Background

Proton pump inhibitors (PPIs) are widely used drugs in the United States. Atrial fibrillation (AF) affects 2.7 million people in this country and often leads to other serious medical conditions. There is a growing body of evidence suggesting that PPIs may be proarrhythmic. This study seeks to better appreciate the relationship between these popular drugs and the most common arrhythmia.

Methods

Environmental toxicants can alter epigenetic regulatory features such as DNA methylation and microRNA expression. As the sensitivity of epigenomic regulatory features may be greatest during the in utero period, when critical windows are narrow, and when epigenomic profiles are being set, this review will highlight research focused on that period. I will focus on work in human populations, where the impact of environmental toxicants in utero, including cigarette smoke and toxic trace metals such as arsenic, mercury and manganese, on genome-wide, gene-specific DNA methylation has been assessed. The provision of any new prevention product in clinical trial settings is often vastly different from implementation in “real life” public sector settings. In addition, use of antiretrovirals (ARV), such as tenofovir gel, for pre-exposure prophylaxis requires regular HIV testing to ensure users are uninfected before initiating and continuing product use. Therefore, before any ARV-based prevention product can be introduced to women, information is needed on HIV testing practices, how best to integrate products into existing primary health care systems, and women’s risk perception and health-seeking behaviors. Epidemiologic surveillance indicates an increased susceptibility to obesity among children with special health care needs (SHCN) in comparison to children without. The present study investigated this disparity in weight severity between both groups, with a focus on the provision of obesity management counseling by physicians. This study consisted of a retrospective medical record review that acknowledged the effect of patient-provider interactions on health behaviors and risk perceptions. An independent sample t test compared the incidence of clinician-initiated obesity management counseling received by children with SHCN to that which was received by children without SHCN. This study investigates 1) the relationship between menthol cigarette smoking and obesity and 2) the association of body mass index with the nicotine metabolite ratio among menthol and non-menthol daily smokers aged 18–35 (n = 175). A brief survey on smoking and measures of height and weight, carbon monoxide, and saliva samples were collected from participants from May to December 2013 in Honolulu, Hawaii. Multiple regression was used to estimate differences in body mass index among menthol and non-menthol smokers and the association of menthol smoking with obesity. Diabetes is among the fastest growing diseases in the world. In the U.S., Hispanics are the third most affected population. Nonetheless, there are Hispanics diagnosed with diabetes who have, against all odds, found solutions to manage their disease. This study focused on the intrapersonal and interpersonal acts and behaviors of "positive deviant" Hispanics living on the U.S. - Mexico border that effectively managed their diabetes. They are referred to as "positive deviants" because they accrue "positive" outcomes and "deviants" because they are not the norm.

The number of adults aged 65 years and over has been rising quickly, as has the rate of new onset sexually transmitted diseases within this population. Discussions of sexual health between physicians and older adults are currently lacking in frequency and effectiveness. Using the foundation of stereotype embodiment theory, the purpose of this study was to identify the factors that predict frequency of discussing sexual health with older adult patients. End of life hospice services have consistently been underused by African Americans. This disproportionate use of hospice has contributed to poor quality of life and a lack of cost-effective care for patients with terminal illness. Driven by the theory of reasoned action, the purpose of this quantitative study was to test associations between African Americans' perceptions of hospice, decisions to use hospice, and religiosity. Avoidable hospitalizations are those hospitalizations that could have been avoided had there been appropriate levels of primary care, and higher levels of avoidable hospitalizations have been linked to higher rates of poverty and of race/ethnicity and to individuals who are on Medicaid or who are uninsured. The association with physician density has, however, been inconsistent, with research linking both better and worse physician density to higher rates of avoidable hospitalizations. Very little research, however, has been conducted on other physician characteristics and the rate of avoidable hospitalizations.

Using data from Behavioral Risk Factor Surveillance System (BRFSS), a telephone survey of adults in the United States, the first chapter of my dissertation studies the effect of state level and county level income inequality on health status and the pathway via public health spending. This investigation focused on a unique collection of archived human bones obtained post-mortem from 7 patients, who had received long-term parenteral nutrition (PN) for up to 21 years. Parenterals are administered intravenously as a nutritional supplement, and may be contaminated with metals, which are completely absorbed into the blood compartment. Some metal contaminants may be deposited in the body’s reservoir for inorganic elements – bone.

Prevalence of breastfeeding initiation in New York State (NYS) nearly meets Healthy People 2020 goals. Improvements in breastfeeding duration and exclusive breastfeeding (EBF) have lagged behind those achieved for initiation. Implementation of Ten Steps to Successful Breastfeeding in hospital maternity care has improved breastfeeding initiation, duration and exclusivity; but data on the independent association with specific Steps have been equivocal. Utilizing NYS-linked data from the Maternity Practices in Infant Nutrition and Care (mPINC) and the Pregnancy Risk Assessment Monitoring System (PRAMS), the associations of individual maternity care practices/Steps with breastfeeding behaviors were evaluated.
