Parents experience numerous stressors tied to their child's diagnosis as deaf or hard of hearing (DHH). This study sought to inquire about the lived experiences of parents with children who are DHH to inform the types of supports that should be provided within an audiology care coordination system. Semi-structured phone interviews were conducted with parents of children under the age of five who are DHH and patients of the Division of Audiology at Cincinnati Children’s Hospital Medical Center (CCHMC). Children who are born deaf/hard of hearing (D/HH) are at increased risk for delays in language, cognitive and social-emotional development. Early identification through screening and early intervention (EI) can improve outcomes for children who are D/HH. However, a need remains to evaluate the effectiveness and practices of statewide programs for children who are D/HH. The Ohio EHDI Data Linkage Project was created as a state-wide collaborative that included multiple Ohio government agencies and an academic institution. The most commonly used exposure assessment for heat stress is based on Wet Bulb Globe Temperature (WBGT), and the limits are based on empirical relationships that demonstrate a sustainable exposure. The ISO 7933 (2017) describes Predicted Heat Strain (PHS), which is a rational model for heat stress assessment that is used to assess time-limited exposures. Investigators have examined PHS validity under a variety of time-limited conditions. The purpose of this paper is to evaluate if PHS can predict sustainable exposures.

BACKGROUND: Adnexal torsion requires a high degree of suspicion in women presenting with pelvic pain. Polycystic ovaries are an infrequent cause of ovarian torsion but should be considered in cases without adnexal masses.

CASE: A 25-year-old woman had a delayed diagnosis of adnexal torsion due to polycystic ovaries despite typical presentation and imaging. A unique finding was ovarian fusion; separation was required in order to resolve the torsion. Oophoropexy was performed to prevent the recurrence of torsion.

CONCLUSION: Ovarian torsion should be suspected in the setting of abdominal pain, enlarged polycystic ovaries, and absent adnexal blood flow on ultrasound. This …

Observational studies have identified risk factors that have been helpful in lowering the death rate, including hypertension, high cholesterol, diabetes, smoking, physical inactivity and poor diet. The effects of these risk factors on CAD remain unclear. The number of people living with HIV (PLWH) ≥65 years old is increasing in the United States (US) as PLWH live longer. In 2015, there were nearly 1 million people living with diagnosed HIV in the US and under 10% were age ≥65. By 2035, the proportion of PLWH in this age group is projected to be 27%. Like the general population of elderly individuals, as they age, PLWH face age-related comorbidities, many of which require routine medical care and daily medications, in addition to daily antiretroviral therapy (ART) for treatment of HIV. The World Health Organization and the Center for Disease Control reports Chagas disease, or American trypanosomiasis, as a major neglected tropical disease prevalent in 21 endemic Latin American countries. The agent of the disease is a single-celled protozoan parasite, Trypanosoma cruzi, and common modes of transmission include infected feces of a triatomine bug, crossing the placental barrier, and blood products. Periodontitis is a chronic inflammation that destroys periodontal tissues caused by the accumulation of bacterial biofilms that can be affected by environmental factors. This report describes an association study to evaluate the relationship of environmental factors to the expression of periodontitis using the National Health and Nutrition Examination Study (NHANES) from 1999–2004. A wide range of environmental variables (156) were assessed in patients categorized for periodontitis (n = 8884). Multiple statistical approaches were used to explore this dataset and identify environmental variable patterns that enhanced or lowered the prevalence of periodontitis. In-ear dosimetry and noise exposure feedback were used to modify worker attitude and behavior regarding hearing protection use. The study specifically addressed whether providing in-ear noise exposure data to workers resulted in a reduction in average noise dose rate equivalent continuous levels. Nineteen combat arms instructors (impulse noise group) and heavy equipment operators (continuous noise group) working for the United States Air Force volunteered to participate in a six-month field study using in-ear dosimeters to collect daily noise level data. Participants served as their own control group, receiving periodic noise exposure feedback reports in the latter half of the study …

In 2013, over half of surveyed Sri Lankan men and women expressed gender-inequitable attitudes equating masculinity with violence, and femininity with obedience to men. Gender-inequitable attitudes have been shown to be linked to gender-based violence (GBV) in multiple contexts.
The goal of this research was to identify points of intervention at which programmes and policies could cultivate gender-equitable attitudes among youth in Sri Lanka, with a goal of reducing GBV in adulthood.

These challenges can present geographically and structurally within rural healthcare systems, as well as larger healthcare delivery entities. Due to these access barriers, rural Americans are at higher risk for developing poor health outcomes, as compared to their urban counterparts. Among these are higher mortality and morbidity rates, higher instances of chronic conditions, and higher rates of childhood obesity. OBJECTIVE: Early psychosis is an important window for establishing long-term trajectories. Involuntary hospitalization during this period may impact subsequent service engagement in people with newly diagnosed psychotic disorder. However, population-based studies of involuntary hospitalization in early psychosis are lacking. We sought to estimate the proportion of people aged 16 to 35 years with early psychosis in Ontario who are hospitalized involuntarily at first admission, and to identify the associated risk factors and outcomes.

Background: Socioeconomic status is associated with differences in risk factors for cardiovascular disease incidence and outcomes, including mortality. However, it is unclear whether the associations between cardiovascular disease and common measures of socioeconomic status-wealth and education-differ among high-income, middle-income, and low-income countries, and, if so, why these differences exist. We explored the association between education and household wealth and cardiovascular disease and mortality to assess which marker is the stronger predictor of outcomes, and examined whether any differences in cardiovascular disease by socioeconomic status parallel differences in risk factor levels or differences in management.

Background: Oral rapid HIV testing (ORHT) is implemented in the dental setting to make individuals aware of their possibly undiagnosed HIV infection. The testing process and characteristics of clients willing to receive ORHT has yet, however, to be systematically collected. Case description: Three dental hygiene clinics located in academic institutions implemented ORHT from March 2016 to April 2017. Results: 231 persons received ORHT; all had non-reactive results. This dissertation was designed to: (1) explore variations in clinicians’ views about patient self-management, (2) explore variation in patient activation and (3) explore variation in patients’ and their primary care physicians’ characteristics in the use of a patient portal and the main effects of patient activation and primary care physician support for patient self-management on portal use in an understudied closed multi-specialty ambulatory center. The main hypothesis is that primary care physician support for patient self-management will be an effect modifier on the association between patient activation and patient portal use.

Stigma is an established driver of population-level health outcomes. Antidiscrimination laws can generate or alleviate stigma and, thus, are a critical component in the study of improving population health.

Currently, antidiscrimination laws are often underenforced and are sometimes conceptualized by courts and lawmakers in ways that are too narrow to fully reach all forms of stigma and all individuals who are stigmatized.

To remedy these limitations, we propose the creation of a new population-level surveillance system of antidiscrimination law and its enforcement, a central body to enforce antidiscrimination laws, as well as a collaborative research initiative to enhance the study …

Interferon gamma has long been studied as a critical mediator of tumor immunity. In recent years, the complexity of cellular interactions that take place in the tumor microenvironment has become better appreciated in the context of immunotherapy. While checkpoint inhibitors have dramatically improved remission rates in cancer treatment, IFN-Γ and related effectors continue to be identified as strong predictors of treatment success. In this review, we provide an overview of the multiple immunosuppressive barriers that IFN-Γ has to overcome to eliminate tumors, and potential avenues for modulating the immune response in favor of tumor rejection.
