Patient-Centered Medical Homes And Hospital Value-Based Purchasing: Investigating Provider Responses To Incentives, Lauryn Walker

Provider incentives are a commonly used policy tool to mold provider behaviors.¹ However, while we frequently measure the change in patient outcomes, failure to consistently produce changes in outcomes does not mean that providers are not changing their behavior. This paper focuses on two programs with null or inconsistent quality outcomes to try to identify why such inconsistency occurs. The two programs, both ratified in the Affordable Care Act, are 1) patient-centered medical homes (PCMHs), and 2) the Medicare Hospital Value-Based Purchasing (HVBP) program.

A popular model for causal inference is the Rubin Causal Model (RCM), which typically seeks to estimate the average difference in study units' potential outcomes. If the exposure Z is binary, then we may express this as E[Y(Z=1)-Y(Z=0)]. An important assumption under RCM is no interference; that is, the potential outcomes of one unit are not affected by the exposure status of other units. African American women have the highest rates of HIV infection among women of all racial/ethnic groups in the United States, and over 50% of HIV infected young adults are unaware of their infection. In today’s economic, social, and political climate, farmers are exposed to situations and stressors reminiscent of the 1980s Farm Crisis. An added risk is the aging workforce of farmers, as age-related conditions can make farming even riskier. In this study, Naegleria fowleri, a pathogenic amoeba and the causative agent of Primary Amebic Meningoencephalitis (PAM), was utilized to determine the presence or absence of classically conserved Golgi molecules featured in the expression of a Golgi apparatus. Previous studies concluded no Golgi expression via light microscopy and transmission electron microscopy, but a recent report on Naegleria gruberi indicated the presence of dispersed Golgi tubules. My prior work on Chinese undergraduates indicates a high prevalence of lifetime NMUPO (49.2%). Objective: The purpose of this study was to determine whether hospital practices are associated with breastfeeding initiation and breastfeeding duration for ≥10 weeks.

Methods: We analyzed the 2013-2015 South Carolina Pregnancy Risk Assessment Monitoring System (PRAMS) data. The population of interest was mothers who delivered a single live birth in the hospital. The infant was alive and living with the mother at the time of the survey. The purpose of the study was to examine factors predictive of a more positive perceptions following a hookup. The participants included 188 students from Illinois State University. Participants completed a demographics questionnaire, the Hookup Scale, the Rosenberg Self-Esteem Scale, the Sexual Self-Esteem Inventory, religiosity questions, attitudes toward hooking up questionnaire, emotional reactions to hooking up questionnaire, and the Social Academic, Romantic, and Sexual Hooking Up Reactions Scale. The current study found that participants who reported high global self-esteem, high sexual self-esteem, and positive attitudes toward hooking up reported less regret, more positive reactions, and fewer negative reactions following a hookup.

This research investigated the germination and outgrowth of Bacillus globigii in the presence of ammonia in laboratory scale experiments. Germination was measured by monitoring the release of dipicolinic acid, an organic compound present in bacterial spores, while outgrowth was measured using phase-bright microscopy and semi-automated counting procedures. The ammonia-N concentrations investigated generally did not cause statistically significant differences in the initial 1-hr germination rates or the average 3-hr outgrowth rates of Bacillus globigii spores in batch style experiments. Pictorial health warning labels (HWLs) on cigarette packs are used worldwide to curb cigarette smoking, the leading preventable cause of death in the United States While individual-level mediators of HWLs’ effects on cessation have been thoroughly studied, interpersonal communication about HWLs has received less attention in HWL research. However, results from one study demonstrated the importance of interpersonal communication, finding that conversations about HWLs predicted quit attempts in Australia, Canada, and Mexico. Bronchiolitis is prevalent among infants of 0 to 12 months, and usually caused by a variation of viruses. Signs and symptoms consist of coughing, rapid breathing, and an occasional fever in some infants. Bronchiolitis is not treated with antibiotics, but rather with proper medical examination and other procedures. Chest radiographs, steroids, and certain bronchodilators are considered ineffective treatments. This study identified the amount of ineffective procedures being used in infants admitted into the pediatric emergency room, and the effect it had on the patient’s average hospital stay.
