Children With Special Health Care Needs: Minding The Gaps, Christie Provost Peters

National Health Policy Forum

This background paper examines the issue of children with special health care needs and their interaction with the health care system. Results from recent national and state surveys and studies were used to review the medical expenditures, utilization, and insurance coverage of these children. The paper also discusses weaknesses within the private and public delivery and financing systems that may hinder the access of certain families with children with special health care needs to important services.

Emerging Leaders Program Team Projects

The 2005 Project Child Health Impact Assessment (CHIA) at the Boston Medical Center (BMC) for the UMass Emerging Leaders Program (ELP) is closely related to one that members of the 2004 ELP cohort assisted with. The 2004 project team provided BMC with preliminary research regarding similar concepts in Europe, Canada and the United States. With this research in hand, the Project CHIA team at BMC decided to research, develop, write and publish pilot analyses that illustrated the impact of public policy on children's health. On January 31, 2004, a 45-year old female horse farm owner (decedent) died when she was pinned under a golf cart. The woman had driven a gas-powered golf cart from her house on the farm to the horse barn to muck stalls and put horses in the barn. After driving the golf cart into the barn, the farm owner placed two square bales of hay on the golf cart; one on the back and one on the passenger side of the front seat. This paper summarizes the methods and findings from two NIDA-supported studies of methamphetamine (MA) abuse in rural Ohio, Kentucky, and Arkansas, conducted by researchers at the Wright State University Boonshoft School of Medicine, the University of Arkansas for Medical Sciences, and the University of Kentucky. In addition, patterns of MA abuse among young adult MDMA/ecstasy users in urban central Ohio are described. Finally, preliminary findings from the Ohio Substance Abuse Monitoring Network on trends in methamphetamine abuse in Dayton, Ohio, are presented.

Corporate practices, such as advertising, public relations, lobbying, litigation, and sponsoring scientific research, have a significant impact on the health of the people in the United States. Recently, health professionals and advocates have created a new scope of practice that aims to modify corporate practices that harm health. This article describes how corporate policies influence health and reviews recent health campaigns aimed at changing corporate behavior in six industries selected for their central role in the U.S. economy and their influence on major causes of mortality and morbidity. These are the alcohol, automobile, food, gun, pharmaceutical, and tobacco industries. The …

Access to healthcare requires individuals to have a place to go, a healthcare provider to see them, resources to obtain proper care, and the appropriate health services given in an appropriate timeframe. Non-Hispanic Black American men have the highest incidence rate of prostate cancer in the world, 180.6 per 100,000 (NCI, 2004). It is not clear as to why high rates persist, but higher death rates are associated with late detection. Fetal alcohol exposure is a serious public health problem and is 100% preventable. Traditionally, FASD prevention programs and research recommendations have targeted peri-conceptual or pregnant women. Another approach to the prevention of FASD involves preventing pregnancy in women who use alcohol. The purpose of this cross sectional study was to identify factors that predicted women’s intention to use contraception using the theory of planned behavior (TPB), to determine if predictors of the theoretical constructs varied in women depending on alcohol use, and to examine interactions between alcohol use and TPB constructs. To evaluate how life expectancy changed by age and secular time from 1960 through 1988, two cohort studies, the Adventist Mortality Study (AMS, 1960-1985, n = 27,530) and the Adventist Health Study (AHS, 1976-1988, n = 34,192) were combined. The life expectancy of omnivores was compared to vegetarians separately in never-smoking males and females for all-cause mortality, and where the cause of death was cardiovascular disease or all site-cancer. Life expectancies were calculated for subjects who had survived to ages 30, 50, 70, and 80 during six calendar periods: 1960- 1964, 1965-1969, 1970-1974, 1975-1979, 1980-1984, and 1985-1988. Breast cancer is a debilitating and many times fatal disease that will affect approximately 215,990 women in the United States alone this year. Treatment for breast cancer can involve many physically and psychologically straining features. The illness perception theory states that individuals form illness representations to make sense of health threats and illness. These representations contain a number of individual, specific attributes about the illness identity, and cause, time-line, consequences of, and cure/control of the illness. Youth and young adults who are injection drug users (IDU’s) often live and survive in physical and emotional environments that are associated with negative behaviors and outcomes. Past environmental and social factors, such as participation in social welfare systems or institutionalization in foster care or juvenile hall, have been found to be associated with IDU-involved young adults’ health behaviors when they become older. Social networks, which include drug, sex, or hangout networks, may influence their health outcomes and behaviors. History:

Cancer registration in Louisiana began in 1947 at the Charity Hospital Tumor Registry in New Orleans. In 1974, as part of its Surveillance, Epidemiology and End Results (SEER) Program, the National Cancer Institute provided funds for a population-based cancer incidence and survival registry covering Jefferson, Orleans, and St. Bernard parishes. The purpose of this study was to determine the effects of long term walnut consumption on plasma levels of tocopherols and the inflammatory mediators interleukin-1β (IL-1β ), interleukin-6 (IL-6), tumor necrosis factor-a (TNF-a) and Creactive protein (CRP) in healthy participants. Ninety participants were recruited into a 12-month randomized crossover intervention study, which consisted of two phases, the walnut supplemented diet and the habitual diet. Participants were randomly assigned to either of these two phases for six months and then switched to the other phase for the second six months. As an epidemiological parameter, the population attributable fraction is an important measure to quantify the public health attributable risk of an exposure to morbidity and mortality. In this article, we extend this parameter to the attributable fraction function in survival analysis of time-to-event outcomes, and further establish its estimation and inference procedures based on the widely used proportional hazards models. Numerical examples and simulations studies are presented to validate and demonstrate the proposed methods.

Female breast cancer incidence rates in Utah are among the lowest in the U.S. The influence of the Church of Jesus Christ of Latter-day Saint (LDS or Mormon) religion on these rates, as well as on disease-specific survival, will be explored for individuals diagnosed with breast cancer in Utah from 1985 through 1999. Methods: Population-based records for incident female breast cancer patients were linked with membership records from the LDS Church to determine religious affiliation and, for LDS Church members, level of religiosity. On Monday, March 31, 2003, a 39-year-old male (decedent) highway traffic technician, working alone, received fatal head injuries after falling out of the bucket of an aerial boom truck. Injuries sustained in the fall resulted in his death four days later. He was changing a burned-out green traffic bulb in a traffic signal suspended over 2 lanes of traffic. As he worked from the aerial lift suspended over traffic to change the burned out bulb, a cargo truck drove underneath the bucket, striking it, and causing the decedent to fall to the asphalt below.
