BACKGROUND

In this scoping review, we identified and reviewed 23 original articles from the PubMed database that investigated the relationship between nonacute opioid use (NOU) and cardiovascular outcomes.

METHODS AND RESULTS

We defined NOU to include both long-term opioid therapy and opioid use disorder. We summarized the association between NOU and 5 classes of cardiovascular disease, including infective endocarditis, coronary heart disease (including myocardial infarction), congestive heart failure, cardiac arrythmia (including cardiac arrest), and stroke. The most commonly studied outcomes were coronary heart disease and infective endocarditis. There was generally consistent evidence of a positive association between community prevalence of …

Contemporary research on informal caregiving recognizes that, though stressful, providing care for a disabled family member or friend can bring psychological benefits, such as increased meaning or purpose in life, growing closer to the care recipient, and learning new skills. Handgrip strength is a widely used measure of muscle strength and a predictor of a range of morbidities including cardiovascular diseases and all-cause mortality. Previous genome-wide association studies of handgrip strength have focused on common variants primarily in persons of European descent. We aimed to identify rare and ancestry-specific genetic variants associated with handgrip strength by conducting whole-genome sequence association analyses using 13,552 participants from six studies representing diverse population groups from the Trans-Omics in Precision Medicine (TOPMed) Program. By leveraging multiple handgrip strength measures performed in study participants over time, we increased our effective sample size by 7-12%. Single-variant …

Students enrolled in Advanced Placement (AP) courses and International Baccalaureate (IB) programs represent a unique group of adolescents given the high demands of their rigorous coursework and the elevated stress they experience compared to peers in the general education (Suldo & Shaunessy-Dedrick, 2013). These students are often missed in traditional screening procedures that tend to identify students struggling academically or exhibiting disruptive behaviors. Fortunately, Shaunessy-Dedrick and colleagues (2021) developed a comprehensive school-based intervention program, including universal (Tier 1) and selective (Tier 2) components, which aims to support the well-being of AP/IB students. Classical And Neural Network Machine Learning To Determine The Risk Of Marijuana Use, Laura Zoboroski, Torrey J. Wagner, Brent T. Langhals

Marijuana is the most commonly abused drug for military personnel tested at the Air Force Drug Testing Laboratory. A publicly available dataset of drug use, personality trait scores and demographic data was modeled with logistic regression, decision tree and neural network models to determine the extent to which marijuana use can be predicted using personality traits. While the logistic regression model had lower performance than the neural network model, it matched the sensitivity of prior work (0.80), achieved a high level of significance (p < 0.05) and yielded valuable inferences. It implied that younger, less educated individuals who exhibit sensation-seeking behavior and are open to experience tend to be at higher risk for THC use. A method for performing an iterative multidimensional neural network hyperparameter search is presented, and two iterations of a 6-dimensional search were performed. Metrics were used to select a family of 8 promising models from a cohort of 4600 models, and the best NN model’s 0.87 sensitivity improved upon the literature. The model met an f1 overfitting threshold on the test and holdout datasets, and an accuracy sensitivity analysis on a holdout-equivalent dataset yielded a 95% CI of 0.86 ± 0.04. These results have the potential to increase the efficacy of drug prevention and intervention programs.

The Human Immunodeficiency Virus (HIV) pandemic has taken the greatest toll on racial and ethnic minorities in the United States. Blacks and Latinxs suffer greater disease incidence, prevalence, morbidity and mortality from HIV as compared with Whites. Similarly, the Covid-19 pandemic caused by SARS-CoV-2 has disproportionately affected Blacks, Latinxs, and Native Americans causing higher rates of infection, more severe disease, and higher rates of mortality as compared with Whites. This paper seeks to evaluate the potential efficacy of a community gardening intervention on the Navajo Nation to increase gardening and healthy eating behaviors, which are potentially important in preventing obesity and related health conditions. Rates of obesity are high among American Indians, including those living on Navajo Nation land. Eating fresh fruits and vegetables is part of healthy eating. However, availability and access to fresh fruits and vegetables are severely limited on the Navajo Nation, due to distance and cost. Health disparities in African Americans is a persistent issue. Higher rates and severity of the novel coronavirus-19 [COVID-19] among African Americans only widens health disparities. Effective COVID-19 treatment options are imperative, requiring representation of African Americans in clinical research. However, low participation and under-representation of African Americans is complex in nature. This article describes health disparities, the impact of COVID-19, and participation in clinical research among African Americans. We offer strategies for researchers to enhance the inclusion of African Americans. We also offer strategies in conducting clinical research during COVID-19.

Objectives: This paper examines productive interactions—a cornerstone of the Chronic Care Model—between health care providers and vulnerable populations with chronic illnesses and/or disabilities. Methods: We conducted qualitative analysis of 16 focus groups and 29 interviews with patients and/or caregivers and 195 interviews with program leadership and providers across 15 Health Care Innovation Awards that targeted vulnerable populations. We analyzed how awardees addressed health concerns and social determinants of health (SDOH), and identified key components of productive interactions. Objectives: To investigate the relationship between insurance status and the risk of acute kidney failure (AKF) and consequential hospitalization outcomes.

Methods: A cross-sectional regression analysis was conducted for inpatients ages 18-64 in South Carolina 2012–2013. One dichotomous dependent variable - diagnosed with AKF at hospital admission, and two continuous dependent variables of hospital outcomes - total charge and length of stay, were examined. The key explanatory variable was the patient’s insurance status. Other covariates included patient’s age, gender, and race as well as AKF risk factors - Type 2 diabetic mellitus (T2DM), chronic kidney disease (CKD), hypertension, and proteinuria.

One of the prerequisite skills of effective public health and health practitioners is cultural competency. Cultural competency, however, requires a deep and profound understanding of individuals who are shaped by different life experiences than one’s own. Previous authors have described study abroad and service-learning as established strategies for enhancing cultural competency among emerging health professionals. The purpose of this study was to increase PPSV23 vaccination rates in adults over the age of 65 years in a Midwestern primary care clinic.

For this project, staff was educated regarding interventions including patient awareness, provider recommendation, staff and provider education, vaccine access, use of the immunization information systems (IIS) and appropriate use of electronic health record (EHR) reminders that have been shown to increase vaccination rates.

Introduction HIV transmission within serodifferent heterosexual couples plays a key role in sustaining the global HIV pandemic. In the USA, transmission within established mixed-status couples accounts for up to half of all new HIV infections among heterosexuals. Oral HIV pre-exposure prophylaxis (PrEP) is a highly effective prevention method, although underutilised among serodifferent couples. Achieving health equity has proven elusive for two reasons. First, most research has focused on changing the behavior of individuals; however, policies that address socioeconomic factors or change the context to facilitate healthy decisions tend to be more effective. Second, health disparity science and evidence are not consistently used to guide policymakers, even those seeking health equity. In this perspective, we discuss economic evaluation tools that researchers can use to assist decision-makers in conducting research or evaluating policy: self-reported health-related quality-of-life surveys and cost–benefit analysis evaluations informed with the willingness to pay for research and analyses.

Patients in rural areas are more likely to experience worse health outcomes than patients in urban or suburban areas. The reasons for this discrepancy are multi-faceted, including inequities in resources and access, as well as differences in the environment. Rural providers understand what education and resources patients are missing as they often interact with patients in multiple contexts. We interviewed rural providers and performed an inductive thematic content analysis of the interviews to shed some insight into what information rural patients would need to be better healthcare advocates for themselves.
