The Community Health System In Bangladesh: An Overview, Frontline Health Project

This brief provides an overview of the community health system in Bangladesh. Bangladesh is divided into eight administrative divisions, 64 districts, and 545 subdistricts, known as upazilas. Each rural area within an upazila is divided into union parishads and mouzas, which are further divided into villages. The Ministry of Health and Family Welfare has an extensive health infrastructure that is pluralistic and follows the country’s administrative pattern. Union parishads are the smallest administrative unit in rural areas. At the lowest administrative level, traditional healers, nonqualified allopathic practitioners, and community health workers (CHWs) are typically the only providers available. This brief …

Enabling women living with HIV to use contraception effectively can decrease unintended pregnancies and in turn reduce maternal mortality and vertical transmission of HIV. This brief describes the implementation process and intervention tested under the Evidence Project for delivering integrated FP and HIV services at the community level in Kenya. It outlines the implementation steps, best practices, and lessons learned of an intervention that was tested within the existing community health structure. Community health volunteers were trained to offer FP as part of their routine services to women living with HIV, and community health units were prepared to sustain those …

In Haiti, the health system is overseen at the national level by the Ministry of Public Health and Population (Ministère de la Santé Publique et de la Population-MSPP) which provides policies and standards for service delivery at all levels, including the community level. Health service delivery is managed and coordinated across three levels: national (level III), health department (level II), and district and community levels (level I). Statewide Dissemination Of An Evidenced-Based Email Walking Program Delivered Through Cooperative Extension, Elizabeth A. Richards, Stephanie Woodcox, Anna Forster

Using the RE-AIM framework, this study evaluates the statewide dissemination of an evidenced-based, email-delivered physical activity intervention implemented through Cooperative Extension. The Get WalkIN' program is comprised of 16 email messages sent over 12 weeks. Email messages target social cognitive theory constructs of self-efficacy, goal-setting, self-monitoring, and social support. Program reach, effectiveness, adoption, implementation, and maintenance were assessed with quantitative measures in a pre-post design. Findings indicate that within the Extension system, program adoption was high and program maintenance was moderate. Program effectiveness was demonstrated with an increase of 77.1 ± 49.9 min in weekly walking post-program. This increase in …

An important feature of COVID-19, the disease produced by the new coronavirus SARS-CoV-2, is the high number of health care workers (HCWs) that acquire the disease. In an initial report of 138 patients hospitalized with COVID-19 pneumonia in China, 40 patients (29%) were HCWs. One reason why HCWs are at higher risk of acquiring COVID-19 is that some patients with COVID-19 are admitted to the hospital without the classical presentation and are therefore not tested for the disease early during hospitalization. BACKGROUND: This case provides a rare case of

CASE: A 23-year old gravida 0 presented as a new patient to her gynecologist with complaints of irregular menses. A pelvic ultrasound showed ascites and the ovaries appeared heterogenous with irregular borders. A CA125 was 432. The patient was taken to the operating room by gynecologic oncology for a diagnostic laparoscopy. Biopsies were taken and final pathology resulted as "diffuse granulomatous inflammation." Post-operatively, the etiology remained unknown. The patient was brought back to the office for more testing. She tested positive for Chlamydia and was diagnosed with pelvic inflammatory disease.

Comorbidity associated with Alzheimer’s Disease (AD) is highly prevalent but largely understudied. In this study, we sought to determine 1) the prevalence of AD by race (White, African American, and Hispanics); 2) 20 chronic conditions’ commonly comorbid with AD and the prevalence, mortality rate, and health care expenditure of common AD and comorbid condition by race. A sample of 86,875 Florida 2010 Medicare beneficiaries aged 65 and older with 12 months of fee-for-service (FFS) enrollment were used in this study. With the support of medical professionals and non-restrictive advertisement laws, the widespread use of this deadly product exploded. The ‘Big Tobacco’ industry and the federal government made enormous amounts in profit and tax revenue. In 2011, 2.8 million of the 6.1 million pregnancies in the United States each year were unintended. They experience many safety and health hazards on the front-line that can lead to injuries, chronic illnesses, and even death. Chronic illnesses include cancer, heart diseases, and behavioral health issues that often cause first responders life expectancy to be significantly shortened.

According to the National Fire Protection Association, there were 64 on-duty firefighter deaths in 2018. Sudden cardiac death accounted for about 40% of the on-duty fatalities. The United States Department of Agriculture (USDA) outlines various methods and variables for defining food deserts, in attempts to standardize what constitutes a food desert or their characteristics. The USDA identifies the state of Nebraska as having both rural and urban food deserts, with an increase of food insecurity from 1.1% – 3.0% between 2007 and 2012 and warns of further increase of food deserts and its impact if measures are not taken. Increases in physical activity are associated with improvements in symptoms of mental illness and reduction in cardiometabolic risk. However, levels of life satisfaction, especially low levels of life satisfaction, can have a direct effect on health. Low levels of life satisfaction cause an increase in self-reported stress. hs-cTnT (high-sensitivity cardiac troponin T), but not NT-proBNP (N-terminal pro-B natriuretic peptide), has been shown to predict bleeding in patients with atrial fibrillation. Whether these biomarkers are independently associated with bleeding in the general population is unknown.

Methods and Results

We used Cox proportional hazards models to examine the association of hs‐cTnT and NT‐proBNP with incident bleeding (defined by International Classification of Diseases, Ninth Revision [ICD‐9] codes) among 9550 middle‐aged men and women without a history of cardiovascular disease or bleeding.
