CI, 1.02-1.41]). CONCLUSIONS: In the borderline-high LDL-C population, those with the lowest RC levels showed no significant risk difference compared with controls, and this pattern remained consistent across different sexes and age subgroups. In the high LDL-C population, even when RC was at the lowest level, ASCVD risk remained significantly higher than that of controls.

Background: The triglyceride-glucose (TyG) index has increasingly been recognised an indicator for stroke risk. We aimed to explore the relationship between TyG and blood pressure (BP) and the combined effects of cumulative TyG (CumTyG) and cumulative BP (CumBP) on stroke risk in a cohort of Chinese middle-aged and older adults. Methods: We enrolled 5138 participants from the 2011–2018 waves of the China Health and Retirement Longitudinal Study. Cumulative TyG and BP exposures were recorded between 2011 and 2015, and stroke incidence was then followed up until 2018. We conducted a cross-lagged panel analysis to address the temporal and directional relationships …

Burden of diseases measured as disability-adjusted life years (DALYs) per 100,000 people can be mined from public domain data, when they are made available by population health surveillance systems. This can be analysed to allow insightful comparisons with the national average, and to understand differences in trends between the sexes, age groups, time periods, geographic regions, and sub-regions. In this illustrative case study, we have analysed the Scottish burden of disease database to understand what ailed the population of the Grampian region before the COVID-19 pandemic. We have identified that selected cancers, ischaemic heart disease, Alzheimer’s disease and other dementias …

Research outputs 2022 to 2026

Background: Epilepsy is a common neurological disorder characterised by spontaneous seizures without an identifiable cause, leading to high morbidity and a significant public health burden. Understanding trends in its incidence, prevalence, mortality, and disability-adjusted life years (DALYs) is essential for developing effective prevention and treatment strategies. We aim to describe global trends in epilepsy over the past 32 years and explore the impact of sociodemographic index (SDI) levels on disease burden using the Global Burden of Disease 2021 data. Methods: We analysed the global burden of epilepsy (1990–2019) using age-standardised rates (ASRs) and their 95% uncertainty intervals (UI). We assessed …

Research outputs 2022 to 2026

Background and aims: Abdominal aortic calcification (AAC) reflects subclinical atherosclerotic cardiovascular disease (ASCVD), but its lifestyle determinants remain uncertain. We investigated how a Healthy Lifestyle Score (HLS) relates to AAC and whether AAC mediates the HLS-ASCVD association. Methods and results: Participants from the UK Biobank Imaging study were assessed for AAC from DXA images. A Healthy Lifestyle Score (HLS), based on smoking, physical activity, diet quality, alcohol consumption, sleep, and BMI was developed, and categorised as low, moderate and high. Cross-sectional associations with (i) any AAC (AAC≥1) and (ii) high AAC (AAC≥6) were analysed using multivariable-adjusted logistic regression models. The …

Vaccination elicits systemic immune responses that extend beyond antibody and lymphocyte activation, engaging coordinated metabolic and lipid pathways that modulate immune function. Nevertheless, how systemic molecular responses differ across COVID-19 vaccine platforms, including closely related mRNA platforms, remains poorly understood. Here, we performed integrated plasma proteomic, lipidomic, and metabolomic profiling of 53 SARS-CoV-2–naïve adults vaccinated with Pfizer-BioNTech, Moderna, or AstraZeneca COVID-19 vaccines. Samples were analysed across multiple timepoints using differential analysis and DIABLO-based multi-omics integration. Our findings reveal distinct platform-associated multi-omic molecular signatures not captured by unsupervised analyses alone. Pfizer-BioNTech vaccination was associated with widespread increases in immunoglobulin and complement …
